    Segmental skeletal defects have always presented a problem for surgeons. With one of the most innovative contributions to orthopedic surgery - intercalary bone transport missing osseous tissue of any ... length can be reconstructed wihout the need for bone grafting. In the mid 1960s the concept was developed by G.A. Ilizarov. Corticotomy with preservation of blood supply is one of the cornerstones of the method. With a stable fixation bone forms in a distractiongap. A delay before distraction is necessary. Slow high frequency distraction creates stresses within bone that causes osteogenesis within the gap. The process is completed with a stage of corticalisation and remodeling of the cortex. Weight bearing and motion of the adjacent joints are important.Bone transport techniques are demanding and fraught with potential complications.The treatment is long- term and before the method is undertaken the patient s personality and expectations must be evaluated.
