    Pain is often a rather non-specific symptom that accompanies neurological diseases and trauma. It may arise as nociceptive pain, as transient pain due to nerve lesions (neurogenic pain), and also as ... chronic pain syndrome due to involvement of the somatosensory afferent (spinothalamic) nervous system (neuropathic pain). The latter may develop after a time interval following an acute neurologic lesion. Neurogenic pain is not uncommon in peripheral nervoussystem lesions (involving roots, plexus or peripheral nerves). Chronification of pain may lead to chronic (peripheral) neuropathic pain syndrome. This includes postherpethic neuralgia, chronic neuropathic pain syndrome that accompanies polyneuropathies and causalgia following nerve lesions (complex regional pain syndrome type II). Central neuropathic pain maydevelop after various central nervous system lesions, for instance stroke or spinal cord trauma. Central neuropathic pain can develop even without a central nervous system lesion, as in the case of phantom pain after loss of body parts. The diagnostics and therapy of nociceptive and neu rogenic pain inneurological diseases and trauma is usually not intrinsically problematic. Chronic neuropathic pain syndromes are, however, frequently missed and they are also therapeutically very demanding. Non-pharmacological modalities (electrical stimulation, psychotherapy) should be combined with tricyclic antidepressants (Amyzol) and antiepileptic drugs. In case of poor efficacy, opiates can be tried, as there is a subgroup of patients with neuropathic painwho are opiate responsive. Intrathecal morphine administration is indicated in severe cases and is available also in Slovenia.
