    Perioperative cardiac morbidity remains the main cause of death following anesthesia and surgery. In cardiovascular surgery the cardiologist, the anesthesiologist, and the cardiac surgeon focus on ... the same topic: their patient's cardiac problem. In noncardiac surgery the underlying cardiac diseases and disorders are the main determinant of perioperative complicationsand thus present an obstacle on the way to a resolution of another problem and successful surgical outcome. Cardiology consultant has a paramount role in estimating the risk and suggesting both timing of surgery and necessary preoperative diagnostic and therapeutic measures. The cornerstones in the evaluation of the risk of perioperative morbidity and mortality are based on history and physical examination and simple laboratory tests. Only selected cases require more sophisticated exercise, stress, radionuclide, and echocardiographic tests. Age is one of the risk factors for cardiac morbidity. It may not be an important independant risk factor, but itsassociation with other diseases (CAD, diabetes mellitus, peripheral arterial disease, arrhythmias, and arterial hypertension) makes it a useful univariate predictor of cardiac risk. Elderly patients with a reduced level ofactivity are difficult to evaluate because many simptoms and diagnostic tests are exercise-related. Multifactorial risk indices have been developed topredict perioperative cardiac complications. Optimization of patients' physical status and preoperative treatment also improves chances for favourable outcome. Cardiologist or other physician evaluating a patient preoperatively should make clear whether the patient is at increased risk of perioperative morbidity and mortality, what further diagnostic tests sholuld be performed, what should be done preoperatively to minimize the risks, and whether the anesthesia and surgery should be changed, postponed, or cancelled.
