Introduction Le myxœdème prétibial (MP) est une des manifestations extra-thyroïdiennes de la maladie de Basedow (MdB). Sa prévalence est estimée de 0,4 à 4 % dans les années 1980. Cette dermatopathie ...est habituellement paucisymptomatique et ne nécessite pas de traitement. En cas de gêne ou dysesthésie les dermocorticoïdes améliorent les lésions dans environ 50 % des cas. Les traitements de 2e intention sont : injections intra-lésionnelles de glucorticoïdes, Octréotide, Pentoxyfilline, Immunoglobulines polyvalentes, plasmaphérèses. Observation Nous rapportons le cas d’une patiente avec MdB diagnostiquée à 51 ans rapidement traitée par thyroïdectomie. Son hyperthyroïdie récidiva 3 ans plus tard sur persistance d’un résidu thyroïdien et elle développa une orbitopathie sévère. Elle bénéficia de bolus de Solumédrol puis d’une Ira-thérapie sous couvert d’une corticothérapie orale. Un MP apparu 8 ans après le diagnostic de MdB, il fut traité par dermocorticoïdes puis Colchicine sans efficacité. Un traitement par 3 bolus de Solumédrol fut efficace mais rapidement suivi d’un effet rebond. Son MP s’aggrava avec apparition de douleurs neuropathiques. Trois cures d’immunoglobulines polyvalentes furent faiblement efficaces. Des plasmaphérèses bi-hebdomadaire furent initiées, sans grande efficacité. Enfin une cure de rituximab (1 g répété à 15 jours) en association avec les plasmaphérèses hebdomadaires permit une nette amélioration du MP. Discussion Ce cas mérite d’être rapporté devant la complexité de la présentation et l’efficacité du traitement par Rituximab. 2 cas de MP sévère et résistant traités avec efficacité par rituximab ont été décrits dans la littérature 1,2.
Le syndrome de Hoigné est une complication classique des injections intramusculaires de procaïne pénicilline caractérisée par la survenue de manifestions aiguës essentiellement psychosensorielles.
...Nous rapportons la survenue d’un syndrome de Hoigné au décours immédiat d’une injection intraveineuse de ceftriaxone ; il s’agit à notre connaissance du premier cas décrit avec cet antibiotique.
Nous discutons les mécanismes physiopathologiques qui sous-tendent ce syndrome. Il importe de garder à l’esprit les manifestations cliniques du syndrome de Hoigné, qui pourraient être confondues avec des manifestations de type allergique, alors qu’elles n’interdisent pas la reprise ultérieure du traitement.
Hoigne's syndrome is characterized by the development of acute clinical manifestations which are mainly psycho-sensorial. Classically, these features immediately follow the injection of procaine penicillin G.
We report a 59-year-old man who presented with psycho-organic manifestations that occurred just after the intravenous injection of ceftriaxone; to our knowledge, this is the first case of Hoigne's syndrome reported after an injection of this antibiotic.
The pathophysiologic basis of this syndrome is still unknown. It is important to keep in mind its clinical characteristics, which may mimic immuno-allergic symptoms. It should be differentiated from anaphylactic manifestations because Hoigne's syndrome allows the continuation of the treatment.
Anti-transcriptional intermediary factor-1γ (TIF-1γ) autoantibody may be associated with cancer in adult patients with dermatomyositis. The aim of this study was to evaluate the risk of cancer in the ...presence of anti-TIF-1γ autoantibody in adult dermatomyositis. A comprehensive database search of EMBASE, MEDLINE and the Cochrane Library up to May 2018 was performed using the main key words "dermatomyositis", ""myositis", "inflammatory myopathies" and "anti-TIF-1". Eighteen studies, with a total of 1,962 dermatomyositis, were included in the meta-analysis. The pooled prevalence of cancer-associated dermatomyositis in patients with anti-TIF-1γ autoantibody was 0.41 (95% confidence interval (CI) 0.36-0.45). In the presence of anti-TIF-1γ autoantibody, the overall diagnostic odds ratio of cancer was 9.37 (95% CI 5.37-16.34) with low heterogeneity (Cochran's Q: 14.88 (df = 17, p = 0.604); I2 = 0%). The results of this systematic review confirm that detection of anti-TIF-1γ autoantibody is a valuable tool to identify a subset of adult dermatomyositis patients with higher risk of cancer.

Neuronal activity in the brain generates synchronous oscillations of the Local Field Potential (LFP). The traditional analyses of the LFPs are based on decomposing the signal into simpler components, ...such as sinusoidal harmonics. However, a common drawback of such methods is that the decomposition primitives are usually presumed from the onset, which may bias our understanding of the signal's structure. Here, we introduce an alternative approach that allows an impartial, high resolution, hands-off decomposition of the brain waves into a small number of discrete, frequency-modulated oscillatory processes, which we call oscillons. In particular, we demonstrate that mouse hippocampal LFP contain a single oscillon that occupies the θ-frequency band and a couple of γ-oscillons that correspond, respectively, to slow and fast γ-waves. Since the oscillons were identified empirically, they may represent the actual, physical structure of synchronous oscillations in neuronal ensembles, whereas Fourier-defined "brain waves" are nothing but poorly resolved oscillons.
