    Background. Total hip replacement is a good solution for the patients with hiparthrosis. Aseptic loosening is the major long-term problem which is manifested with pain, especially with weight ... bearing. Interface between bone ad implant is of crucial importance for the developing of aseptic loosening. Due to forces and pressures acting on the cement mantle, micromovements and frank fissures develop, leading to the development of soft tissue interface membrane. Due to citochemical factors acting in the interface chronic inflamation and finally osteolysis and loosening issue. Conclusions. In the article historical backroung of total hip replacement and mechanical and biological factors in relation to aseptic loosening are presented.
