    Erythrocytosis is characterized by pathologic elevation of red blood cell mass. The normal red cell mass at sea level is 26 to 32 mL per kilogram of body weight for men and 23 to 29 mL per kilogram ... for women. Erythrocytosis canbe classified by the underlying pathophysiologic mechanisms as primary or secondary erythrocytosis. Primary erythrocytosis represents autonomous proliferation of erythroid progenitors as a result of an intrinsic cellular defect. Secondary erythrocytosis is a heterogeneous group of disorders where erythrocytosis is because of either a decreased tissue oxygenation or the physiologically inappropriate production of erythropoietin. Erythrocytosis is initially considered on the basis of elevated hematocrit. A man or a woman with hematocrit level of 0.60 or higher or 0.57 or higher, respectively, virtually always has a true increase in red blood cell mass (i. e., primary orsecondary erythrocytosis). If patient's hematocrit is below these values butabove normal, a red blood cell mass study should be done. With this we identify patients having elevation of hematocrit because of a plasma volume contraction (spurious erythrocytosis).
