Estudo descritivo, desenvolvido junto a seis mães de crianças com leucemia, internadas em um hospital especializado, com o objetivo de conhecer quem são as crianças atendidas no serviço e identificar ...como as mães percebem a experiência de cuidar no domicílio e como elas avaliam as orientações recebidas para este cuidado. Os resultados revelaram que apesar das dúvidas, dificuldades e conflitos no interior da família, as mães cuidam com desvelo e se sentem bem em poder fazer isto, e buscam na religiosidade forças para enfrentar a situação. Observou-se ainda que elas sentem necessidade de maior interação com a equipe de enfermagem e que não se sentem à vontade para expressar expectativas ou questionamentos, mesmo quando não compreendem as orientações, revelando a existência de lacunas no âmbito da função educativa-assistencial dos profissionais enfermeiros, como subsídio para atuação da mãe/cuidadora no contexto domiciliar.
This is a descriptive study carried out with six mothers of children suffering from leukemia admitted to a specialized hospital. The objective of the study was to find out who the children being treated in this service are and to identify how mothers perceive the experience of caring for their child at home and how they evaluate the orientation they receive for this type of care. The results revealed that, in spite of doubts, difficulties and conflicts within the family, mothers care for their children with selflessness and feel positive about doing so, resorting to religion to get strength to face the situation. It was also observed that mothers feel a need for further interaction with the nursing staff, but do not feel at ease to express their expectations and questions, even at times when they do not understand the orientations. This is an indication of serious gaps regarding the nursing professional's function of providing assistance and educational support to the mother/caregiver within the family context.
Se trata de un estudio descriptivo, desarrollado con seis madres de niños con leucemia, internadas en un hospital especializado, con el objetivo de conocer quiénes son los niños atendidos en el servicio e identificar cómo perciben las madres la experiencia de cuidar en el domicilio y cómo evalúan las orientaciones recibidas para este cuidado. Los resultados revelaron que a pesar de las dudas, dificultades y conflictos en el interior de la familia, las madres cuidan con desvelo y se sienten bien de poder hacer esto, y buscan en la religiosidad fuerzas para enfrentar la situación. Además se observó que ellas sienten necesidad de más interacción con el equipo de enfermería y que no se sienten cómodas para expresar expectativas o cuestionamientos, aun cuando no comprenden las orientaciones, revelando la existencia de lagunas en el ámbito de la función educativa-asistencial de los profesionales enfermeros, como subsidio para la actuación de la madre/cuidadora en el contexto domiciliario.
OBJETIVO: Foi realizada uma revisão integrativa da literatura com o objetivo de avaliar as evidências disponíveis na literatura o conhecimento científico produzido relacionado à qualidade de vida ...(QV) do paciente adulto com câncer de cólon e reto. MÉTODOS: Selecionamos artigos publicados nas bases de dados LILACS, CINAHL e MEDLINE, entre 1997 e 2007. RESULTADOS: Encontramos que o diagnóstico e o tratamento para o câncer afetam vários domínios da QV do paciente e que fatores como a prática de exercícios, aumento da aptidão física, o estado de boa nutrição, a presença de suporte psicossocial e de rede social favorecem uma QV melhor. CONCLUSÃO: Como lacunas no conhecimento, detectamos falta de produção científica nacional, escassa contribuição do enfermeiro, predomínio de estudos descritivos, de nível de evidência considerado fraco. Sugerimos a atuação do enfermeiro em pesquisas de intervenções de suporte psicossocial e de programas de atividade física que possibilitem melhorar a QV do paciente oncológico.

The purpose of this program evaluation study was to determine the difference in patient satisfaction with overall nursing care and perceived nurse caring when a nursing staff standard of care protocol, which included caring activities, was implemented within a nursing department at a National Cancer Institute-designated comprehensive cancer center.
A pre-experimental pre-/post-test design with comparison group and a post-test-only design were used to test the effect of the caring protocol on patient satisfaction with nursing care and perceived nurse caring. The study was conducted on seven inpatient units.
Patient satisfaction with nursing care increased for some items, and perceived nurse caring was ranked highly immediately before discharge. Additional program evaluation studies involving a modified caring protocol may improve satisfaction with nursing care in other settings.

A patient with a mucinous appendiceal cancer presents to the surgeon complaining of abdominal discomfort and nausea. Having undergone a prior right hemicolectomy, the patient has been disease free ...and on surveillance with clinical and carcinogenic antigen (CEA) monitoring. The CEA was noted to be elevated and a computed tomography scan revealed peritoneal nodules throughout the abdomen with a presumptive diagnosis of pseudomyxoma peritonei (progressive peritoneal implants from a mucinous primary). Several therapeutic options were offered and the patient selected to undergo cytoreductive surgery (CRS) with the potential to receive hyperthermic interoperative chemotherapy (HIPEC). Extensive resection was performed, including removal of the entire greater omentum, partial gastrectomy, and total pelvic exenteration with end colostomy and ileal conduit. Reassessment of the peritoneal cavity after the resections revealed almost complete cytoreduction. HIPEC was performed with mitomycin C and, after drainage and abdominal washing, the intestinal segments were anastomosed and the abdominal wall closed. Seven days postoperatively, an acute abdomen with septic shock developed as a result of a leak from the ileocolonic anastomosis. The patient returned to the operating room and an exploratory laparotomy, a small bowel resection, a resection of the ileocolonic anastomosis, and an abdominal washout were performed. Edema of the bowel caused by peritonitis resulting from the anastomotic leak necessitated delayed closure of the abdominal wall. A temporary abdominal closure using the ABThera™ Open Abdomen Negative Pressure Therapy system was applied and the abdomen was eventually closed.

Failure mode and effects analysis (FMEA), a prospective, systems-focused risk assessment methodology, was undertaken in biweekly team meetings and included mapping the chemotherapy administration process, identifying and numerically prioritizing potential errors (failure modes) for each process step, and generating remedial strategies to counteract them.
The analysis aimed to identify chemotherapy administration failure modes and to generate remedial strategies to address them. User feedback on the FMEA process also was collected.
Several specific chemotherapy failure modes were identified, the majority of which had not previously been recognized, and several novel strategies to counteract them were generated. Many of the strategies were specific, environment-focused actions, which are simple, quick, and inexpensive to implement; however, more substantive, longer-term initiatives also were generated. User feedback generally was very positive, and the process of undertaking the analysis improved multidisciplinary teamwork and communication.
Although time and resource intensive, FMEA is a useful safety improvement tool.
Nurses should be aware of and informed about FMEA as a tool they can use in partnership with management and other disciplines to proactively and collectively improve the safety of high-risk oncology procedures such as chemotherapy administration.

Esta pesquisa teve como objetivo compreender a percepção dos cônjuges de mulheres mastectomizadas em relação à convivência pós-cirúrgica. Estudo qualitativo, realizado em 2006, no domicílio de cinco ...homens que viviam maritalmente com mulheres mastectomizadas, em Fortaleza-CE. Utilizamos a entrevista semi-estruturada. Os resultados foram agrupados em três categorias de análise. Identificamos um nível de desinformação quanto à doença, tranquilidade atribuída à fé em Deus, e a forte convicção de que câncer e morte são sinônimos. Cabe aos profissionais de enfermagem/saúde favorecer o conhecimento sobre câncer de mama, e abrir um painel de integração marido/esposa, priorizando uma convivência saudável.
The objective of this study was to understand how the partners of mastectomized women perceived life after the surgery. The study is both descriptive and exploratory, and was conducted in 2006 with five men who lived with mastectomized women in Fortaleza, Brazil. Semi-structured interviews were performed and the results were grouped into three analytic categories. We identified a level of misinformation about the illness, tranquility attributed to a faith in God, and a strong conviction that cancer and death are synonyms. Nursing/health professionals are responsible for providing knowledge about breast cancer and for fostering for integration between the woman and her partner, prioritizing a healthy life together.
Esta investigación tuvo como objetivo comprender la percepción de los cónyuges de mujeres mastectomizadas respecto de la convivencia posquirúrgica. El estudio fue de carácter cualitativo, realizado en 2006, en los hogares de cinco hombres que convivían maritalmente con mujeres mastectomizadas en Fortaleza, Ceará, Brasil. Se utilizó el sistema de entrevista semiestructurada, los resultados fueron agrupados en tres categorías de análisis. Identificamos un nivel de falta de información respecto de la enfermedad, cierto grado de tranquilidad atribuible a la fe en Dios y la fuerte seguridad de que cáncer y muerte son sinónimos. Corresponde entonces a los profesionales de enfermería y otros profesionales de la salud favorecer los conocimientos respecto del cáncer de mama y abril un panel de integración esposo/ esposa, apuntando como prioridad a una convivencia saludable.
Asignificant increase in nursing research is being conducted as the nursing profession shifts from "ritual" clinical decisions to practice based on research evidence. Evidence-based practice is now ...an accepted, essential foundation for high-quality patient care. Initially, best practice was based on a few randomized, controlled trials that reflected similar clinical problems. However, with the plethora of nursing research to date, evidence-based nursing practice currently is grounded in summaries of research or research reviews, resulting in robust findings used in the development of clinical guidelines. Several terms exist for reviews, such as literature, integrative, systematic, meta-analysis, and metasynthesis. Similarities can be noted among the types of reviews; however, the objectives and goals of each method differ and the terms should not be used synonymously. This article will define each of the literature and research reviews and discuss methodologic procedures for conducting each method.
