When facilitating mental health interventions, therapists typically involve clients in discussion within a room containing seating and a table. We argue that digital technologies can be leveraged to ...encourage physiological, multisensory experiences for users to work through their challenges. In the context of trauma in mental healthcare, where the body's involvement can play a critical role in the recovery journey, such an approach can offer potential in altering the dynamic of how interventions are delivered and received. This infers a client-led process where environment and features become inclusive to a holistic treatment pathway. We developed an intervention called The Timeline, situated in an immersive interactive virtual environment (IIVE). The iterative process was informed by interviews with 12 “Experts by Profession”, as frontline mental health workers with an average engagement of 16 years. We then invited 12 “Experts by Experience”, with lived understanding of a range of trauma, to trial and evaluate the system at a UK-based hospital. We report on data from participants across both studies, evidencing how The Timeline, as bespoke content, could lead to participatory choice and personalized control in the context of digital therapy. Finally, we discuss how IIVEs could become a new platform towards a more integrative therapeutic delivery in mental healthcare.
Context: The Ras effector NORE1A (RASSF5A) is a putative tumor suppressor and is inactivated in several human cancers. NORE1A has not been studied in thyroid cancer.
Objective: The objective of this ...study was to investigate whether NORE1A is involved in follicular thyroid cancer (FTC) development.
Design: We analyzed NORE1A expression in 25 FTCs, eight follicular thyroid adenomas, and seven normal thyroid tissues by TaqMan quantitative RT-PCR. The results were evaluated in relation to RASSF1A expression, RAS mutations, and PAX8-PPARγ fusions assessed in the same material. NORE1A promoter methylation was assessed by the combined bisulfite restriction endonuclease assay.
Results: Although the NORE1A mRNA levels of the majority of the tumors were similar to those in the normal controls, the cases harboring a PAX8-PPARγ translocation (n = 6) exhibited dramatically reduced NORE1A expression (P < 0.001). In contrast, RAS mutations (n = 5) and NORE1A down-regulation were mutually exclusive. A significant reduction in the expression of the NORE1A homolog and the bona fide tumor suppressor gene RASSF1A was observed, but with weak correlation to the respective NORE1A values. No NORE1A promoter methylation was detected in the 32 thyroid tumors analyzed.
Conclusions: Our experiments demonstrate the suppression of NORE1A, a known Ras effector, in PAX8-PPARγ carrying FTCs.
As the permafrost region warms, its large organic carbon pool will be increasingly vulnerable to decomposition, combustion, and hydrologic export. Models predict that some portion of this release ...will be offset by increased production of Arctic and boreal biomass; however, the lack of robust estimates of net carbon balance increases the risk of further overshooting international emissions targets. Precise empirical or model-based assessments of the critical factors driving carbon balance are unlikely in the near future, so to address this gap, we present estimates from 98 permafrost-region experts of the response of biomass, wild␣re, and hydrologic carbon ␣ux to climate change. Results suggest that contrary to model projections, total permafrost-region biomass could decrease due to water stress and disturbance, factors that are not adequately incorporated in current models. Assessments indicate that end-of-the-century organic carbon release from Arctic rivers and collapsing coastlines could increase by 75% while carbon loss via burning could increase four-fold. Experts identi␣ed water balance, shifts in vegetation community, and permafrost degradation as the key sources of uncertainty in predicting future system response. In combination with previous ␣ndings, results suggest the permafrost region will become a carbon source to the atmosphere by 2100 regardless of warming scenario but that 65%–85% of permafrost carbon release can still be avoided if human emissions are actively reduced.
The ecology of the St Lucia estuary in South Africa is of unique international importance. During droughts the estuary experiences high salinities, with values above that of seawater. Ion-poor ...groundwater flowing into the estuary from prominent sand aquifers along its eastern shoreline forms low-salinity habitats for salt-sensitive biota. During droughts, plants and animals can take refuge in the groundwater discharge zone until the condition in the estuary regains tolerable salinity. Simulations of the groundwater discharge indicate that the flow can persist during droughts over at least a decade, and be of great important for the resilience of the estuary. Anthropogenic activities have reduced the river inflow and made the St Lucia estuary more sensitive to droughts. The groundwater has thereby become increasingly important for the estuary's ecology. Protection of the groundwater discharge along the shoreline itself and actions to increase the groundwater recharge are therefore important management tasks.
The distribution and lineage of vascular cell adhesion molecule-1 (VCAM-1)-positive cells was investigated in 43 lesions from the brain tissue of patients with multiple sclerosis (MS). Numerous ...VCAM-1-positive macrophages/microglia were detected at the edges of MS lesions. Quantitative analysis of 6 active, 7 chronic active, and 4 chronic inactive MS lesions identified most VCAM-1-positive cells at the actively demyelinating borders of active (102/mm) and chronic active (29/mm) lesions, but rarely in chronic inactive lesions (4/mm). Further, approximately 17% of the VCAM-1-positive cells closely apposed or surrounded oligodendrocyte perikarya at the edges of active and chronic active lesions that were sites of ongoing demyelination. Endothelial cells were VCAM-1-negative in both lesion and non-lesion MS brain tissue. This report is the first to document direct microglial interaction with oligodendrocytes in MS.
In October 2017, the International Olympic Committee (IOC) hosted an international expert group of physiotherapists and orthopaedic surgeons who specialise in treating and researching paediatric ACL ...injuries. The purpose of this consensus statement was to provide a comprehensive, evidence-informed summary to support the clinician, and help children with ACL injury and their parents/guardians make the best possible decisions. Representatives from the American Orthopaedic Society for Sports Medicine, European Paediatric Orthopaedic Society, European Society for Sports Traumatology, Knee Surgery & Arthroscopy, International Society of Arthroscopy Knee Surgery and Orthopaedic Sports Medicine, Pediatric Orthopaedic Society of North America, and Sociedad Latinoamericana de Artroscopia, Rodilla y Deporte attended. Physiotherapists and orthopaedic surgeons with clinical and research experience in the field, and an ethics expert with substantial experience in the area of sports injuries also participated. This consensus statement addresses 6 fundamental clinical questions regarding the prevention, diagnosis, and management of paediatric ACL injuries. Injury management is challenging in the current landscape of clinical uncertainty and limited scientific knowledge. Injury management decisions also occur against the backdrop of the complexity of shared decision-making with children and the potential long-term ramifications of the injury.
Abstract
We present a critical analysis of the monograph of A.S.D. Synnestvedt (1869) "En anatomisk beskrivelse af de paa over- og underestremiteterne forekommende Bursae mucosae". The analysis was ...completed using anatomical information from the historically oldest publications dealing with the bursae of the extremities: Albinus (1734), Monro (1788), Rosenmüller (1799). We are of the opinion that Synnestvedt's publication is important, not only historically but also as a source of information for recent medical practitioners. Synnestvedt's monograph has a wealth of literary citations, unambiguous opinions of seasoned anatomists regarding the structure and function of the synovial membrane, and detailed descriptions of dissections he performed on fetal and adult cadavers. The information in this publication may enhance the diagnosis of bursopathies and enthesopathies of the extremities.
