Of all the neoplastic sites, colorectal cancer (CRC) is one of the most common cancers in the family. Studies estimate that approximately 30% of all the CRC cases are a hereditary form of the disease ...with a potentially high impact on the quality of life (QoL), with a high risk of recurrence, and with bio–psycho–social functioning. The aim of the study was to assess the QoL of colorectal cancer patients with genetic risk by using the European Organisation for Research and Treatment of Cancer (EORTC) questionnaire version QLQ-CR29. Materials and methods: The cross-sectional, single-center study was performed on a group of 32 patients with genetic risk of colorectal cancer, who underwent preoperative chemoradiation and surgery. Results: The series of values for the scores on the symptom scale varied between 15 and 30, and the average level of symptom scores did not differ significantly between gender (22.0 vs. 22.75; p = 0.636), highlighting a moderate impairment of QoL. Scores for the emotional functioning scale were significantly lower in men (10.33 vs. 13.25; p = 0.049), as were the scores for the physical functions (15.67 vs. 19.15; p = 0.039), showing a decrease in QoL. Conclusions: The overall score showed an average QoL in patients with colorectal cancer with genetic risk, highlighting significant differences in psycho-emotional functioning between women and men.
Objectif. L’objectif de notre étude est d’évaluer la pression artérielle et la pression différentielle chez les patients atteints du syndrome métabolique (SM), afin de déceler et de prévenir les ...complications cardiovasculaires qui peuvent survenir chez ces patients. Matériels et méthodes. L’analyse des cas‑témoin a été faite sur un total de 1528 patients, dans un cabinet de médecine familiale, les sujets étant répartis en deux groupes: le lot avec SM et le groupe témoin (sans SM). Le groupe avec SM se composait de 388 patients diagnostiqués selon les critères harmonisés. Le groupe témoin sans SM était composé de 1140 patients n’ayant pas présenté au moins trois critères diagnostiques du syndrome métabolique. Résultats. La pression différentielle, les valeurs calculées pour les patients atteints de syndrome métabolique ont été généralement plus élevées que le taux moyen escompté à 40 mmHg, avec une valeur moyenne de 47,86 ± 12,02 mmHg. Le groupe de contrôle a une valeur moyenne (40,28 ±12.20 mmHg) du pouls, une pression significativement plus faible (p<0.0001). Conclusions. La tension différentielle est associée aux complications cardiovasculaires positives, en particulier chez les personnes âgées et surtout aux valeurs supérieure ou égale à 60 mmHg, mais cette association n’est pas indépendante de la tension artérielle.
The purpose of this study was to evaluate the in vivo biological potential of new azetidine-2-one derivatives of ferulic acid (6a–f). First, the in vivo acute toxicity of azetidine-2-one derivatives ...of ferulic acid on Swiss white mice was investigated and, based on the obtained results, it can be stated that the studied derivatives belong to compounds with moderate toxicity. The in vivo anti-inflammatory potential of these derivatives was determined in a model of acute inflammation induced by carrageenan in rats and in a chronic inflammation model induced in rats using the granuloma test. In the acute inflammation model, all the studied compounds had a maximum anti-inflammatory effect 24 h after administration, which suggests that these compounds may be classified, from a pharmacokinetic point of view, in the category of long-acting compounds. The most active compound in the series was found to be compound 6b. In the case of the chronic inflammation model, it was observed that the studied compounds (6a–f) reduced the formation of granulation tissue compared to the control group, having an intense effect of inhibiting the proliferative component. The most important inhibitory effect of inhibiting the proliferative component was recorded for compound 6b. Additionally, the investigation of liver function was performed by determining the serum levels of liver enzymes aspartate transaminase (AST), alanine aminotransferase (ALT), lactate dehydrogenase (LDH) and bilirubin (total and direct). The results showed that, in the series of azetidin-2-one derivatives, the liver enzymes concentration values were close to those recorded for the reference anti-inflammatories (diclofenac sodium and indomethacin) and slightly higher compared to the values for the healthy control group. At the end of the experiment, the animals were euthanized and fragments of liver, lung, and kidney tissue were taken from all groups in the study. These were processed for histopathological examination, and we noticed no major changes in the groups treated with the azetidine 2-one derivatives of ferulic acid compared to the healthy groups.
This paper aims to present a retrospective analysis of activity from Palliative Care Compartment of Regional Institute of Oncology Iasi during 12 months of activity and to propose a model of ...integration of the palliative care in oncology.
Data were collected from 415 patients hospitalized to the Palliative Care Compartment using the computerized database, records and books of existing appointments.
Data analysis reveals that 170 cases (40%) were aged higher or equal to 70 years and a number of 142 patients (34%) were aged between 60-69 years. Results regarding the provenience of the patients shows that most of them are from urban areas. Gastrointestinal neoplasms were present at about one-third of the patients--133 cases (32%) and 95 (71%) of these cases had metastases. Most patients--114 cases (27%) required hospitalization for a period between 8 and 14 days. Palliative care has interdisciplinary relations with other specialties. In 2013 from other specialties were transferred to palliative care a number of 156 patients, most of them (87 cases) from Oncology Department.
Integrating palliative care in oncology would increase the quality of life of patients, would relieve the other sections or hospitals by patients who need palliative care, would decrease hospitalization costs and would avoid performing aggressive maneuvers at the end of life.
The objective was to assess the role of negative pressure wound therapy (NPWT) in the management of wounds of various etiologies, infected or not, acute or chronic. The study was conducted in a group ...of 37 patients (24 men and 13 women) aged 26 to 86 years with acute or chronic wounds, posttraumatic or due to chronic diseases located on the lower limbs or following oncological abdominal surgery. In all cases a Vacuum Assisted Closure (VAC) system was continuously applied using a subatmospheric pressure ranging from 90 to 120 mmHg. Beside sex, age and etiology of soft tissue defect the following parameters were studied: surgical treatment performed prior to NPWT application, frequency of dressing changes, duration of NPWT, level of subatmospheric pressure used in each case, number of hospital days, complications arising from NPWT use. The outcome was favorable in all cases without such complications as infection or bleeding, a perilesional erythema (contact dermatitis caused by the used adhesive tape) being noted in only 13.51% of cases. The average duration of NPWT was 8.1 days. The frequency of dressing changes was in most cases of 3 days (89.18%) with a group mean of 3.05 days, and the average applied negative pressure was -110.83 mmHg, range -90mmHg to -120 mm Hg. No complications requiring the interruption of NPWT use were recorded. NPWT, by stimulating wound constriction and granulation tissue formation is an option in the management of acute or chronic wounds of various etiologies, whether infected or not, resulting in a decrease in the number of surgeries and their complexity, length of hospital stay and, last but not least, reduced patient suffering.
Introduction: Considering the generalized Internet access as well as the electronic communication, teens and young adults are at risk for a new type of dependency, the Internet addiction, with a high ...impact on the personal and professional life, as well as on the school performance. Objective: The current paper aims to introduce in the clinical practice a validated diagnosis tool for this new condition, allowing the proper identification of the problem cases for counselling and specific approach by the Family Physician and the Child Psychiatrist. Method: The IAT (Internet Addiction Test), created by Kimberly Young, was selected and applied on two small groups of patients from a Family Medicine practice (Group A) and a Child Psychiatry practice (Group B) from an urban setting, city of Iasi. Results: Accordingly to the test standard interpretation, the patients have been divided in: average user (20-49p)--28 cases (70%), occasional or frequent problem-presenting user (50-79p)--11 cases (27,5%), significant problem-presenting user (80-100p)--1 case (2,5%). The group of patients presenting to the Child Psychiatry practice for other issues had a higher incidence of Internet addiction problems. Conclusions: The Family Physician must screen for Internet-associated problems to discover early stage excessive users. The validated screening tools are useful in the current practice and the specialty counselling is extremely important for this specific population. The aim of this study was to assess the basic biochemical and clinical characteristics of patients with hypertension and type 2 diabetes mellitus (T2DM), office blood pressure (BP) and 24-h BP ...profile, their risk factors and associated comorbidities. Compared with non-diabetics, hypertensive patients with T2DM were older, with a longer duration of hypertension (5.9 vs. 4.7 years), greater office blood pressure and ambulatory BP values, increased incidence of multiple risk factors, target organ damage and cardiovascular disease. Biochemical data in hypertension with T2DM revealed significantly high levels of LDL cholesterol, triglycerides, creatinine, micro- and macro-albuminuria and a reduced estimated glomerular filtration rate. The presence of diabetes was associated with obesity, represented by a BMI 30 kg/m2 (OR 2.08 95% CI 1.26-3.45, p = 0.004), abdominal obesity (OR 1.85 95% CI 1.11-3.04, p = 0.016), high LDL cholesterol (OR 2.02 95% CI 1.22-3.35, p = 0.006) and high triglycerides (OR 1.86 95% CI 1.11-3.11, p = 0.017).

Society has undergone many changes in the last decades with a direct and profound impact on the physician-patient relationship. This paper aims to analyze different communication models existing in ...the literature, as described up to now in different empirical studies. The issue of the communication in the oncological practice is dominated by two aspects: to tell or not to tell the diagnostic to the patient and if the answer is affirmative, the manner in which the diagnostic must be communicated. A large number of patients asserted that they prefer the physician to take their opinions into account when making a decision, reminding therefore the controversial nature of what is best for a patient. It is considered that the information offered to the patients as well as patients’ autonomy were key variables in the communication models, having a powerful impact on the patient-physician interaction. The physicians manifesting an authentic interest in the patient as person, who transmit an authentic empathy, who provide adequate and advisable information represent a source of social support for the patients. By giving the patient the opportunity to choose between his own preferences and those of his physician, the circumstances for exercising his autonomy are ensured.
The connection between oxidative stress and ocular injuries was highly debated, given the fact that, as a result of its particular structure, the eye may have different reactions when exposed to ...oxidative stress and visual activity and functionality are considerable compromised. Our experimental research focused on two types of induced oxidative stress on rats (disruption of circadian rhythm and diabetes mellitus) with impact on biochemical markers of oxidative stress. We have analysed the amplitude of significant antioxidant markers correlated with each individual type of antioxidant disorder. We observed alterations of stress biomarkers for all induced types of stress, but the most relevant are associated with diabetes mellitus and light exposure.
