Angiotensin II type 1 receptor antagonists (ARBs) have become an important drug class in the treatment of hypertension and heart failure and the protection from diabetic nephropathy. Eight ARBs are ...clinically available azilsartan, candesartan, eprosartan, irbesartan, losartan, olmesartan, telmisartan, valsartan. Azilsartan (in some countries), candesartan, and olmesartan are orally administered as prodrugs, whereas the blocking action of some is mediated through active metabolites. On the basis of their chemical structures, ARBs use different binding pockets in the receptor, which are associated with differences in dissociation times and, in most cases, apparently insurmountable antagonism. The physicochemical differences between ARBs also manifest in different tissue penetration, including passage through the blood-brain barrier. Differences in binding mode and tissue penetration are also associated with differences in pharmacokinetic profile, particularly duration of action. Although generally highly specific for angiotensin II type 1 receptors, some ARBs, particularly telmisartan, are partial agonists at peroxisome proliferator-activated receptor-γ. All of these properties are comprehensively reviewed in this article. Although there is general consensus that a continuous receptor blockade over a 24-hour period is desirable, the clinical relevance of other pharmacological differences between individual ARBs remains to be assessed.
Height and weight are key variables in epidemiologic research, including studies of reproductive outcomes. Optimally, this information is collected by trained study personnel. However, direct ...measurements are not always feasible. The purpose of this study was to investigate the accuracy of self-reported height and weight, as well as the resultant body mass index (BMI) calculated from self-reported height and weight (referred to as self-reported BMI), among a group of women of reproductive age according to select demographic variables.
A total of 381 women provided self-reported height, weight, and demographic information on a questionnaire for a study of contraceptive trends while attending a Family Medicine clinic. Height and weight were also abstracted from medical records for 275 of these study participants. Self-reported and measured values for height, weight, and BMI were compared using paired t-tests. Analysis of variance, chi-square tests, and Fisher's Exact tests were used to examine how differences between self-reported and measured values varied by select demographic factors.
Women underestimated weight by 4.6 pounds, overestimated height by 0.1 inches, and underestimated BMI by 0.8 kg/m(2). All women, regardless of age, education, race/ethnicity, or marital status, underestimated their weight. These differences were statistically significant for all groups (p<0.01) with the exception of women with a high school education. Self-reported height and weight measures classified 84% of women into appropriate BMI categories.
Overall, self-reported height and weight were found to give an accurate representation of true BMI in this study. There were some demographic differences in the ability to accurately report height and weight, particularly with respect to race/ethnicity. Future studies should investigate these racial/ethnic differences among a larger population.
Pathological Internet use (PIU) has been conceptualized as an impulse-control disorder that shares characteristics with behavioral addiction. Research has indicated a potential link between PIU and ...psychopathology; however, the significance of the correlation remains ambiguous. The primary objective of this systematic review was to identify and evaluate studies performed on the correlation between PIU and comorbid psychopathology; the secondary aims were to map the geographical distribution of studies, present a current synthesis of the evidence, and assess the quality of available research.
An electronic literature search was conducted using the following databases: MEDLINE, PsycARTICLES, PsychINFO, Global Health, and Web of Science. PIU and known synonyms were included in the search. Data were extracted based on PIU and psychopathology, including depression, anxiety, symptoms of attention deficit and hyperactivity disorder (ADHD), obsessive-compulsive symptoms, social phobia and hostility/aggression. Effect sizes for the correlations observed were identified from either the respective publication or calculated using Cohen's d or R(2). The potential effect of publication bias was assessed using a funnel plot model and evaluated by Egger's test based on a linear regression.
The majority of research was conducted in Asia and comprised cross-sectional designs. Only one prospective study was identified. Twenty articles met the preset inclusion and exclusion criteria; 75% reported significant correlations of PIU with depression, 57% with anxiety, 100% with symptoms of ADHD, 60% with obsessive-compulsive symptoms, and 66% with hostility/aggression. No study reported associations between PIU and social phobia. The majority of studies reported a higher rate of PIU among males than females. The relative risks ranged from an OR of 1.02 to an OR of 11.66. The strongest correlations were observed between PIU and depression; the weakest was hostility/aggression.
Depression and symptoms of ADHD appeared to have the most significant and consistent correlation with PIU. Associations were reported to be higher among males in all age groups. Limitations included heterogeneity in the definition and diagnosis of PIU. More studies with prospective designs in Western countries are critically needed.
A crucial requirement for quantum-information processing is the realization of multiple-qubit quantum gates. Here, we demonstrate an electron spin-based all-electrical two-qubit gate consisting of ...single-spin rotations and interdot spin exchange in a double quantum dot. A partially entangled output state is obtained by the application of the two-qubit gate to an initial, uncorrelated state. We find that the degree of entanglement is controllable by the exchange operation time. The approach represents a key step towards the realization of universal multiple-qubit gates.
We have reviewed the effects of angiotensin II type 1 receptor antagonists (ARBs) in various animal models of hypertension, atherosclerosis, cardiac function, hypertrophy and fibrosis, glucose and ...lipid metabolism, and renal function and morphology. Those of azilsartan and telmisartan have been included comprehensively whereas those of other ARBs have been included systematically but without intention of completeness. ARBs as a class lower blood pressure in established hypertension and prevent hypertension development in all applicable animal models except those with a markedly suppressed renin-angiotensin system; blood pressure lowering even persists for a considerable time after discontinuation of treatment. This translates into a reduced mortality, particularly in models exhibiting marked hypertension. The retrieved data on vascular, cardiac and renal function and morphology as well as on glucose and lipid metabolism are discussed to address three main questions: 1. Can ARB effects on blood vessels, heart, kidney and metabolic function be explained by blood pressure lowering alone or are they additionally directly related to blockade of the renin-angiotensin system? 2. Are they shared by other inhibitors of the renin-angiotensin system, e.g. angiotensin converting enzyme inhibitors? 3. Are some effects specific for one or more compounds within the ARB class? Taken together these data profile ARBs as a drug class with unique properties that have beneficial effects far beyond those on blood pressure reduction and, in some cases distinct from those of angiotensin converting enzyme inhibitors. The clinical relevance of angiotensin receptor-independent effects of some ARBs remains to be determined.
So far, all previous attempts to apply nanostructures for perfect transmission have not achieved maximum transmittance beyond 99.5% due to the limited regularity of the nanoscale surface geometry: ...too low for many high-end applications. Here we demonstrate a nanostructured stealth surface, with minimal reflectance (<0.02%) and maximal transmittance (>99.8%) for a wavelength range, covering visible and near-infrared. Compared to multilayer thin film coatings for near-infrared applications our antireflective surfaces operate within a much broader wavelength range, are mechanical stable to resist human touch or contamination, show a 44% higher laser-induced damage threshold, and are suitable for bended interfaces such as microlenses as well.
Introduction
Nationally, cesarean birth is one of the most performed surgical procedures, yet cesarean births have been linked to an increased risk of delivery complications. Prenatal care (PNC) and ...education are possible strategies to reduce the number of cesarean births. However, there is scant research assessing the impact of these strategies on safely reducing primary cesarean births. This study evaluates the association between the adequacy of PNC utilization and primary cesarean birth.
Methods
The analysis used 2018 birth certificate data, and the sample included nulliparous women with no reported pregnancy or delivery complications (
N
= 729,140). Logistic regression was used to model the association between the adequacy of PNC utilization and delivery method, as well as identify other factors associated with the delivery method.
Results
Among women with a primary cesarean birth, 36.2% had received adequate plus PNC. After adjustment, there was no significant association between women receiving inadequate, intermediate, or adequate PNC and primary cesarean birth. However, women who received adequate plus PNC had an increased odds of having a primary cesarean birth compared to women with no PNC (OR, 1.23; 95% CI, 1.18–1.28).
Discussion
Findings from this study highlight the need to further understand the role of PNC and its potential impact on the delivery method. Within the patient-provider relationship, healthcare providers have the unique opportunity to provide education and inform patients of the risks and benefits of all delivery options. Thus, there is an increased opportunity to safely reduce primary cesarean births.
Significance
In the United States, cesarean births remain relatively high and are associated with potentially adverse effects. However, literature describing the potential impact of strategies to reduce cesarean births, such as prenatal care, and its association with the delivery method is lacking. Using 2018 birth certificate data, researchers were able to evaluate the adequacy of prenatal care utilization and primary cesarean birth association. This study found that women with a high utilization of prenatal care were more likely to have a primary cesarean birth; therefore, healthcare providers are essential in advising pregnant women about the risks and benefits of primary cesarean birth.
Chronic disease management requires the individual to perform varying forms of self-care behaviors. Self-efficacy, a widely used psychosocial concept, is associated with the ability to manage chronic ...disease. In this study, we examine the association between self-efficacy to manage hypertension and six clinically prescribed hypertension self-care behaviors. We interviewed 190 African Americans with hypertension who resided in the greater metropolitan Charlotte area about their self-efficacy and their hypertension self-care activities. Logistic regression for correlated observations was used to model the relationship between self-efficacy and adherence to hypertension self-care behaviors. Since the hypertension self-care behavior outcomes were not rare occurrences, an odds ratio correction method was used to provide a more reliable measure of the prevalence ratio (PR). Over half (59%) of participants reported having good self-efficacy to manage their hypertension. Good self-efficacy was statistically significantly associated with increased prevalence of adherence to medication (PR = 1.23, 95% CI: 1.08, 1.32), eating a low-salt diet (PR = 1.64, 95% CI: 1.07–2.20), engaging in physical activity (PR = 1.27, 95% CI: 1.08–1.39), not smoking (PR = 1.10, 95% CI: 1.01–1.15), and practicing weight management techniques (PR = 1.63, 95% CI: 1.30–1.87). Hypertension self-efficacy is strongly associated with adherence to five of six prescribed self-care activities among African Americans with hypertension. Ensuring that African Americans feel confident that hypertension is a manageable condition and that they are knowledgeable about appropriate self-care behaviors are important factors in improving hypertension self-care and blood pressure control. Health practitioners should assess individuals’ self-care activities and direct them toward practical techniques to help boost their confidence in managing their blood pressure.
We present the galaxy two-point angular correlation function for galaxies selected from the seventh data release of the Sloan Digital Sky Survey (SDSS). The galaxy sample was selected with r-band ...apparent magnitudes between 17 and 21, and we measure the correlation function for the full sample as well as for the four magnitude ranges: 17-18, 18-19, 19-20 and 20-21. We update the flag criteria to select a clean galaxy catalogue and detail specific tests that we perform to characterize systematic effects, including the effects of seeing, Galactic extinction and the overall survey uniformity. Notably, we find that optimally we can use observed regions with seeing <1.5 arcsec, and r-band extinction <0.13 mag, smaller than previously published results. Furthermore, we confirm that the uniformity of the SDSS photometry is minimally affected by the stripe geometry. We find that, overall, the two-point angular correlation function can be described by a power law, ω(θ) = A
ωθ(1 − γ) with γ 1.72, over the range 0
005-10°. We also find similar relationships for the four magnitude subsamples, but the amplitude within the same angular interval for the four subsamples is found to decrease with fainter magnitudes, in agreement with previous results. We find that the systematic signals are well below the galaxy angular correlation function for angles less than approximately 5°, which limits the modelling of galaxy angular correlations on larger scales. Finally, we present our custom, highly parallelized two-point correlation code that we used in this analysis.
We combine the quasi-stellar object (QSO) samples from the 2dF QSO Redshift Survey (2QZ) and the 2dF-Sloan Digital Sky Survey luminous red galaxy (LRG) and QSO Survey (2dF-SDSS LRG and QSO, hereafter ...2SLAQ) in order to investigate the clustering of z∼ 1.5 QSOs and measure the correlation function (ξ). The clustering signal in redshift-space and projected along the sky direction is similar to that previously obtained from the 2QZ sample alone. By fitting functional forms for ξ(σ, π), the correlation function measured along and across the line of sight, we find, as expected, that β, the dynamical infall parameter and Ω0
m, the cosmological density parameter, are degenerate. However, this degeneracy can be lifted by using linear theory predictions under different cosmological scenarios. Using the combination of the 2QZ and 2SLAQ QSO data, we obtain: βQSO(z= 1.5) = 0.60+0.14
−0.11, Ω0
m= 0.25+0.09
−0.07 which imply a value for the QSO bias, b(z= 1.4) = 1.5 ± 0.2.
The combination of the 2QZ with the fainter 2SLAQ QSO sample further reveals that QSO clustering does not depend strongly on luminosity at fixed redshift. This result is inconsistent with the expectation of simple 'high peaks' biasing models where more luminous, rare QSOs are assumed to inhabit higher mass haloes. The data are more consistent with models which predict that QSOs of different luminosities reside in haloes of similar mass. By assuming ellipsoidal models for the collapse of density perturbations, we estimate the mass of the dark matter haloes which the QSOs inhabit as ∼3 × 1012
h
−1M⊙. We find that this halo mass does not evolve strongly with redshift nor depend on QSO luminosity. Assuming a range of relations which relate halo to black hole mass, we investigate how black hole mass correlates with luminosity and redshift, and ascertain the relation between Eddington efficiency and black hole mass. Our results suggest that QSOs of different luminosities may contain black holes of similar mass.
