    Heart failure is a clinical syndrome caused by the inability of heart to provide adequate perfusion to peripheral tissues and organs. In the last decade the number of patients with heart failure has ... been steadily increasing.The current guidelines represent an efficient and useful tool fortreatment of heart failure on primary and secondary level of care. However, despite the significant progress in the treatment of heart failure, a substantial number of patients reach an advanced stage of the disease, which needs to be treated with sophisticated and novel techniques avail` able only in specialized tertiary medical centers. In Ljubljana Medical Center, the currently available modalities for treatment of advanced heart failure includeheart transplantation, biventricular pacing and newer pharmacologic approaches. These modalities represent the fundament for introduction of noveltherapies including left ventricular assist device support, immunoadsorption, and stem cell transplantation.
