We ... isolated L. pneumophila serogrup 1 from the hospitals old water supply system which provided water to the faucetsin the semi-intensive and the coronary units and their sanitary facilities. Thirty-one out of 71 personnel in the medical wards and 7 out of 111 patients in the medical wards fell ill. Contrary to the expectations, healthy personnel without known risk factors became more often ill with Legionella in this epidemic than bed-ridden patients in various degrees of immunosuppresion. To explain this contradiction we hypothesised that the source of infection was the contaminated aerosol, formed at the faucet, to which the personnel was exposed due to their work in vicinityč the bed-ridden patients, in the same room but at a distance of more than 3m, were not infected because the aerosol was less concentrated there, thus less contagious. The hot water system in the semi-intensive and in the coronary units was connected to the old system ĆblindĆ branch without circulation. The faucet had to be let full open for over 2 min to obtain warm enough water for patients care in the semi-intensive and coronary units. The water jet rebounded from the ceramic basin and formed aerosol. Relative air humidity 60%or higher is known to be of advantage for the survival of Legionella, whileat 30% this survival drops sharply. Hygrometer measurements showed the relative air humidity in immediate vicinity of the open faucets to reach 55% to 85% for a short period of time, after 10 min this dropped to original levels of 30-40%. The relative air humidity at 3m distance remained unchanged throughout, the aerosol did not reach that far.
