  • Nova farmakološka možnost zdravljenja nevropatske bolečine - novejše klinične raziskave = New pharmacological treatment options for neuropathic pain - recent studies

    Anticonvulsants are frequently used for treating neuropathic pain conditions. Neuropathic pain is defined as pain initiated or caused by a primary lesion ordysfunction in the nervous system. The ... mechanisms of neuropathic pain are complex and not completely understood. In case of peripheral neuropathic pain,the effects of anticonvulsants are associated with sodium channels activity, while the effects on central neuropathic pain seem to be related to calcium channel blockade. Inhibitory circuits are other possible targets of anticonvulsants. Although anticonvulsants are widely used in the treatment of neuropathic pain, relatively few randomised placebo controlled trials have been published. Carbamazepine is the treatment of choice for trigeminal neuralgia. The therapeutic use of phenytoin is limited due to its unfavourableside effects. Valproate has a wide range of actions on various neuropathic pain states. The application of gabapentin in neuropathic pain conditions is increasing. Pregabalin is a promising, newer anticonvulsant in neuropathic pain treatment. Lamotrigine and topiramate have shown good effectsin some neuropathic states. The diagnostic and therapeutic goal in neuropathic pain is evaluation of the underlying pathophysiologic mechanism and its treatment.
