    Study objective. To evaluate the problem of preterm delivery associated with different grades of uterine anomalies-groups 5 and 6, according to the American Association of Fertility and Sterility ... (AFS) classification-before and after hysteroscopic resections of uterine septa by resectoscope. Methods. A retrospective analysis of 468 hysteroscopic resections of uterine septum performed between 1993 and 1998. The 5% dextrose was used as a distending medium and the automated gravitational Vario Flow was the distending system. 1. Enquiry data on preterm deliveries before and after 468-hysteroscopic resection in different grades of uterine septum were compared. 2. Preterm delivery rates before and after hysteroscopic resection in the group of 176 women after hysteroscopic resection, included into the National Perinatal Information System (NPIS), were compared. 3. Perinatal data on 176 women afterhysteroscopic resection, included into the NPIS, were compared to data on354 non operated women. Results. 1. Enquiry data on 486 women after hysteroscopic resection. The structures of 574 pregnancies before the operation significantly differed from the structures of 306 pregnancies after the operation (P<0.0001). Preterm delivery decreased from 51 to 11% of all deliveries (P<0.0001). There were 43% AFS 6 septa and 57% AFS 5 septa. Pretermdelivery rate decreased from 56% to 8% after the resection of AFS 6 septa (P<0.0001), and from 47% to 12% after the resection of AFS 5 septa (P<0.002). 2. Preterm delivery rates before and after hysteroscopic resection in the group of 176 women after hysteroscopic resection, included into NPIS, showed a decrease from 44.9% in 69 deliveries before to 8.5% in 176 deliveriesafter the operation (P<0.000). The need for neonatal intensive care unit (NICU) was reduced from 27.5% before to 7.9% after the operation (P<0.002). (Abstract truncated at 2000 characters).
