Background: In the course of our research on polymorphism in the UGT1A1 gene of healthy full-term newborns, we studied the relationship between pathological jaundice and the newborn's genotype. The ...latter could be one of the risk factors for hyperbilirubinemia. The possibility of a pharmacogenetic approach for treating individuals with Gilbert's syndrome was investigated since the polymorphism may influence the toxic metabolism of certain therapeutic drugs. Methods: Blood was collected from 185 healthy full-term newborns, born in May 2009, on the third day of life. Bilirubin was measured according to the Jendrassik-Grof method and all samples were genotyped (dHPLC method). ANOVA, χ2-test and t - test were used for statistical analysis. Results: A significant association of genotype with bilirubin levels was observed (p< 0.001), the highest bilirubin level occurring with the genotype (TA)7/7, characteristic of Gilbert 's syndrome; 68 of the 185 newborns had pathological jaundice; 20 cases were so severe that they needed phototherapy during hospitalization. Differences in bilirubin levels between the groups with clinical characteristics of jaundice were statistically significant (physiological / pathological (p< 0.001) and irradiated / nonirradiated (p = 0,034)). The genotype (TA)7/7 was significantly more frequent in the group with pathological jaundice than in the group with physiological jaundice (p = 0,028). The aims of this study were: (1) to perform the Amiel-Tison Neurological Assessment (ATNA) in a group of infants with different risk factors for brain damage; (2) to analyze the results of the ...examinations in light of the risk factors and presumed aetiology; (3) to compare results of examinations with results of cranial ultrasound, electroencephalography (EEG), and cerebral function monitoring (CFM); and (4) to evaluate neurological outcome at 12 to 15 months of age using the Amiel-Tison and Gosselin method, and developmental outcome using the Bayley Scales of Infant Development. Participants were 52 term, newborn infants (31 males, 21 females) with risk factors for brain damage. Mean birthweight was 3288g (SD 661g) and mean gestational age was 39.4wks (SD 1.2wks); range 38 to 41.3wks. Mean age at admission to a neonatal special care unit was 75h, (SD 13.7h). The group with a dynamic (evolving) clinical profile differed significantly from the group with a static (stable) profile in terms of aetiology, while the group with signs of prenatal brain damage differed from the group without these signs regarding aetiology and the level of severity of neurological signs. Sensitivity of the ATNA to detect infants with abnormal ultrasound was 0.97, with EEG 0.89, and with CFM 0.88. At follow-up at 12 to 15 months 47 children were examined: neurological examination was normal in 25 and five children had a minor, five a moderate, and 12 a severe neurological deficit. Agreement of the ATNA with neurological and developmental assessment at follow-up was very good. Our findings suggest that the ATNA is also of value in assessing aetiology and timing of brain lesions.

Objectives: To evaluate carboxyhemoglobin (COHb) values measured with a CO-oximeter (Roche-cobas b 221) in jaundiced newborns with or without hemolysis and healthy controls in order to assess whether ...COHb measurement determined with a CO-oximeter could be used as an indicator of hemolysis in newborns with ABO alloimmunization.
Methods: A total of 86 term newborn infants were prospectively studied. The study cohort consisted of three subgroups: 18 infants with ABO HDN, 21 infants with hyperbilirubinemia without hemolytic disease who required phototherapy, and 47 healthy controls. The COHb, bilirubin, and Hb levels were measured.
Results: The three subgroups did not differ significantly with respect to birth weight, gestational age, gender, Apgar score, or mode of delivery. The ABO HDN infants had significantly higher COHb values than the healthy controls (median 2.4% versus 1.3%, p < 0.0005) and the group with hyperbilirubinemia without hemolytic disease (median 2.4% versus 1.3%, p < 0.0005), although the infants with hyperbilirubinemia without hemolytic disease did not have significantly higher COHb values compared with the healthy controls. The cut-off value of 1.7% COHb had 72% sensitivity and 97% specificity for confirming hemolysis in ABO alloimmunization.
Conclusions: Our data show that COHb values determined with CO-oximeters are higher in newborns with hemolysis than in those without hemolysis. COHb measured with CO-oximeters could be used to confirm hemolysis in infants with ABO alloimmunization.
Background: Bilirubin is an antioxidant that protects newborns against free radicals during the first days of their lives, but at high levels is neurotoxic and causes brain damage. In this study we ...aimed to determine whether the measurement of bilirubin level in noninvasively taken cord blood could be a predictor of a pathological jaundice requiring medical treatment. Methods: Cord blood and a control sample taken on the third day of life were taken from 185 healthy full-term newborns of both genders. Bilirubin levels were determined according to the Jandrassik-Grof method. Statistical characteristics were calculated using statistic programs ANOVA and t-test. Results: 117 (63.2 %) of the newborns had physiological jaundice with 28 ± 7.8 μmol/L bilirubin in cord blood, and 68 (36.8 %) had pathological jaundice with 35 ± 7.8 μmol/L bilirubin. 20 (10.8 %) newborns had been treated by phototherapy, and had levels of 40 ± 8.5 μmol/L bilirubin. Differences in cord blood bilirubin levels between groups were statistically significant (p < 0.001). Taking a cut-off value of 28 μmol/L cord blood bilirubin level, pathological jaundice can be predicted by 85.3 % and whether newborns require treatment or not by 100 % clinical sensitivity. Conclusions: The results of our study confirm that measurement of cord blood bilirubin level can be used as a screening test for predicting the development of pathological jaundice that requires medical treatment.

Majčino mlijeko ima važnu ulogu u razvoju crijevne mikrobiote, te u zaštiti dojenčadi od patogenih mikroorganizama. Svrha je ovoga rada bila istražiti mikrobni sastav majčinog mlijeka, uzorkovanog iz ...lijeve (L) i desne (R) dojke 47 dojilja, tridesetog dana nakon poroda. Kvantificirane su neke glavne skupine bakterija u majčinom mlijeku, uspoređene kultivabilne bakterije iz lijeve i desne dojke, te je identificirana raznolikost sojeva laktobacila. Rezultati pokazuju da majčino mlijeko sadržava bakterije mliječne kiseline, bifidobakterije i mezofilne aerobne bakterije, od kojih je posljednja skupina najviše zastupljena. Iako je mikrobiološki sastav majčinog mlijeka u uzorcima L i R bio usporediv, koncentracija se bakterija u ta dva uzorka od iste majke mogla razlikovati, pa uzorak mlijeka uzet samo iz jedne dojke ne odražava prosječni mikrobiološki sastav. Na osnovi rezultata dobivenih metodom RAPD (engl. random amplified polymorphic DNA), 86 je bakterijskih sojeva za koje se pretpostavljalo da pripadaju rodu Lactobacillus razvrstano u jedanaest skupina, te identificirano sekvenciranjem 16S rDNA. Od jedanaest analiziranih skupina izolata, za njih je četiri (21 % svih sojeva) ustanovljeno da pripadaju vrsti Lactobacillus gasseri. Utvrđeno je da 48 % izolata najzastupljenijeg profila RAPD pripada vrsti Lactobacillus fermentum, a u preostalim su skupinama izolata identificirane vrste L. salivarius, L. reuteri, Enterococcus faecium, Staphylococcus epidermidis i Bifidobacterium breve.
