Little information exists regarding factors influencing perioperative cardiac arrests and their outcome. This survey evaluated the incidence, causes and outcome of perioperative cardiac arrests in a ...Brazilian tertiary general teaching hospital between April 1996 and March 2005.
The incidence of cardiac arrest during anaesthesia was prospectively identified from an anaesthesia database. There were 53 718 anaesthetics during the study period. Data collected included patient characteristics, surgical procedures (elective, urgent or emergency), ASA physical status classification, anaesthesia provider information, type of surgery, surgical areas and outcome. All cardiac arrests were retrospectively reviewed and grouped by cause of arrest and death into one of four groups: totally anaesthesia related, partially anaesthesia related, totally surgery related or totally patient disease or condition related.
One hundred and eighty-six cardiac arrests (34.6:10 000) and 118 deaths (21.97:10 000) were found. Major risk factors for cardiac arrest were neonates, children under 1 yr and the elderly (P<0.05), male patients with ASA III or poorer physical status (P<0.05), in emergency surgery (P<0.05) and under general anaesthesia (P<0.05). Patient disease/condition was the major cause of cardiac arrest or death (P<0.05). There were 18 anaesthesia-related cardiac arrests (3.35:10 000)—10 totally attributed (1.86:10 000) and 8 partially related to anaesthesia (1.49:10 000). There were 6 anaesthesia-related deaths (1.12:10 000)—3 totally attributable and 3 partially related to anaesthesia (0.56:10 000 in both cases). The main causes of anaesthesia-related cardiac arrest were respiratory events (55.5%) and medication-related events (44.5%).
Perioperative cardiac arrests were relatively higher in neonates, infants, the elderly and in males with severe underlying disease and under emergency surgery. All anaesthesia-related cardiac arrests were related to airway management and medication administration which is important for prevention strategies.
To elucidate fungicultural specializations contributing to ecological dominance of leafcutter ants, we estimate the phylogeny of fungi cultivated by fungus‐growing (attine) ants, including fungal ...cultivars from (i) the entire leafcutter range from southern South America to southern North America, (ii) all higher‐attine ant lineages (leafcutting genera Atta, Acromyrmex; nonleafcutting genera Trachymyrmex, Sericomyrmex) and (iii) all lower‐attine lineages. Higher‐attine fungi form two clades, Clade‐A fungi (Leucocoprinus gongylophorus, formerly Attamyces) previously thought to be cultivated only by leafcutter ants, and a sister clade, Clade‐B fungi, previously thought to be cultivated only by Trachymyrmex and Sericomyrmex ants. Contradicting this traditional view, we find that (i) leafcutter ants are not specialized to cultivate only Clade‐A fungi because some leafcutter species ranging across South America cultivate Clade‐B fungi; (ii) Trachymyrmex ants are not specialized to cultivate only Clade‐B fungi because some Trachymyrmex species cultivate Clade‐A fungi and other Trachymyrmex species cultivate fungi known so far only from lower‐attine ants; (iii) in some locations, single higher‐attine ant species or closely related cryptic species cultivate both Clade‐A and Clade‐B fungi; and (iv) ant–fungus co‐evolution among higher‐attine mutualisms is therefore less specialized than previously thought. Sympatric leafcutter ants can be ecologically dominant when cultivating either Clade‐A or Clade‐B fungi, sustaining with either cultivar‐type huge nests that command large foraging territories; conversely, sympatric Trachymyrmex ants cultivating either Clade‐A or Clade‐B fungi can be locally abundant without achieving the ecological dominance of leafcutter ants. Ecological dominance of leafcutter ants therefore does not depend primarily on specialized fungiculture of L. gongylophorus (Clade‐A), but must derive from ant–fungus synergisms and unique ant adaptations.
A púrpura trombocitopênica aloimune neonatal (TNA) é uma patologia grave, em que a mãe produz anticorpos contra antígenos plaquetários do feto herdados do pai. Na gravidez, a mãe forma anticorpos ...antiplaquetários da classe de imunoglobulina G (IgG) contra o antígeno, que passa a placenta e destrói as plaquetas fetais que apresentam antígeno paterno. Condição rara, com incidência de 1 em 10.000. Afeta a primeira gestação, podendo causar hemorragia intracraniana (HIC), com tendência a ocorrer de modo mais grave em gestações futuras. No presente relato, trata-se do caso de uma recém-nascida (RN) investigada por quadro de plaquetopenia e icterícia neonatal.
RN, feminina, de parto cesáreo 40+3, Apgar 9/10, triagem neonatal normal. Filha de puérpera de 14 anos, primigesta, pré-natal iniciado no 2° trimestre, com oligoidrâmnio, em uso de sulfato ferroso, O RhD negativo, sorologias negativas, hemograma completo, coagulograma normais e pesquisa de subgrupo de incompatibilidade sanguínea entre a mãe e RN negativa. RN, no 2° dia de vida apresentou icterícia zona 1 e petéquias isoladas no abdome. Evoluiu para icterícia zona 4, já no 3° dia, com bilirrubina transcutânea de 13,4. Prosseguiu investigação com tipagem sanguínea O RhD positivo, bilirrubinas totais 14,42, bilirrubina direta 2,19, bilirrubina indireta 12,23, Coagulograma normal e plaquetas 41.0000 mm³. Exames como enzimas hepáticas, sorologias para infecção (CMV, rubéola e herpes) normais. No 5° dia de vida, elevação de plaquetas (80.000 mm3) seguido de nova queda (47.000 mm3) no 6° dia, sendo indicado transfusão de plaquetas irradiadas da mãe para a RN, não realizada, pois era menor de idade e estava anêmica (Hb/Ht: 9 g/dL/29%). A icterícia diminuiu e, no 9° dia de vida plaquetas eram de 208.000 mm. Enviado para o HEMORIO amostra de sangue do bebê e da mãe para investigação de plaquetopenia por etiologia imunológica. Os resultados demonstram justificativas imunológicas para a plaquetopenia neonatal podendo estar relacionado a anticorpos HLA e HPA. A mãe apresentou anticorpos contra o antígeno A2 presente na filha. Somado a isto a mãe foi negativa para três diferentes antígenos HPA na filha HPA-1a, HPA-5b, HPA-15b, o que evidenciou plaquetopenia de causa imunológica envolvendo anticorpos HPA em especial HPA-1a. A RN teve alta e continuará em acompanhando ambulatorial.
A TNA é definida como contagem de plaquetas inferior a 150.000 mm3 devido a incompatibilidade materno-fetal plaquetária. O HPA-1a é o antígeno plaquetário mais comumente envolvido, sendo o pai normalmente HPA-1a positivo e a mãe HPA-1a negativa. O RN pode apresentar trombocitopenia leve à grave levando à HIC, podendo ser fatal. HIC fetal ou plaquetas abaixo de 100.000 mm3 dentro de 7 dias pós-nascimento, na ausência de outras causas, são critérios sugestivos desta condição. O diagnóstico é feito por genotipagem plaquetária, que confirma a incompatibilidade HPA materno-fetal, além da detecção de anticorpos anti-HPA no soro da mãe. O tratamento materno consiste em IgIV. Está indicada a transfusão de plaquetas HPA compatíveis nas primeiras 24 horas de vida quando o RN possui contagem menor de 30.000 mm3 ou sinais de sangramento. Outra alternativa é a doação de plaquetas materna.
Essa patologia além de rara é grave. A suspeita diagnóstica precoce é primordial para um melhor desfecho, uma vez que as medidas de tratamento devem ser iniciadas precocemente. No presente caso, houve um resultado favorável, contudo necessita manter acompanhamento da paciente e aconselhamento materno para futuras gestações.
A pandemia SARS-CoV-2, conhecido como COVID-19, trouxe dificuldades importantes para as diversas áreas da saúde, especialmente no que se refere a doação de sangue, que foi reduzida a números ...preocupantes. Com isso, fez-se necessário instituir meios para amenizar os baixos estoques de sangue neste período. Assim, o Serviço de Hemoterapia do Hospital São Vicente de Paulo (SHHSVP) e o Programa de Extensão ComSaúde da Universidade de Passo Fundo, encontraram na Sétima Região Tradicionalista, que concentra em torno de 140 entidades (Centros Tradicionalistas Gaúcho e Piquetes de Laço), e na Academia Passo-Fundense de Letras (APL) apoiadores para realização de ações sobre doação de sangue com a temática: “Gaúcho doador: doe sangue, mostre seu valor”. As atividades da campanha incluíram lives educativas sobre o processo de doação sangue, bem como oficinas online de confecção de poesias, concurso de poesias gaudérias e grupos de doação de sangue.
O projeto gaúcho doador iniciou-se em setembro, mês de comemorações tradicionalistas, em especial do dia do gaúcho. A ideia surgiu da necessidade de doações de sangue devido aos baixos estoques nos bancos de sangue, e iniciou com a sensibilização de possíveis doadores na comunidade da Sétima Região Tradicionalista, que envolve 42 municípios e 140 entidades, através da educação e da cultura regional, neste período da pandemia. Além disso, desenvolver atividades de cidadania na comunidade de Passo Fundo e na região por meio de ações educativas em um ambiente virtual com disseminação das informações via redes sociais, a exemplo da live sobre doação de sangue, em que se explicou o funcionamento de todo processo de doação bem como respondeu dúvidas dos participantes, e oficina de produção de poesias com a ajuda das instituições responsáveis. A partir disso, foi lançado um desafio por meio de um concurso de poesias sobre o tema doação na linguagem gauchesca: “Gaúcho Doador. Doe sangue, Mostre Seu Valor”, em diferentes categorias, categorizadas por idade desde crianças a idosos, coordenada pela APL, que além da oficina sobre as regras do concurso elegeu os vencedores, que foram premiados em uma solenidade no dia 25 de novembro de 2020, também comemorado dia do doador, no SHHSVP, conduzido pela APL com a participação dos vencedores do concurso, autoridades, parceiros do projeto e o Hemozito pilchado, o qual foi confeccionado especialmente para o evento. Essa ação foi extremamente importante, visto que, por estarmos em um período de pandemia, as doações de sangue sofreram grande impacto negativo. Além de aumentar o número de doações espontâneas no SHHSVP esse projeto foi de suma importância para a interação da comunidade, uma vez que uniu cultura e educação em prol da saúde. Ademais, a campanha Gaúcho Doador trouxe visibilidade nas mídias sociais e nas entidades tradicionalistas para a importância da doação de sangue e inclusão de novos doadores, os quais são potenciais futuros doadores e disseminadores da importância dessa ação.
A doação de sangue é um ato que salva vidas, por isso sua carência gera preocupação. A campanha Gaúcho Doador atuou como uma alternativa para captar novos doadores e valorizar a cultura tradicionalista. Esta campanha proporcionou que os valores gaúchos impulsionassem a doação de sangue e assim, juntos, eles criaram uma nova tradição no coração dos gaúchos.
A simulation study of a microreactor for steam reforming of ethanol (ESR) over a washcoated Pd catalyst is reported in the present manuscript. Both co- and countercurrent configurations are ...considered for the streams of reactants and flue gas through microchannels of a square section. Two contiguous channels, assumed as representative of the whole reactor behavior, are simulated using a 1D pseudohomogeneous model. The microreactor performance is analyzed for different heat-supply strategies and channel sizes. The results demonstrate that the ESR in the microreactor is strongly controlled by the heat supply. The hydrogen yield has a marked dependence on the total amount of heat transferred from the flue gas to the reformate and the axial distribution of the heat flow. The cocurrent-flow configuration proved to yield the higher performance. While smaller channels work better for cocurrent operation, an optimum compromise value is found for the channel size in countercurrent flow.
Gelatin microparticles containing propolis ethanolic extractive solution were prepared by spray-drying technique. Particles with regular morphology, mean diameter ranging of 2.27 microm to 2.48 ...microm, and good entrapment efficiency for propolis were obtained. The in vitro antimicrobial activity of microparticles was evaluated against microorganisms of oral importance (Enterococcus faecalis, Streptococcus salivarius, Streptococcus sanguinis, Streptococcus mitis, Streptococcus mutans, Streptococcus sobrinus, Candida albicans, and Lactobacillus casei). The utilized techniques were diffusion in agar and determination of minimum inhibitory concentration. The choice of the method to evaluate the antimicrobial activity of microparticles showed be very important. The microparticles displayed activity against all tested strains of similar way to the propolis, showing greater activity against the strains of E. salivarius, S. sanguinis, S. mitis, and C. albicans.

The steam reforming of ethanol is carried out in a microreactor for hydrogen production. The heat for the endothermic reactions is supplied by means of ethanol combustion in air, which is carried out in contiguous microchannels. The same Pd-based catalyst is assumed to be coated on both reforming and combustion channels. By means of a 1D heterogeneous mathematical model, the influence of the feed temperatures of both streams on the reactor performance is analyzed. The results show that the degree of preheating of both streams has a strong influence on the hydrogen yields and maximum temperatures. The effect of the flowrate and composition of the fuel stream on the hydrogen yields is also studied.
Fairly high hydrogen yields were obtained (2.6<ηH2<3.4) with low methane slips, within a feasible range of temperatures for the Pd catalyst (700<TMAX<770°C) to avoid catalyst deactivation. These maximum temperatures could still be further reduced by means of an optimal selection of the air flowrate and ethanol molar fraction on the fuel side, or using distributed feed of fuel along the reactor length.
An analysis of changes experienced during the last century by certain indicators of the intensity of geomorphic processes in northern Spain is presented, in order to test a previously formulated ...hypothesis. The hypothesis, already tested in the Rio de la Plata Basin, is that there is a global geomorphic change which implies an acceleration of geomorphic processes in general, and that such acceleration is mainly due to the transformation of land surface by human activities, not to climate factors. Sedimentation rates obtained in eight estuaries in northern Spain, through Pb-210 and Cs-137 dating of sediment cores, show a general increase since the beginning of last century, and particularly after the 1950s. Similar increases in landslide frequency have been formerly observed in some areas of the same region. Trends of change in sedimentation rates have been compared with those of potential natural (rainfall) and human (indicators of the intensity of human activities that can contribute to land-surface transformation) drivers. There seems to be no relationship between rainfall and sedimentation rate trends but the magnitude and trends of several indicators of human activity are similar to those of the latter. Data on landslide frequency obtained in a small study area also show a possible relationship with land-use change and infrastructure development, but not with rainfall. The results described are coherent with the hypothesis and suggest that the process described could have a global character. If this is so, the worldwide observed increase in flood and landslide disasters might be a characteristic of the Anthropocene and due to human-induced geomorphic change, rather than climate change (also human-induced), as often assumed.
► 20th century sedimentation rates in N Spain show in general increasing trends. ► Increasing rates do not seem to be related to rainfall trends. ► Indicators of human activity provide a better match with sedimentation rate trends. ► Results are coherent with an acceleration of processes (global geomorphic change). ► Increasing geomorphic hazards appear to be a consequence of that change.

The basic idea to arrive at such relations is not new, but the specific application reported in this paper is new, and it is likely to open the way to several analogous new findings.
