  • Diagnostika intersticijskih pljučnih bolezni iz bronhoskopsko odvzetih koščkov tkiva = Bronchoscopic tissue sampling for the diagnosis of interstitial lung diseases
    Kern, Izidor
Background. Bronchoscopic lung biopsy (BLB) is a method used for histopathological diagnosis of lung parenchymal lesions, occurring also in interstitial lung diseases. The objective of this study was ... to determine the diagnostic yield of BLB at this institution relative to the results published in the literature. Material and methods. All BPBs performed in 1990 and in thepast 6 months were included in the analysis. According to the original histopathological diagnosis, the specimens were assigned to the following fourgroups: nonrepresentative/nondiagnostic, nonspecific, possibly specific and specific. Our previous results and the results from other institutions were categorized by the same method. Results. The proportion of nonrepresentative/nondiagnostic specimens was 11%. It was lower than our previous results and lower than the findings published by other authors. Approximately one third of histopathological changes were diagnosed as nonspecific. The proportion of possibly specific results was higher than the percentage reported in the literature. The proportion of specific diagnoses did not differ significantly from the published data. The diagnostic yield, i.e. the sum of proportions of specific and possibly specific diagnoses, was 55%; it was higher than the yield documented at this institution in the previous years and the percentage publiished in the literature. The diagnosed infections and neoplasms that accounted for 11% to 53% of specific diagnoses contributed considerably to the total diagnostic yield of BLB. Sarcoidosis wasthe most common entity among specific diagnoses; other conditions were diagnosed rarely. Conclusions. BLB is a useful technique used for diagnosing sarcoidosis and some rare interstitial lung diseases. It is valuable in excluding other diseases with characteristic histopathological changes, such as infections and neoplasms, which are not classified as interstitial lung diseases.
