Prikazujemo 58-godišnjeg bolesnika s prirođenom hipoplazijom donjeg dijela lijevoga m. psoasa. Ta rijetka anomalija, koju treba razlikovati od drugih patoloških stanja (destruktivni procesi; tumori; ...metastaze; inaktivitetna atrofija), otkrivena je tijekom MR pretrage lumbosakralne (LS) kralježnice kojoj se bolesnik podvrgao zbog pogoršanja kronične boli u križima. Desni m. psoas bio je hipertrofičan. CT abdomena pokazao je istovjetan nalaz. MR pretraga LS kralježnice pokazala je jake i opsežne degenerativne promjene. Osim hipoplazije donjeg dijela lijevoga m. psoasa, radiološke pretrage LS kralježnice (MR) i abdomena (CT) nisu pokazale druge prirođene anomalije. Uzimajući u obzir anamnestičke podatke, klinički nalaz, rezultate radioloških i drugih pretraga (EMNG), kao i poznate činjenice o anatomiji i funkciji m. psoasa (stabilizator lumbalne kralježnice, zdjelice i kuka; fleksor kuka i trupa; lateralni fleksor trupa), u ovome prikazu slučaja analizirali smo kliničke manifestacije (postranična devijacija lumbalne kralježnice; asimetrija zdjelice – torzija zdjelice; povećana ekstenzija kuka na strani hipoplastičnog m. psoasa) i kasne posljedice unilateralne hipoplazije m. psoasa (unilateralna hipoplazija m. psoasa preopterećenje LS kralježnice i drugoga m. psoasa kronična bol u križima kompenzatorna hipertrofija drugoga m. psoasa jake i opsežne degenerativne promjene LS kralježnice).
Spinalna bol, a napose kronična križobolja jedan je od najvažnijih zdravstvenih i javnozdravstvenih problema. Bolje razumijevanje patofiziologije boli i preciznija vizualizacija primjenom suvremenih ...slikovnih metoda pomažu nam u usmjeravanju terapije i postizanju boljeg ishoda. U većine pacijenata s vertebrološkom problematikom simptomi se smanjuju, a funkcija oporavlja primjenom metoda neoperativnog liječenja. U ovom radu izvješćuje se o novostima u patofiziologiji spinalne boli, dijagnostičkim mogućnostima magnetske rezonancije i mogućnostima konzervativnog liječenja (lijekovi, terapijske vježbe, fizikalna terapija, ortoze) u bolnim stanjima kralježnice i u skoliozama. Osim što je informativan, zajedno s člankom o novostima u invazivnom/operativnom liječenju, ovaj tematski članak može pomoći u pristupu bolesnicima s tim stanjima.
U članku su obrađene novosti i perspektive u invazivnom liječenju u vertebrologiji s naglaskom na interventne i poluinvazivne postupke te minimalno invazivnu kirurgiju u liječenju hernije ...intervertebralnog diska slabinske kralježnice. Osim toga opisane su novosti u neuroablativnim postupcima u liječenju križobolje, fiksaciji kralježnice i operativnom liječenju skolioza. Članak se osvrće i na metode koje se još ne primjenjuju kod nas, a koje dosta obećavaju kao što su rekonstrukcije nukleusa pulpozusa transplantacijom autolognih hondrocita, a prikazane su najnovije spoznaje u kirurgiji vratne kralježnice. Upozoreno je na uspješnost svake pojedine kirurške metode liječenja. Osim što je informativan, zajedno s odgovarajućim člankom o patofiziologiji spinalne boli, magnetskoj rezonanciji i konzervativnom liječenju ovaj tematski članak o najnovijim spoznajama u tom području može pomoći u pristupu bolesnicima.
Une méthode non intrusive de construction d’un modèle de remplacement de l’écoulement dans une cavité de système d’air secondaire de turboréacteur est recherchée. Le modèle réduit doit pouvoir être ...intégré dans un modèle de l’ensemble du moteur et couplé à la thermique de la structure pour simuler son comportement thermique sur une mission complète sous aile. Pour cela, il doit prendre un grand nombre de paramètres en entrée, retourner autant de sorties et être utilisable sur des intervalles de variations étendus de ces paramètres. Plusieurs approches sont envisagées et implémentées, puis appliquées à la modélisation d’une cavité sous turbine fictive :— Création de surfaces de réponse des termes de la décomposition ANOVA des flux pariétaux.— Création de surfaces de réponse des flux pariétaux combinée avec une méthode de raffinement adaptative exploitant la trajectoire dans l’espace d’entrée issue du couplage modèle réduit - modèle de structure.— Réduction de dimension des champs d’interface échangés à partir de résultats des itérations du couplage des modèles thermiques de l’écoulement et de la structure, puis création de surfaces de réponse des coordonnées réduites.Cette dernière voie permets d'obtenir des résultats encourageants sur le cas test proposé d'abord dans le cas à conditions limites d'entrée de l'écoulement fixées, puis en incluant des variations de certaines d'entre-elles.
A non intrusive method to create surrogate models describing the flow in jet engines’ secondary air system is desired. The resulting model must be integrated in a thermal model describing the whole engine during a complete mission under the wing. This requires the model to use a high number of input and output parameters and to be valid on a broad domain of variation of its parameters. Several approches are explored in this thesis and applied to a simplified turbine cavity :— Surrogate modeling of terms of the ANOVA decomposition of wall fluxes.— Surrogate modeling of wall fluxes combined with an adaptive refinement method exploiting the trajectory followed by the input parameters during the coupling between the metamodel and the structural model.— Dimensionality reduction of the interface data exchanged during the coupling between flow and structure thermal model and surrogate modeling of the resulting reduced coordinate.This last approach leads to good results on the test case considered in this thesis with fixed inlet boundary conditions and then with variations of some of the inlet parameters.
Objective: The objective of this study was to evaluate in vitro apical microleakage of root canal filling materials for primary teeth.Methods: Fifty extracted primary anterior teeth were distributed ...into 6 groups: 4 experimental groups; 1 negative control group; and
1 positive control group. Zinc oxide and eugenol (ZOE), Kri paste, calcium hydroxide (Ca(OH)2), and Vitapex were inserted into the canal using a lentulo spiral mounted in a slow-speed handpiece. All teeth were then subjected to a dye leakage test using 2% methylene blue as the tracer.Results:
The highest dye penetration was recorded for teeth filled with ZOE, with mean values of 9.01±1.23 mm. Vitapex showed the lowest dye penetration, with mean values of 4.02±1.79 mm. Comparison between groups showed significant difference when the Vitapex and Kri paste groups were
compared to the ZOE group (P<.0001). Also, a significant difference between the Vitapex and Ca(OH)2 group (P=.014) was detected. The result showed no difference between Vitapex and Kri paste (P=.658) nor between Kri paste and Ca(OH)2 (P=.185).Conclusions:
All the resorbable root canal filling materials used in primary teeth showed apical leakage. The highest apical leakage was recorded for ZOE, while Vitapex showed minimum dye leakage.
