Amaç: Bu çalısmada, oral koroziv madde alımı ile acil servise basvuran hastaların demografik özelliklerinin, mortalite ve morbidite oranlarının açığa çıkarılmasını amaçladık.
Materyal ve Metod: Oral ...koroziv madde alımı nedeniyle 01.01.2009 ile 31.12.2010 tarihleri arasında başvuran vakalar geriye dönük incelendi. Hastaların yaşı, cinsiyeti, mesleği, alınan koroziv maddenin cinsi, alım nedeni (kazara, özkıyım), acile başvuru sekli, alım ile acile ilk başvuru arasında geçen süre, başvuru sırasındaki şikâyetleri kaydedildi. Endoskopisi yapılan ve yapılmayan hastaların tanı ve tedavi süreçleri, hastanede yatış süresi ve hasta sonlanım şekli ile ilgili veriler kaydedildi. Tanı sürecinde endoskopi bulguları Di Costanzo Evreleme Sistemi’ne göre sınıflandırıldı. Bulgular: Çalışmaya 59 hasta (30 kadın, 29 erkek) dahil edildi. Olguların yaş ortalaması 38.67±18.56 idi. Koroziv madde alımının % 74.58'i kazara, % 25.42'si özkıyım amaçlı idi. Alınan korozivlerin % 55,93'ü sodyum hipoklorit idi. Hastaların % 95'ine ilk 24 saat içinde endoskopi uygulaması yapıldığı görüldü. Endoskopi sonrası en sık rastlanan hasarlanma evre 2a (% 31) idi. Acil servise başvuran hastaların % 57.6'sı hastaneye yatırıldı. Yoğun bakımda takip edilen bir hasta eksitus oldu.
Sonuç: Koroziv madde alımları morbidite ve mortaliteye yol açabilen zehirlenmelerdir. Zehirlenmelerin en aza indirilmesi için toplumun eğitilmesi, ambalajsız temizlik maddesi satışının yasaklanması, temizlik maddelerinin açıkta bırakılmaması ve başka kaplarda saklanmaması gerekmektedir.
Introduction: In this study, we aimed to compare Carbon-14 Urea Breath Test (C-14 UBT) and endoscopy in diagnosis of Helicobacter pylori (HP) and show the availability of the C-14 UBT in follow-up. ...Methods: We enrolled 183 patients who were admitted to Dumlupınar University Faculty of Medicine, Kütahya Evliya Çelebi Education and Research Hospital between April 2012-February 2015 with dyspeptic complaints and C-14 UBT performed due to the suspicion of HP infection. Patient records were analyzed retrospectively. Results: We enrolled 183 patients with age range 7-71 years. The mean age was 31 ± 15 years and 129 of patients were female (7%), and 54 were male (30%). According to the results of C-14 UBT, HP was negative in 77 of patients (42%), HP was probable in 2 of patients (1%) and HP was positive in 104 patients (57%). There was no significant difference between HP positive and HP negative groups in terms of age and gender. When compared the results of C-14 UBT with upper gastrointestinal system endoscopy, it was found that there was no difference between the two methods for the determination of the HP positivity (p gt;0.05). Conclusion: C-14 UBT is a reliable, noninvasive and practical test which can be used in the diagnosis of HP.
Surgeons have been using sutures made up of different materials to reunite disrupted tissues, throughout centuries. But these materials may have certain disadvantages like foreign body reaction, ...infection, scarring, need for removal and cost. Their application is time-consuming and needs skill and experience. Whereas tissue adhesives is one of alternative to conventional suturing and has some additional advantages. The use of fibrin glue, as in concentrated human fibrinogen in combination with thrombin for hemostasis and tissue adhesion has been an important advance in a variety of surgical applications. The purpose of this study is to share our experience about fibrin glue.
Yüzyıllardır cerrahlar dokuları onarmak için farklı malzemelerden yapılmış sütürleri kullanmaktadırlar. Ancak, bu materyallerin yabancı cisim reaksiyonu ve iltihap gibi bazı dezavantajları mevcuttur. Bunlarla birlikte, operasyon sahasında skar dokusu oluşturdukları gibi maliyeti de artırmaktadırlar. Diğer bir yandan, bu yöntemlerle onarım beceri, tecrübe ve daha fazla zaman gerektirmektedir. Oysaki doku yapıştırıcıları konvansiyonel yöntemlere bir alternatif olup bazı avantajları da mevcuttur. Trombin ile kombine halde yoğunlaştırılmış haldeki insan fibrinojeni olarak fibrin yapıştırıcı kullanımı, hemostaz ve dokuların yapışmasında önemli avantajlar sağlamaktadır. Bu çalışmamızdaki amacımız, fibrin glue ile ilgili deneyimimizi paylaşmaktır.
