  • Natural history and treatment of membranous nephropathy

    Membranous nephropathy is an immune-complex mediated disease, well defined histopathologically and clinically, but with unpredictable course of the disease. The aim of this study was to present ... natural course of idiopathic membranous nephropathy and influence of the treatment. Among our biopsied patients we selected 58, 36 male and 22 female, aged 33.2 +- 11.5 who were nottreated or their treatment can be neglected to present the natural course of the disease. On the other hand, to present the response to therapy, we selected 39 patients with stage II of the disease, all nephrotic treated on different ways. They were divided in 4 groups: 10 without treatment, 13 treated with steroids, 13 treated with chlorambucil and steroids and 3 treatedwith cyclosporine. We noted 5 different outcomes of the disease in patients with membranous nephropathy: 1) long remissions, 2) relapsing disease, 3) slow progressive disease, 4) rapid progression and 5) relapsing nephrotic syndrome and renal failure. Patients treated with steroids presentedsigificant decrease of proteinuria (p 0.01), and also the patients treated with steroids and chlorambucil (p 0.05). It was not noted in control subjects and in cases treated with cyclosporine. Treated patients presented lower incidence of chronic renal failure compared with controls. We can conclude that natural course of the disease is unpredictable and our treated patients with the same histopathological degree present significant decrease of proteinuria and lower incidence of development of chronic renal failure during follow-up.
