    Plasma concentration of aminoacid homocysteine is elevated in patients with chronic renal insufficiency and end-stage renal disease, and represents independent risk factor for cardiovascular ... diseases. Etiology of elevated homocysteine level which correlates with worsening of renal function is not known. This paper gives results of studies of homocysteine metabolism in chronic renal insufficeincy. Review of associations of elevated homocysteine with cardiovascular risk and up to date results of hyperhomocysteinemia treatment in renal insufficiency are also given. Renal transplant recipients are special subgroup of renal patients and the results of folic acid treatmentin this group are better than in patients on hemodialysis. Vascular acces thrombosis in dialysis patients is also probably associated with hyperhomocysteinemia. Further prospective studies of impact of hyperhomocysteinemia treatment on cardiovascular risk reduction in chronic renal insufficiency and end-stage renal disease patients are needed.
