  • Klinična genetika hipertenzije = Clinical genetics of hypertension

    This review article is intended to introduce the uninitiated pediatrician to the basic concepts, aims and questions of the clinical genetics of hypertension. It separates the rare monogenetic ... hypertensive disorders from the vast majority of patients with essential hypertension, which is a complex, polygenic, multifactorial disorder resulting from interaction of several genes with each other and with the environment. It highlights some familial studies, the role of ethnicity, sodium, sympathetic system and histocompatibility antigenes. Genes implicated in the etiopathogeness of essential hypertension (NHE-1 gene, renin gene, ACE gene, AT1 gene, SA gene, NOS gene, atrial natriuretic peptice gene, alpha-adducin gene, insulin receptor gene and angiotensinogen gene) are briefly discussed. The five different forms of monogenetic hypertension (Liddle's syndrome, Gordons syndrome, glucocorticoid-remediable aldosteronism, the syndrome of apparent mineralocorticoid excess and the syndrome of brachydactyly type E and hypertension) are listed. It is hopped that new, more efficient methods of genetic analysis will yield clinically meaningful information.
