  • Arthroscopic treatment of osteochondral lesions of the ankle = Artroskopsko zdravljenje osteohondralnih poškodb gležnja

    Osteochondral lesions of the talus are significant cause of persistent disability of the ankle. Patients with osteochondral lesions of the talus present with persistent ankle pain, episodes of ... swelling, catching, grinding and sensations of stiffness. This article reviews the current concepts on definitions, causes, preoperative planing and indications for surgery, arthroscopic techniques and postoperative management of osteochondral lesions.Treatment options, although varied, should improve outcome when implemented appropriately. Arthroscopic treatment is based on the stage of thelesion. Indication for surgery are detached lesions and lesions that not respond to conservative management. Our recommended treatment in displaced lesions is removal of the osteochondral fragments and antegrade drilling or abrasion of the bed to viable, bleeding subchondral bone. Arthroscopic management of the OCD lesions of the talus is very useful for accurately determining the stability of the fragment and this treatment has yielded good outcomes. The arthroscope provides direct visualisation of the defect, a less invasive means for operative treatment that arthrotomy and rapid mobilisation of the patients postoperatively.
