The two volume set LNCS 7439 and 7440 comprises the proceedings of the 12th International Conference on Algorithms and Architectures for Parallel Processing, ICA3PP 2012, as well as some workshop ...papers of the CDCN 2012 workshop which was held in conjunction with this conference. The 40 regular paper and 26 short papers included in these proceedings were carefully reviewed and selected from 156 submissions. The CDCN workshop attracted a total of 19 original submissions, 8 of which are included in part II of these proceedings. The papers cover many dimensions of parallel algorithms and architectures, encompassing fundamental theoretical approaches, practical experimental results, and commercial components and systems.

Purpose - Hybrid cloud computing is considered a viable and cost-effective approach to satisfy the inability of a private cloud to meet the user requirements. The information status of the selected ...public cloud service may change at runtime which should be reflected at the broker server database. This research illustrates a mechanism to assist the IaaS discovery system to assess the status change of the selected public cloud service and to update the broker server database. The paper aims to discuss these issues. Design/methodology/approach - A prototype was developed with the broker server as the main component in the service discovery system containing the status information of the selected public cloud service. The merge-ontology and patch-update methods were proposed, the processing cost details of each were measured, and the methods were evaluated. Findings - Experimental results showed that in the merge update, the merging process incurred much longer time than its required communication, contributing to long overall time. Relatively, the patch update method incurred much less time than its counterpart. Research limitations/implications - The proposed mechanism is experimental with some ideal assumptions, and so further work in real conditions is needed for its improvement. Originality/value - This research is believed to be the first proposal to investigate ontology merge/patch methods to support ontology update in the broker server database of a hybrid cloud and will serve as a reference to researchers in the field.
Establish recommendations for the management of UTIs in MS patients.
Urinary tract infections (UTIs) are common during multiple sclerosis (MS) and are one of the most common comorbidities potentially ...responsible for deaths from urinary sepsis.
The recommendations attempt to answer three main questions about UTIs and MS. The French Group for Recommendations in MS (France4MS) did a systematic review of articles from PubMed and universities databases (01/1980–12/2019). The RAND/UCLA appropriateness method, which has been developed to synthesize the scientific literature and expert opinions on health care topics, was used for reaching a formal agreement. 26 MS experts worked on the full-text review and a group of 70 multidisciplinary health care specialists validated the final evaluation of summarized evidences.
UTIs are not associated with an increased risk of relapse and permanent worsening of disability. Only febrile UTIs worsen transient disability through the Uhthoff phenomenon. Some immunosuppressive treatments increase the risk of UTIs in MS patients and require special attention especially in case of hypogammaglobulinemia. Experts recommend to treat UTIs in patients with MS, according to recommendations of the general population. Prevention of recurrent UTIs requires stabilization of the neurogenic bladder. In some cases, weekly oral cycling antibiotics can be proposed after specialist advice. Asymptomatic bacteriuria should not be screened for or treated systematically except in special cases (pregnancy and invasive urological procedures).
Physicians and patients should be aware of the updated recommendations for UTis and MS.
Purpose To determine whether reversal of DWI lesions (DWIr) on the DWI-ASPECTS (diffusion weighted imaging Alberta Stroke Program CT Score) template should serve as a predictor of 90-day clinical ...outcome in acute ischemic stroke (AIS) patients with pretreatment diffusion-weighted imaging (DWI)-ASPECTS 0-5 treated with thrombectomy, and to determine its predictors in current practice. Methods We analyzed data of all consecutive patients included in the prospective multicenter national Endovascular Treatment in Ischemic Stroke Registry between 1 January 2015 and 31 December 2020 with a premorbid mRS less than or equal toâ¯2, who presented with a pretreatment DWI-ASPECTS 0-5 score, underwent thrombectomy and had an available 24â¯h post-interventional MRI follow-up. Multivariable analyses were performed to evaluate the clinical impact of DWIr on early neurological improvement (ENI), 3âmonth modified Rankin scale (mRS) score distribution (shift analysis) and to define independent predictors of DWIr. Results Early neurological improvement was detected in 82/211 (41.7%) of patients while 3âmonth functional independence was achieved by 75 (35.5%) patients. The DWI reversal (39/211, 18.9%) resulted an independent predictor of both ENI (aOR 3.6, 95% CI 1.2-7.7; p 0.018) and 3âmonth clinical outcome (aOR for mRS shift: 2.2, 95% CI 1-4.6; p 0.030). Only successful recanalization (mTICI 2c-3) independently predicted DWIr in the studied population (aOR 3.3, 95% CI 1.3-7.9; p 0.009). Conclusion The DWI reversal occurs in a non-negligible proportion of DWI-ASPECTS 0-5 patients subjected to thrombectomy and significantly influences clinical outcome. The mTICI 2c-3 recanalization emerged as an independent DWIr predictor.

Évaluer la fréquence des formes inflammatoires d’AA, au sein du recrutement hospitalier et de la consultation mémoire d’un CHG.
Étude rétrospective sur la période de 2008 à 2014. Nous avons recensé tous les cas d’AA, sur la base du codage des pathologies (PMSI) et étendu notre recherche aux cas de démences vasculaires recensés dans la base de données Calliope (consultations « mémoire »). Afin de vérifier le diagnostic d’AA, nous avons appliqué les critères de Boston modifiés et les critères proposés par Chung et al. pour le diagnostic d’ABRA.
Soixante-huit cas d’AA probables et 2 cas d’AA possibles furent recensés, à partir du PMSI, ainsi que 35 autres cas d’AA probables, à partir des 156 cas de démences vasculaires issus de la base de données Calliope. Parmi eux, 6 cas d’ABRA furent identifiés. Les signes d’appel étaient les suivants : crises comitiales (2 cas), confusion/troubles du langage (2 cas), céphalées/gêne visuelle (1 cas) et confusion/trouble visuel (1 cas). L’évolution a toujours été spontanément favorable.
La fréquence de l’ABRA n’est pas connue. En 2014, 94 cas étaient recensés dans la littérature. Des critères cliniques (début aigu, pas d’autre cause, âge ; 40ans, céphalée/trouble cognitif ou du comportement/crise ou déficit focal) et radiologiques (AA avec hypersignal asymétrique en FLAIR) ont été proposés (Chung et al.). Si un traitement est souvent proposé, certaines formes sont d’évolution spontanément favorable.
Une confusion, une crise comitiale, un trouble visuel ou du langage chez un patient souffrant d’une AA doit faire rechercher une ABRA. La fréquence de cette affection paraît sous-estimée, mais reste à déterminer.

Introduction
It is important to confirm the effectiveness and tolerability of disease-modifying treatments for relapsing–remitting multiple sclerosis (RRMS) in real-world treatment settings. This ...prospective observational cohort study (VIRGILE) was performed at the request of the French health authorities. The primary objective was to evaluate the effectiveness of fingolimod 0.5 mg in reducing the annualised relapse rate (ARR) in patients with RRMS.
Methods
Participating neurologists enrolled all adult patients with RRMS starting fingolimod treatment between 2014 and 2016, who were followed for 3 years. Follow-up consultations took place at the investigator’s discretion. The primary outcome measure was the change in ARR at month 24 after fingolimod initiation. Relapses and adverse events were documented at each consultation; disability assessment (EDSS) and magnetic resonance imagery were performed at the investigator’s discretion.
Results
Of 1055 eligible patients, 633 patients were assessable at month 36; 405 (64.0%) were treated continuously with fingolimod for 3 years. The ARR decreased from 0.92 ± 0.92 at inclusion to 0.31 ± 0.51 at month 24, a significant reduction of 0.58 95% CI − 0.51 to − 0.65 relapses/year (
p
< 0.001). Since starting fingolimod, 461 patients (60.9%) remained relapse-free at month 24 and 366 patients (55.5%) at month 36. In multivariate analysis, no previous disease-modifying treatment, number of relapses in the previous year and lower EDSS score at inclusion were associated with a greater on-treatment reduction in ARR. The mean EDSS score remained stable over the course of the study. Sixty-one out of 289 (21.1%) patients presented new radiological signs of disease activity. Treatment-related serious adverse events were lymphopenia (

Conclusions
The effectiveness and tolerability of fingolimod in everyday clinical practice are consistent with findings of previous phase III studies. Our study highlights the utility of fingolimod for the long-term management of patients with multiple sclerosis.
