  • Cistični karcinom ledvice: radiološke in patološke značilnosti pri petih bolnikih = Cystic renal cell carcinoma: radiological and pathological features in five cases

    Background. Cystic renal cell carcinoma (CRCC) is rare, but since a cyst is the most common focal lesion found in the kidney, it is imperative to exclude the possibility of CRCC in every "atypical" ... renal cystic lesion. We present our experience in five cases of CRCC. Methods. Between January 1996 and May 1999, of 44 patients with RCC, 5 (11,3%) had CRCC. Imaging studies in patientswith CRCC included ultrasonography (US) and multiphasic contrast-enhanced spiral CT (in all cases), angiography (in 2 cases), cyst aspiration (in 2 cases), US-guided core biopsy (in 2 cases) and sonography with Levovist (in 1 case). Flow cytometry of tumor tissue was performed in 2 cases. Results. Using the Bosniak classification system, the lesion was characterised as cystic neoplasm (cathegory IV) in 4 patients and as intermediate (cathegory lll) in 1 patient on the basis of US and CT studies. Angiography and cyst aspiration were not diagnostic. Core biopsy confirmed thediagnosis in 2 cases, intraoperative histology in 1 case. The tumors were multilocular in 2 cases, mixed (solid/cystic) in 2 cases, and unilocular cyst with tumor nodule in the wall in 1 case. In all 5 cases there was no evidence of vascular or collecting system invasion, overgrowth of the capsule or regional spread. In 4 patients there was no evidence of metastatic disease;1 patient presented with skeletal metastases. All neoplasms were clear-cell carcinomas of low nuclear grade, well differentiated; DNA histogram in 2 caseswas diploid, with low S-fraction. Conclusions. Using the Bosniak classification system, US and CT are capable to correctly characterise majority of renal cystic lesions. High index of suspition is necessary to suggest and confirm the possibility of CRCC.
