Branislava Nušića. Riječ je o drami s obilježjima komedije situacije, komedije karaktera i ...satire, u kojoj su podjednako zastupljeni verbalni i situacijski humor. Analiza je provedena s ciljem utvrđivanja pragmatičkih sredstava i strategija postizanja verbalnog humora zastupljenih u drami, a pri njezinu provođenju korišten je pojmovni aparat preuzet iz suvremenih lingvističkih, prvenstveno pragmatičkih teorija humora zasnovanih na pojmu nekongruentnosti. U prvom dijelu rada predstavljen je teorijski okvir istraživanja, dok je drugi dio posvećen analizi. U njemu su na izabranim primjerima iz drame demonstrirani i analizirani najfrekventniji pragmatički okidači humora, koji su podijeljeni u tri skupine: (1) okidači koji se zasnivaju na izigravanju/kršenju kooperativnog principa, a ostvaruju se prijenosom implicitnih značenja, (2) okidači koji se zasnivaju na izigravanju/kršenju principa uljudnosti, a ostvaruju se posredstvom neuljudnih i lažno uljudnih iskaza, (3)

dramskih likova, koji se ogledava u frekventnoj upotrebi ekspresivnih izraza
te upotrebi neprikladnog i neočekivanog jezičnog registra.
The paper presents a pragma-stylistic analysis of the play The Minister's Wife (1929) by the Serbian writer Branislav Nušić. This play contains both verbal and situational humour, and shows characteristics of situation comedy, comedy of character and satire. The analysis was carried out with the aim of determining the pragmatic means and strategies for achieving verbal humour in the play. The theoretical apparatus was taken from the contemporary linguistic (primarily pragmatic) theories of humour, which are based on the concept of incongruity. In the first part of the paper, the theoretical framework of the research is presented, while the second part is devoted to the analysis. The most frequent pragmatic triggers of humour are demonstrated and analysed onselected examples from the play. These triggers are classified into three groups: (1) triggers based on flouting/violating the cooperative principle, which is reflected in the transmission of implicit meanings, (2) triggers based on flouting/violating the principle of politeness, which is reflected in the use of impolite and falsely polite utterances, (3) colourful language of dramatic characters, which is characterized by the frequent use of affective expressions and the use of inappropriate and unexpected register.
We report the case of a full term male child with multiple organ dysfunctions caused by mutations of the TTN gene. Our patient had cardiac abnormalities verified by fetal echocardiography at 26 weeks ...of gestational age, when fetal hydrops due to cardiac decompensation was shown.Due to specific appearance seen at birth – generalized oedema, light facial and sexual dimorphism with hypogenitalism and chryptorchidism, axial hypotonia in addition to the proven congenital heart defects, a genetic disorder or syndrome with muscle involvement was suspected. Multiple comorbidities were present since birth, mainly cardiac and neurologic, some of which have been described in patients with core myopathies (atrial and ventricular septal defects, supraventricular tachycardia, hypotonia, developmental delay, myopathy). Due to the complexity of the heart defect at the age of 2 months a partial pulmonary trunk banding and ligation of the ductus Botalli (PDA) was initially carried out, and at 9 months a complete correction of heart defect was performed. Our patient also had failure to thrive as well as recurrent vomiting, wich was later on asociated with extraluminal compression of the proximal duodenum, intestinal malrotation and malfixation. Although a surgical approach somewhat increased tolerance of peroral food intake, chronic diarrhoea and failure to thrive persisted.In addition, our patient had several metabolic and endocrine disorders including hypocalcemia, hypomagnesemia, hyponatremia, repeatedly elevated levels of PTH, hypothyroidism etc. Our patient had recurrent, mainly respiratory infections with multiresistant microorganisms, and recuired intubation and long-lasting mechanical ventilation. The above mentioned multiple disorders of various organ systems, with complications caused by multiple sepses, ultimately resulted in multiorgan failure, and despite all the applied treatment at the ICU, resulted in a lethal outcome. Coresponding findings described in other patients with core myopathies included neonatal hypotonia, poor suckling, severe motor skill delay, complex heart defects and cardiomyopathy.
Cilj istraživanja je dobivanje uvida u izazove udomiteljstva djece s problemima u ponašanju iz perspektive stručnjaka, kao i prikazivanje njihovih prijedloga za unapređenje udomiteljstva, a sve u ...svrhu identificiranja smjernica za razvoj specijaliziranog udomiteljstva i zaštite dobrobiti djece s problemima u ponašanju.
Podaci su prikupljeni putem polustrukturiranih intervjua (N=7), a kao metoda obrade prikupljenih podataka korištena je analiza tematskog okvira. Rezultati pokazuju da su iz perspektive stručnjaka glavni problemi prakse sustava udomiteljstva u RH prije svega u tome što specijalizirano udomiteljstvo nije zaživjelo, iako postoje zakonske osnove za taj oblik udomiteljstva. Stručnjaci svoja iskustva u području udomiteljstva djece s PUP-om opisuju kroz teme: ograničenja sustava, nestabilnost i negativno iskustvo smještavanja djeteta s PUP-om u udomiteljstvo te čimbenike koji pridonose pozitivnom iskustvu udomiteljstva djeteta s PUP-o. Kao izazove udomiteljstva djece s PUP-om stručnjaci navode nepripremljenost udomitelja na skrb za dijete s PUP-om, poteškoće vezane uz manifestirana ponašanja djeteta, nedovoljnu podršku udomiteljima te okolinske čimbenike rizika za dijete. Prijedlozi stručnjaka za unapređenje udomiteljske skrbi za djecu s PUP-om su pokretanje specijaliziranog udomiteljstva u praksi što podrazumijeva prije svega postavljanje jasnih kriterija za bavljenje specijaliziranim udomiteljstvom, organizaciju sustavne pomoći i podrške za udomitelje, potrebu regrutiranje novih udomitelja i profesionalizaciju udomiteljstva.
Scapula fractures are very rare, resulting from high-energy trauma and are mostly associated with other fractures of the locomotor system and injuries to the head, chest or abdominal organs. Most ...often, they are treated conservatively and the indications for surgical treatment are not yet clearly agreed. One of the conditions requiring surgical treatment is a scapula fracture with fragment migration into the thoracic cavity. In this paper, we are presenting an extremely rare, isolated scapula fracture, without injury to other parts of the locomotor system, with an intrathoracic fragment migration that was treated conservatively with an excellent functional outcome.
This study investigated the effect of consumption of table eggs enriched with n-3 polyunsaturated fatty acids (n-3 PUFA), lutein, vitamin E and selenium on microvascular function, oxidative stress ...and inflammatory mediators in patients after acute coronary syndrome (ACS).
In a prospective, randomized, interventional, double-blind clinical trial, ACS patients were assigned to either the Nutri4 (N=15, mean age: 57.2 ± 9.2 years), or the Control group (N=13; mean age 56.8 ± 9.6 years). The Nutri4 group consumed three enriched hen eggs daily for three weeks, providing approximately 1.785 mg of vitamin E, 0.330 mg of lutein, 0.054 mg of selenium and 438 mg of n-3 PUFAs. Biochemical parameters, including serum lipids, liver enzymes, nutrient concentrations, serum antioxidant enzyme activity (catalase (CAT), glutathione peroxidase (GPx), superoxide dismutase (SOD)), and markers of oxidative stress (thiobarbituric acid reactive substances (TBARS) and ferric reducing ability (FRAP)), were assessed before and after the dietary interventions. Additionally, arterial blood pressure, heart rate, body composition, fluid status, anthropometric measurements, and skin microvascular blood flow responses to various stimuli (postocclusive reactive hyperemia (PORH), acetylcholine- (Ach ID), and sodium nitroprusside- (SNP ID)) were measured using laser Doppler flowmetry (LDF) throughout the study.
The intake of Nutri4 eggs led to a significant reduction in LDL cholesterol levels, while the levels of total cholesterol remained within the established reference values. Consuming Nutri4 eggs resulted in a 12.7% increase in serum vitamin E levels, an 8.6% increase in selenium levels, and demonstrated a favorable impact on microvascular reactivity, as evidenced by markedly improved PORH and ACh ID. Nutri4 eggs exerted a significant influence on the activity of GPx and SOD, with no observed changes in TBARS or FRAP values.
The consumption of Nutri4 eggs positively influenced microvascular function in individuals with ACS, without eliciting adverse effects on oxidative stress.
Although self-limiting viral infections are the principal cause of fever in the majority of cases, it is estimated that 5% of febrile children younger than three years of age would have a urinary ...tract infection (UTI). Despite the increased availability of laboratory testing and development of various clinical scores, there is still no reliable predictor of UTI in children.Recent advancements in the understanding of inflammatory process have highlighted the role of S100 proteins, with many studies suggesting their clinical value as a biomarker of inflammation. This research aims to investigate the possible role of serum calprotectin (sCAL), a heterodimer consisting of S100A8 and S100A9 proteins from S100 family, as a biomarker for bacterial urinary tract infection in children.Patients aged 0-36 months who came to Pediatric Emergency Department of Sestre milosrdnice University Hospital Center with fever ≥38°C, lasting <72h, positive urine leukocyte esterase and no history of chronic illness or ongoing antibiotic use, were enrolled. In every patient, sCAL was measured with Quantum Blu rapid test, along with urine culture analysis and testing of standard inflammatory markers. Patients with bacterial count of ≥10^5 CFU/mL in urine culture were defined as having UTI, while patients with negative culture, absent signs of other bacterial infection, positive influenza, adenovirus, and/or respiratory syncytial virus test, and/or characteristic symptoms were diagnosed with viral acute respiratory infection (ARI).Among 52 patients included in the study (median age 6 months, IQR 3,1 – 13,8), 30 were diagnosed with ARI and 22 with UTI. In patients with UTI, the median value of sCAL was 5,6 ug/mL (IQR 3,9 – 6,3), which was significantly higher (p<0,0001) than in patients with ARI, whose median value of sCAL was 2,6ug/mL (IQR 1,7 – 3,5). There was a moderate positive correlation of sCAL with WBC (r=0,64; p<0,01) and CRP (r=0,55; p<0,01). The sCAL had area under curve (AUC) of 0.93 (95% CI 0.85 – 100) for predicting UTI, and with the optimal cut-off (3,95ug/mL), sensitivity was 93,1% (95% CI78,0 – 98,8%), specificity 76,19% (95% CI 54,9 – 89,4%) and positive likelihood ratio was 3,91.The results of this study have shown that sCAL is a good predictor of bacterial UTI in a febrile child presenting to an emergency department.Besides, the correlation of sCAL with duration of fever and inflammatory markers standardly used to infer the presence of bacterial infection was only moderate or insignificant, suggesting the potential differences in their dynamics. Hence, the sCAL concentration in febrile children could add to timely and accurate diagnosis of UTI, which is essential to reduce treatment delay and improve treatment outcomes.
BACKGROUNDAs a extremely traumatic experience, captivity may cause other mental disorders in addition to posttraumatic stress disorder, which is highly prevalent among ex-prisoners of war, and which ...often occurs in comorbidity with at least one other mental disorder. This objective of this study is to identify the incidence of comorbid mental disorders in Homeland war veterans ex-prisoners of war affected by posttraumatic stress disorder, as well as to identify the factors that influenced psychiatric comorbidity. SUBJECTS AND METHODSThe study sample comprised 264 subjects, all of whom were Croatian Homeland War veterans with combat experience in the defence of the Republic of Croatia, and all of whom fulfilled clinical criteria for posttraumatic stress disorder at the time of the study. The subjects were divided into two groups: the experimental group was composed of ex-prisoners of war, and the control group of veterans who had never been prisoners of war. The methods of sociodemographic questionnaire, posttraumatic stress disorder self-report checklist and the Harvard Trauma Questionnaire were used in the study. Psychiatric comorbidity data were retrieved from the subjects' anamnesis and medical records. RESULTSThe results showed that ex-prisoners of war were exposed to a statistically much higher number of traumatic events, and had a significantly higher total number of psychiatric comorbidities (p<0.01) than the control group. The incidence of acute and transient psychotic disorders, generalized anxiety disorders and psychological and behavioural factors associated with disorders or diseases classified elsewhere was significantly higher among ex-prisoners of war. There was no statistically significant difference in overall posttraumatic stress disorder intensity between the two groups (p<0.05). CONCLUSIONSThe results of the study confirm our hypothesis that the incidence of psychiatric comorbidity is higher in ex-prisoners of war.
In 2019 The Croatian Working Group for Laboratory Hematology, on behalf of the Croatian Society of Medical Biochemistry and Laboratory Medicine, wanted to explore the background in field of ...laboratory hematology routine practice among Croatian laboratories in order to develop future strategies for producing national recommendations, if needed.
During April and May 2019, a comprehensive survey covering all main parts of the total testing process within the field of laboratory hematology among Croatian medical laboratories was conducted. The survey comprised 49 inquiries. Data was collected using Survey Monkey (Palo Alto, CA, USA). All collected data was anonymized.
The response rate was 72%. There is still a substantial number of laboratories that have only three-part differential hematology analyzers (9%). Furthermore, a very high number of laboratories did not perform analyzer verification prior to implementation into routine work (31%). Out of those who have verified their analyzers, a diversity of guidelines and recommendations were used. Nearly 10% of the laboratories do not have a defined policy regarding specimen rejection. The majority of the participants perform internal quality control daily (83%), however, only 51% of respondents evaluate the agreement between different hematology analyzers on daily basis. Although more than 90% of Croatian laboratories have a defined policy regarding specimen rejection, only 61% of respondents continuously monitor quality indicators in routine practice.
The survey revealed substantial differences in all aspects of laboratory hematology practices among Croatian medical laboratories, indicating the need for universal recommendations at the national level.
