  • Sporočanje slabe novice in sporazumevanje z družino težko bolnih in umirajočih = Breaking bad news and communication with the family of terminally ill patient
    Vodopivec Jamšek, Vlasta
Despite the fact that serious illnesses and death represent part of our life, they can foster evolution of mental disorders (anxiety, depression) in the patient or in his/her family. Communication ... with terminally ill patient and his/her family is one of the crucial skills for the work of a family physician. He has to diagnose mental problems in order to diminish their possible negative effects. Breaking bad news is a very difficult task and should be done in a suitable situation, demanding enough time, active listening skills, empathy and honesty of the communicator of bad news. The doctor has to inquire about the patient's knowledge regarding his/her disease and to which extent he/she wants to be informed about his/her problem. The news should be disclosed stepwise and in a gentle manner. The doctor should try to discover patient's emotions and worries and in consensus with him seek the best possible solutions. Terminal illness and death have a major impact onthe family and present a great challenge to the family function. Family physician who undestands family problems and needs, can more easily find the right way to break bad news. Planning care for a terminally ill patient and discussion about reducing certain treatments are stressful for both, the doctor and the family. A family conference at the patient's bed can resolve many problems. Loss of a belowed person presents one of the most stressful life events. The stress of family members is even greater if they feel lonely or without possibility to share emotions. Family physicians have to be look for any signs of unresolved or prolonged mourning.
