    Cerebral autoregulation can be evaluated by testing the cerebrovascular reserve. Stimuli which exert actions on the cerebral vessels through neurons, endothelium or sympathetic fibres may be used. ... The response of cerebral circulation can be monitored by transcranial Doppler sonography. Cerebral autoregulation during the activation of the sympathetic nervous system by the cold pressortest and head-up tilt was studied in healthy subjects and in diabetics with autonomic neuropathy. It was found that large cerebral vessels are involved in this response, and that cerebral autoregulation is impaired inpatients with autonomic neuropathy due to impairment of the sympathetic nervous system. The vasomotor reserve can be determined in patients with hemodynamically significant stenoses of the internal carotid artery by monitoring arterial velocity during carbon dioxide stimulation. This is also helpful in determining risk of stroke. The functional state of the cerebral artery endothelium should be evaluated after intravenous administration of L-arginine, a precursor of nitric oxide, in healthy subjects as well as in patients with cerebrovascular diseases. The vaso-neural coupling and the autoregulation of the occipital lobe can be determined by measuring the flow evoked after visual stimulation.
