Given the diverse clinical progression of prostate cancer (PC) and the evolving significance of histopathological factors in its management, this study aimed to explore the impact of cribriform ...pattern 4 (CP4) on clinical outcomes in PC patients and examine its molecular characteristics.
This retrospective study analyzed data from The Cancer Genome Atlas (TCGA) database and included PC patients who underwent radical prostatectomy (RP) and had pathology slides available for the assessment of CP4. A multivariable competing risk regression analysis was used to assess the association between CP4 and progression-free survival (PFS) while adjusting for established PC prognostic factors. The frequency of genomic alterations was compared between patients with and without CP4 using the Fisher's exact test.
Among the 394 patients analyzed, 129 (32.74%) had CP4. After a median follow-up of 40.50 months (IQR: 23.90, 65.60), the presence of CP4 was significantly associated with lower PFS (AHR, 1.84; 95% CI, 1.08 to 3.114;
= 0.023) after adjusting for covariates. Seven hub genes-KRT13, KRT5, KRT15, COL17A1, KRT14, KRT16, and TP63-had significantly lower mRNA expression levels in patients with CP4 compared to those without.
PC patients with CP4 have distinct genomic alterations and are at a high risk of disease progression following RP. Therefore, these patients may benefit from additional post-RP treatments and should be the subject of a prospective randomized clinical trial.

There is great evidence in recent years that oxygen free radicals play an important role in the pathophysiology of many neuropsychiatric disorders. The present study was performed to assess the ...changes in red blood cells thiobarbituric acid-reactive substances (TBARS) levels, and superoxide dismutase (SOD), catalase (CAT), adenosine deaminase (ADA) and xanthine oxidase (XO) activities in patients with autism (n = 27) compared to age- and sex-matched normal controls (n = 26). In the autistic group, increased TBARS levels (p < 0.001) and XO (p < 0.001) and SOD (p < 0.001) activity, decreased CAT (p < 0.001) activity and unchanged ADA activity were detected. It is proposed that antioxidant status may be changed in autism and this new situation may induce lipid peroxidation. These findings indicated a possible role of increased oxidative stress and altered enzymatic antioxidants, both of which may be relevant to the pathophysiology of autism.

Patients with isolated CAE were detected to have significantly higher levels of plasma soluble ICAM-1, VCAM-1 and E-selectin in comparison with patients with obstructive coronary artery disease without CAE (ICAM, 673±153 versus 381±106, respectively, P<0.001; VCAM-1, 2366±925 versus 1136±208, respectively, P<0.001; E-selectin, 74±21 versus 61±18, respectively, P=0.01) and subjects with normal coronary arteries (ICAM-1, 673±153 versus 303±131, respectively, P<0.001; VCAM-1, 2366±925 versus 729±231, respectively, P<0.001; E-selectin, 74±21 versus 49±9, respectively, P<0.001), suggesting the presence of a more severe and extensive chronic inflammation in the coronary circulation in patients with isolated CAE.
BACKGROUNDThe common coexistence of coronary artery ectasia (CAE) with coronary artery disease (CAD) suggests that it may be a variant of CAD. However, it is not clear why some patients with obstructive CAD develop CAE whereas most do not. İnflammation has been reported to be a major contributing factor to both obstructive and aneurysmatic vascular disorders and therefore, in the present study, the plasma soluble adhesion molecules, intercellular adhesion molecule-1 (ICAM-1), vascular cell adhesion molecule-1 (VCAM-1) and E-selectin levels in isolated CAE were investigated.
METHODSThe study population consisted of three groupsthe first consisted of 32 patients with isolated CAE without stenotic lesion; the second of 32 patients with obstructive CAD without CAE; and the third group of 30 control subjects with normal coronary arteries. Coronary diameters were measured as the maximum diameter of the ectasic segment by use of a computerized quantitative coronary angiography analysis system. According to the angiographic definition used in the Coronary Artery Surgery Study, a vessel is considered to be ectasic when its diameter is ≥1.5 times that of the adjacent normal segment in segmental ectasia. Plasma soluble ICAM-1, VCAM-1 and E-selectin levels were measured in all patients and control subjects using commercially available enzyme-linked immunosorbent assay kits.
RESULTSPatients with isolated CAE were found to have significantly higher levels of plasma soluble ICAM-1, VCAM-1, and E-selectin in comparison with patients with obstructive CAD without CAE (ICAM, 673±153 versus 381±106, respectively; P<0.001; VCAM-1, 2366±925 versus 1136±208, respectively; P<0.001; E-selectin, 74±21 versus 61±18, respectively; P=0.01) and control subjects with normal coronary arteries (ICAM-1, 673±153 versus 303±131, respectively;, P<0.001; VCAM-1, 2366±925 versus 729±231, respectively; P<0.001; E-selectin, 74±21 versus 49±9, respectively; P<0.001). In addition, we detected statistically significant positive correlation between the total length of ectasic segments and the levels of plasma soluble ICAM-1 (r=0.625; P<0.001), VCAM-1 (r=0.548; P=0.001) and E-selectin (r=0.390; P=0.027). Multivariate logistic regression analysis revealed a significant independent relation between isolated CAE and ICAM-1 odds ratio (OR)=1.023; 95% confidence interval (CI)=1.0048–1.0414; P=0.0129 and VCAM-1 (OR=1.0057; 95% CI=1.0007–1.0106; P=0.0240).
CONCLUSIONSWe have shown that patients with isolated CAE have raised levels of plasma soluble ICAM-1, VCAM-1 and E-selectin in comparison with patients with obstructive CAD without CAE and control subjects with normal coronary arteries, suggesting the presence of a more severe and extensive chronic inflammation in the coronary circulation in these patients.
Machining has considerably increased with evolving technology and increasing demand in natural stone production facilities. Different types of accidents may occur in natural stone facilities during ...movement, dimensioning, cutting of blocks and surface processing. These accidents may be due to physical, chemical, ergonomic and mechanical conditions. Therefore, possible work accidents and occupational diseases should be investigated. In this study, an L-matrix analysis is conducted to analyze hazards and forecast risks in natural stone facilities. According to the L-matrix results, three major initiating events are identified. For each of these initiating events, event tree analysis is used to calculate risk scores. These initiating events are hoist rope fatigue and breaking, diamond wire fatigue and breaking, and electrical leakage due to old systems. These events and their results are classified according to the probabilities using event tree analysis.

This review aims to provide a comprehensive overview of the literature on the use of exercise for migraine treatment with regard to its efficacy, mechanism of action and role in practice. Many ...randomized studies have reported the efficacy of prophylactic treatment of migrane with medications such as beta blockers or antiepileptic drugs. Studies on alternative approaches, like aerobic exercise and biofeedback, are however limited but also considered to be effective. Scientific databases were searched with keywords "exercise" and "migraine". The resulting publications were gathered, examined and discussed throughly. Past studies had limitations and were few in number, but more recent randomized controlled studies have concretely provided level of evidence about the effectiveness of exercise in prophylactic treatment of migraine. Core properties of exercise like intensity, duration, frequency, type and warming up period are required to be monitored while treating migraine to increase the beneficial effects and, also to prevent injuries and side effects which may include exertional headache. Isometric neck exercise is helpful when the migraine is accompanied by neck pain. Patient population with low beta endorphin level in blood, high physical fitness and high motivation receives significant benefits from the exercise treatment. The action of exercise on migraine is in general related to neurochemical factors, psychological states and increase in cardivascular and cerebrovascular fitness. Considering its effectiveness and minimal side effects, migraine patients should often be encouraged to practice physical exercise with intensity, frequency and duration that should be carefully instituted to achieve the most beneficial outcome while preventing potential injuries and side effects.
Neurofibromatosis (NF) is a hereditary disease and carries increased risk of both benign and malignant tumor development. Pheochromocytoma or hyperparathyroidism have been reported to be associated ...with NF type 1 (NF1). However, the coexistance of pheochromocytoma and parathyroid adenoma in a patient with NF1 is very rare. We report a case of a 37-year-old male with NF1, bilateral pheochromocytoma and parathyroid adenoma. This association sould be kept in mind in patients with NF1 in initial evaluation as well as during follow-up.
Observation of charmonium like new states since 2003, which do not fit into conventional quark model renewed the interest in exotic states. Many models like meson molecule, tetraquark, and ...quark-gluon hybrid are proposed to explain the observed charmonium spectroscopy. The observation of Y(3940)1 near the J/psi omega threshold motivated the searches for similar structures near J/psivarphi threshold. In this proceeding, we summarize the recent developments on the observation of a peaking structure in the J/psivarphi mass spectrum from B super(+ or -) arrow right J/psivarphiK super(+ or -) decay.
