El diagnóstico de viabilidad miocárdica es importante en aquellos pacientes que van a ser sometidos a una revascularización miocárdica, ya sea quirúrgica o por angioplastia coronaria. Nuestro propósito es presentar algunas de las direcciones actuales en la detección de viabilidad con el empleo de las pruebas de cardiología nuclear. Se hace hincapié en la cuantificación de la captación del radiofármaco y en el uso de vasodilatadores en la gammagrafía de perfusión, principalmente con compuestos marcados con tecnecio. Además, se señalan las posibilidades actuales para el estudio del metabolismo miocárdico con tomografía de emisión de fotón único y, por último, algunas de las implicaciones clínicas de la viabilidad. The myocardial viability diagnosis is important for those who will underge myocardial revascularization wither by surgery or coronary angioplasty. Our purpose is to present some of the present trends in the detection of myocardial viability using nuclear cardiology tests. Emphasis is made on the estimation of radiopharmaceutical uptake and the use of vasodilators in perfusion scintigraphy mainly with technetium-labeled compounds. Also, the current possibilities for a myocardial metabolism study using single-photon emission-computed tomography as well as some clinical implications of myocardial viability are set forth.
