Aim. To evaluate efficiency and tolerability of diabeton MB/metformin combination in patients failing to achieve optimal glycemic control when onmetformin monotherapy and prove advantages of this ...combination over combined low-dose therapy with glibenclamide and metformin. Materials and methods. The study included 464 patients with type 2 diabetes mellitus who poorly responded to metformin monotherapy. It was supplementedby diabeton MB. Efficiency and tolerability of combined treatment was evaluated from dynamics of glycemia and frequency of side-effects.40 patients were included in detailed comparative assessment (laboratory and instrumental, CGMS) of this monotherapy and fixed low-dose combinationof glibenclamide with metformin. Results. Results of comparison show that diabeton MB/metformin combination ensured most optimal glycemic control with a minimal risk of side effects. Conclusion. Diabeton MB/metformin combination is convenient, efficient and safe.
We discuss results of ORIGIN, a multicenter parallel groups study for efficiency assessment of insulin glargin against polyunsaturatedomega-3 fatty acids or placebo regarding cardiovascular and/or ...mortality risk reduction in patients with impaired fasting glycemia,impaired glucose tolerance or type 2 diabetes mellitus (T2DM) on its early stage and high risk for cardiovascular events. 12 537 patientstook part in this study; 6 264 were randomized in insulin glargin group, where dosage was adjusted for complete compensationof fasting glycemia (5.3 mmol/l was set as a therapeutic goal). After treatment with glargin therapeutic goal was achieved and furthermaintained for 6.2 years of follow-up. Compensation of fasting glycemia did not affect the outcome of cardiovascular diseases in patientswith early stages of dysglycemia according to primary endpoints. It was not associated with increase in general morbidity and inrisk of hypoglycemic events. Treatment with insulin glargin delayed progression from prediabetes to clinical onset for 28% (OR 0.72,CI 95% 0.58-0.91; p = 0,006), while lowering incidence of DM. Longtime treatment with insulin glargin does not increase incidenceof malignant tumors of different localization, including patients with prediabetes. Due to results of ORIGIN, insulin glargin (Lantus?)has become the most studied human insulin analogue to date.
Semaglutide is a representative of analogues of the incretin hormone human glucagon-like peptide-1 (GLP-1) and is currently used in Russia for the treatment of type 2 diabetes mellitus (T2DM; in ...monotherapy and in combination therapy), including patients with obesity and overweight.
The aim
of the work was to conduct a comparative assessment of the physicochemical properties, a biological activity, bioequivalence and safety, including tolerability and immunogenicity, of the drug Quincent® (semaglutide, 1.34 mg/ml, a solution for a subcutaneous administration, Promomed Rus LLC, Russia) and the drug Ozempic® (semaglutide, 1.34 mg/ml, a solution for a subcutaneous administration, Novo Nordisk A/S, Denmark) when administered to healthy volunteers.
Materials and methods.

(semaglutide, 1.34 mg/ml, a solution for a subcutaneous administration, Promomed Rus LLC, Russia) with a chemically synthesized active substance to the original (reference) drug Ozempic® (semaglutide, 1.34 mg/ml, a solution for a subcutaneous administration, Novo Nordisk A/S, Denmark), a comparative study of physicochemical properties and a biological activity was carried out. To assess the bioequivalence of the study drug and the reference drug, an open randomized parallel comparative study with the participation of healthy volunteers (

=54), 54 participants of which had been included in the population, was conducted. The volunteers were randomized into 2 groups in a 1:1 ratio, and received a single dose subcutaneously either of the study drug (domestic semaglutide at a dose of 0.5 mg) or the reference drug (foreign semaglutide at a dose of 0.5 mg). The mode of administration was in the morning on an empty stomach. A semaglutide concentration was determined in serum samples using a previously validated enzyme-linked immunosorbent assay (ELISA) method. A quantitative determination of antibodies to semaglutide in the human serum by ELISA was carried out with a microplate photometer using ready-made kits pre-validated by the manufacturer. Conclusion.
In the course of the study, the comparability of the physicochemical properties and biological activity of the studied Russian drug with the chemically synthesized active substance Quincenta® to the reference drug Ozempic® was confirmed: the activity range of the studied drugs was within 80–120% in relation to the standard sample of semaglutide. The bioequivalence and a similar safety profile, including the immunogenicity and tolerability of the Russian drug Quincenta® (semaglutide 1.34 mg/ml, Promomed Rus LLC, Russia) were shown in comparison with the foreign drug Ozempic® (semaglutide 1.34 mg/ml, Novo Nordisk A/C, Denmark).

Maintaining of target glycemic levels is the most important factor in a favorable outcome of COVID-19 in both type 1 and type 2 DM. The choice of antihyperglycemic therapy in a patient with DM in the acute period of COVID-19 depends on the initial therapy, the severity of hyperglycemia, the severity of the viral infection and the patient’s clinical condition.
The article presents the recommendations of the board of experts of the Russian Association of Endocrinologists on glycemic control and the choice of antihyperglycemic therapy in patients with type 2 DM and COVID-19, and also on the use of glucocorticosteroids used in the treatment of COVID-19 in patients with type 2 DM.
IntroductionMost data demonstrating the efficacy and safety of luseogliflozin (luseo) in people with type 2 diabetes mellitus (T2DM) originate from the Japanese population. This study evaluated luseo ...versus placebo (PCB) as add-on to metformin in a Caucasian population with inadequately controlled T2DM.Research design and methodsThis was a multicenter, randomized, double-blind, PCB-controlled, parallel-group study. Patients aged 18–75 years with inadequately controlled T2DM (glycated hemoglobin (HbA1c) ≥7% to ≤10% (≥53 to ≤86 mmol/mol)) despite a diet and exercise program and on a stable metformin regimen were eligible. Patients were randomized to one of three luseo groups (2.5, 5.0 and 10.0 mg) or PCB for 12 weeks (W12). The primary endpoint was change in HbA1c expressed as least-square means from baseline (W0) to W12.ResultsA total of 328 patients were randomized: PCB (n=83) and luseo 2.5 mg (n=80), 5.0 mg (n=86), and 10.0 mg (n=79). Mean age (±SD) was 58.5±8.8 years; 64.6% were women; body mass index was 31.5±3.4 kg/m2; and HbA1c was 8.54±0.70. At W12, mean reductions in HbA1c from W0 were −0.98%, −1.09%, −1.18%, and −0.73% in the luseo 2.5, 5.0 and 10.0 mg, and PCB groups, respectively, all of which were statistically significant. Compared with PCB, HbA1c levels were significantly decreased by 0.25% (p=0.045), 0.36% (p=0.006), and 0.45% (p=0.001) in the luseo 2.5, 5.0, and 10.0 mg groups, respectively. In all luseo dose groups, reductions in body weight were statistically significant compared with PCB. Data from the safety analysis were consistent with the known luseo safety profile.ConclusionsAll doses of luseo as add-on to metformin in Caucasian patients with uncontrolled T2DM demonstrated significant efficacy in decreasing HbA1c after W12 of treatment.Trial registration numberISRCTN39549850.

Цель. Разработка комплексного подхода к ведению пациентов с СД1 на инсулинотерапии с помощью инсулиновой помпы. Материалы и методы. С целью оценки и сравнения показателей гликемии и эффективности ...разных способов введения инсулина на фоне комбинаций препаратов инсулина нами было обследовано 37 больных СД1, которые были разделены на 3 группы. Пациенты группы 1 (11 больных) вводили аналоги человеческого инсулина с помощью инсулиновой помпы. В группах 2 (13 больных) и 3 (13 больных) использовался режим многократных инъекций инсулина с помощью шприц-ручки. Пациенты группы 2 вводили НПХ-инсулин и аналоги человеческого инсулина короткого действия. В группе 3 инсулинотерапия проводилась инсулином гларгин и аналогами человеческого инсулина короткого действия. При помощи системы постоянного контроля гликемии (CGMS, Medtronic Minimed, USA) у пациентов всех групп оценивались средний препрандиальный уровень гликемии, постпрандиальный уровень гликемии, частота гипогликемий, среднее общее значение уровня гликемии (длительность обследования составляла 4 дня), а также средние уровни гликированного гемоглобина (HbA1c) до начала коррекции терапии и через 12 недель наблюдения. Результаты. При изучении метаболизма глюкозы в течение 3 месяцев в группе 1 средний препрандиальный уровень гликемии cоставил 6,2?0,16 ммоль/л, в группе 2 ? 10,2?0,14 ммоль/л, в группе 3 ? 8,4?0,20 ммоль/л. Средний постпрандиальный уровень гликемии в группе 1 составил 7,36?0,02 ммоль/л, в группе 2 ? 11,6?0,20 ммоль/л, в группе 3 ? 9,9?0,18 ммоль/л. Среднее количество гипогликемий в группе 1 составило 1,2?0,12 эпизод, в группе 2 ? 3,8?0,11 эпизода, в группе 3 ? 3,2?0,10 эпизода за 4 дня наблюдения. Среднее суточное значение уровня гликемии в группе 1 составило 6,95?0,02 ммоль/л, в группе 2 ? 11,6?0,17 ммоль/л, в группе 3 ? 9,03?0,04 ммоль/л. Средний уровень гликированного гемоглобина HbA1c до терапии в группе 1 составил 11,68?0,61%, в группе 2 ? 12,13?0,51%, в группе 3 ? 11,50?2,28%. Средний уровень гликированного гемоглобина HbA1c после проведенной терапии в группе 1 составил 6,24?0,15%, в группе 2 ? 9,11?0,19%, в группе 3 ? 8,30?0,04%. Выводы. Проведенное исследование подтвердило эффективность и безопасность применения постоянной подкожной инфузии инсулина с помощью инсулиновой помпы у пациентов с СД1, а использование системы постоянного контроля гликемии позволило изучить индивидуальные особенности углеводного метаболизма у каждого конкретного пациента, выявить бессимптомно протекающие гипогликемии и, соответственно, подобрать адекватную инсулинотерапию. Помповая терапия позволяет наиболее точно приблизиться к физиологическим параметрам инсулинемии, о чем свидетельствуют показатели среднесуточного уровня гликемии, препрандиальных и постпрандиальных колебаний гликемии, количество зарегистрированных гипогликемий, а также уровень гликированного гемоглобина HbA1c после проведенной терапии, полученные в группе 1, которая превосходит по данным показателям группы 2 и 3.

Сочетание двух пероральных сахароснижающих препаратов в одной таблетке позволяет не изменять режим приема лекарств. Такие фиксированные комбинации уже используются для лечения других заболеваний, ...например артериальной гипертензии, но до сих пор редко применяются в диабетологии. Важно, чтобы составляющие обладали не только взаимодополняющим механизмом действия, но и сочетались по фармакокинетическому профилю. Одним из первых таких препаратов стал Глибомет производства компании ?Берлин Хеми?, содержащий в своем составе фиксированную комбинацию 2,5 мг глибенкламида и 400 мг метформина. Сочетание глибенкламида, стимулирующего секрецию инсулина и метформина, повышающего чувствительность тканей к его действию, обеспечивает воздействие на оба патогенных механизма заболевания. Кроме того, фармакокинетические параметры позволяют применять оба препарата дважды в день.
Цель. Оценить применение препаратов тестостерона при андрогенном дефиците у мужчин с сахарным диабетом 2 типа. Материалы и методы. В исследовании приняли участие 92 пациента, 46 (1-я группа) из ...которых получали ?Андрогель? (трансдермальная форма тестостерона) в дозе 5,0 мг. 46 пациентов (2-я группа) получали тестостерон ундеканоат (небидо?) по 1000 мг за инъекцию. Определяли уровень тестостерона, парралельно исследовали уровень глобулина, связывающего половые стероиды (ГСПС). Для исключения гипогонадотропного гипогонадизма определяли уровни ЛГ, ФСГ и эстрадиола в сыворотке крови. Тяжесть ЭД определяли по Международному индексу эректильной функции. Всем пациентам выполнено трансректальное УЗИ предстательной железы. Результаты. Независимо от получаемой терапии у всех пациентов отмечено улучшение сексуальной активности, повышение либидо, снижение общей массы тела и процента жировой ткани. Средний уровень PSA в 1-й группе незначительно возрос. Отмечены позитивные сдвиги в липидном спектре и уровне гликированного гемоглобина. Индекс эректильной функции увеличился и составил в среднем в 1-й группе 20,1?5,3, а во 2-й группе ? 19,8?5,8 балла. Выводы. Для лечения андрогенного дефицита у больных СД 2 типа могут с успехом применяться оба препарата тестостерона. Они позитивно влияют на липидный спектр, массу тела, компенсацию СД, остеопороз, сексуальную функцию и настроение. В обязательное исследование больных СД 2 типа необходимо включить определение уровня тестостерона. Пациенты, получающие терапию гелем тестостерона, должны иметь более серьезную мотивацию.
