  • Pokontrastna nefropatija - nevarnostni dejavniki in preprečevalni ukrepi = Contrast-induced nephropathy - risk factors and preventive measures

    Contrast-induced nephropathy (CIN) is clinically important, iatrogenic and thethird most frequent in-hospital acquired cause of acute renal failure. Contrast-induced nephropathy is defined by the ... absolute (>= 44 micro mol/l) orrelative increase (>= 25%) of the baseline serum creatinine values in 48-72 hours after exposure to a contrast agent, when alternative explanations for renal impairment have bee excluded. The incidence of contrast-induced nephropathy in general population is 0.6-2.3% and 3.3-26% in high risk patients. The most important clinical measure is prevention of CIN by identification of risl` factors as well as individuals at risk for CIN. The most dangerous risk factors seem to be pre-existent renal disease, especially diabetic nephropathy, prerenal factors (hypovolemia, anemia, heart failure), and administration of high volume of contrast media. Volume supplementation remains the cornerstone for the prevention of contrast-induced nephropathy. The review article is intended for operators of invasive diagnostic and therapeutic interventions as well as hospital physicians general practitionersand nurses taping care of patients after exposure to a radiocontrast agent.
