    Nasal polyposis is a multifactorial state of the disease influenced by endogenous and exogenous inflammatory processes in the mucosa of the nasal andparanasal cavities. Polyp formation starts with a ... rupture of the epithelium. Biophysical and biochemical factors promote its further growth. Nasal polyps are prone to recur. Our treatment is not causal. For majority of patients the management of nasal polyposis comprises a combination of medical and surgical therapies. Most efficient drugs proved to be systemic and topicalglucocorticosteroids that cause inflammation in the mucosa and edema inthe polyps to subside. Shrunked polyps are then endoscopically removed and the drainage of the nasal and paranasal cavities is thus reestablished. Postoperative state is maintained by topical steroid. The efficiency of the various surgical approaches in the treatment of nasal polyposis has not been methodically dealt with, at least not in the last decade, when it became obvious that these pathological changes are primarily treated with drugs. In our research we compare the efficacy of the two endoscopic surgical methods that vary only regarding to the extent of the procedure, namely endoscopic simple polypectomy (P) and polypectomy with endoscopic ethmoidectomy (P+E), both combined with uniform medication. The goal of our survey is to establish whether the intensive medical treatment enables less extensive, ab initio planned surgical procedure. In prospective randomized study we included 40 patients with massive bilateral nasal polyposis and no history of previous nasal surgery, who were consecutively treated at The Clinic for Otorhinolaryngology and Cervicofacial Surgery of The Ljubljana University Clinic Centre in the period from October 1993 to May 1995. All our patients received the same systemic and topical corticosteroid medication. Surgical approach was randomized. (Abstract truncated at 2000 characters)
