Echocardiographic measurement of left ventricular (LV) mass is routinely performed in pediatric patients with elevated cardiovascular risk. The complex relationship between heart growth and body ...growth in children requires normalization of LV mass to determine its appropriateness relative to body size. LV mass is strongly determined by lean body mass (LBM). Using new LBM predictive equations, the investigators generated sex-specific LV mass-for-LBM centile curves for children 5 to 18 years of age.
This retrospective study used M-mode echocardiographic data collected from 1995 through 2003 from 939 boys and 771 girls between 5 and 18 years of age (body mass index < 85th percentile for sex and age) to create smoothed sex-specific LV mass-for-LBM reference centile curves using the Lamda Mu Sigma method. The newly developed reference centiles were applied to children with essential hypertension and with chronic kidney disease, groups known to be at high risk for LV hypertrophy (LVH). The identification of LVH using two different normalization approaches was compared: LV mass-for-LBM and LV mass index-for-age percentiles.
Among 231 children at risk for LVH, on average, relative LV mass was higher using the LV mass index-for-age percentile method than the LV mass-for-LBM percentile method. LVH was more likely to be diagnosed among overweight children and less likely among thin children.
This study provides new LV mass reference centiles expressing LV mass relative to LBM, the strongest determinant of LV mass. These reference centiles may allow more accurate stratification of cardiovascular risk in children.
Pulmonary arterial capacitance (PAC) has been associated with right ventricular (RV) workload and mortality in pulmonary hypertension, but is not routinely evaluated in children. We investigated ...whether PAC can be estimated noninvasively by echocardiography.
We retrospectively determined PAC in 31 children with pulmonary hypertension, using echocardiography, and compared the results with those obtained at cardiac catheterization.
PAC derived from echocardiography was similar to that derived from catheterization (mean +/- SD 1.16 +/- 1.0 vs 1.10 +/- 0.95 mL(3) x mm Hg(-1), P = not significant) and the two correlated well (r = 0.74, P < .0001). There was a highly significant polynomial relationship between PAC and RV anterior wall thickness indexed to body surface area (R(2) = 0.54, P < .0001), but not between pulmonary vascular resistance and RV wall thickness. Pulmonary vascular resistance and PAC did not correlate.
Echocardiography can reliably estimate PAC, which strongly correlates with RV hypertrophy, a surrogate for RV work.
We hypothesized that Doppler tissue imaging in the short axis would provide enhanced quantitative information for differentiating the pattern and extent of abnormal septal and posterior wall motion ...in constrictive pericarditis (CP). Using quantitative pulsed wave and color M-mode Doppler tissue imaging, we quantified the pattern of abnormal septal and posterior wall motion and studied its incremental advantage over conventional M-mode and 2-dimensional echocardiography in 40 patients with surgically proven CP. The pattern and extent of abnormalities were compared with 35 age- and sex-matched control subjects and 20 patients with abnormal septal motion of other causes. In 33 patients (82.5%) with CP, the interventricular septum showed high-velocity (>7 cm/s) early diastolic biphasic motion with or without multiple recoil waves (polyphasic diastolic septal fluttering). In the posterior wall, the early diastolic wave was normal but the late diastolic wave was reduced in 24 patients (60%) and absent in 7 (17.5%). In comparison, M-mode and 2-dimensional echocardiography identified abnormal septal or posterior wall motion in 24 patients (60%) ( P = .003). The pattern of abnormal septal motion in CP could be differentiated from abnormal septal motion of other causes in 16 patients (80%). The overall sensitivity and specificity of high-velocity polyphasic septal flutter for differentiating CP from control cases and other diseases was 82.5% and 92.7%, respectively. In CP, Doppler tissue imaging in the short axis provides unique diagnostic information and reliably differentiates CP from control cases and most other causes of abnormal septal motion.
Platypnea-orthodeoxia is a rare syndrome that is often associated with interatrial shunting through a patent foramen ovale or atrial septal defect. We describe the case of a 65-year-old woman with ...progressive dyspnea and hypoxia when standing and walking, which was relieved by assuming the recumbent position. The diagnosis was confirmed by tilt-table transesophageal echocardiography demonstrating a large right-to-left shunt through a patent foramen ovale while the patient was in a semiupright position and no significant shunt while in a recumbent position. This case demonstrates that platypnea-orthodeoxia caused by a patent foramen ovale can be clearly demonstrated by the technique of contrast transesophageal echocardiography performed on a tilt table.
Left atrial stunning after cardioversion is a well-known phenomenon. It has been associated with higher risk of postcardioversion thromboemboli and increased risk of recurrence of atrial ...fibrillation. We present a case of differential atrial stunning after electrical cardioversion for atrial fibrillation. Diagnosis was made by pulsed wave Doppler of mitral, tricuspid, and pulmonary vein inflow and mitral and tricuspid annuli. Differential mechanical atrial stunning may be a common phenomenon after cardioversion and may suggest difference in right and left atrial transport function. Its prevalence needs to be determined by a large study. Doppler tissue imaging might be routinely used in patients after cardioversion for atrial fibrillation to detect atrial stunning.
Cardiac fungal infections have become more prevalent and are being diagnosed with increasing frequency. The most common infective organism is Candida albicans, followed by Aspergillus fumigatus and ...Cryptococcus. Cardiac involvement is usually associated with endocarditis, myocarditis, pericarditis, or intracardiac fungal mass. Early diagnosis is imperative, as these patients have poor outcome once there is cardiac involvement. In this report we describe a patient in whom an intracardiac mass was detected with transesophageal echocardiography and confirmed to be aspergillus fungal ball at surgery.
Two-dimensional echocardiography provides valuable information for the assessment of left ventricular function. Traditionally, evaluation has focused on determination of systolic performance. ...However, recent investigations indicate diastolic dysfunction may also contribute to symptoms of congestive heart failure in many patients despite normal systolic function. Pulsed Doppler echocardiography complements two-dimensional imaging for assessment of left ventricular filling properties that are often altered in the setting of diastolic dysfunction. The concept of diastolic function and recognition of abnormal filling patterns detected by pulsed Doppler echocardiography are reviewed.
