    Background. Two basic facts stress the need to discuss the optimal length of antidepressant treatment:- If antidepressant are withdrawn soon after disappearance of depressive symptoms, patients are ... at high risk of relapse. - Affective disorders often run an episodic course. Mojority of patients who present with a depressive episode will sooner or later suffer from a recurrence. The article reviews data on time course of depressive disorder andhow it can be influenced by antidepressants. Guidelines for continuation and maintenance treatment are given (indications, contraindications, choice and dosage of antidepressants). Special attention is given to treatment of bipolar affective disorder and antidepressant use in elderly patients. Alternative and/or additional methods of treatment are briefly mentionend. Conclusions. Better adherence to guidelines on length of antidepressant treatment could be achieved by use of modern drugs with better side-effect profile. Still, patient compiance remainsan issue and a challenge for clinicians. Concomitant use of other treatment methods, certain forms of psychotherapy being most important, is recommended in patients with incompleteresponse to antidepressants.
