After post-septoplasty nasal packing removal, a certain proportion of nasal secretion occurs, leading to local and sometimes systemic infections.
The aim was to determine if standardized dry ivy leaf ...extract application after nasal packing removal influences the reduction of nasal secretion and diminish the occurrence of local infections.
The study included 70 post-septoplasty patients (divided into two equal groups) whose nasal packing was removed on the third day after the procedure. Group I was treated with standardized dry ivy leaf extract syrup along with regular nasal irrigation for the five days after the nasal packing removal whereas the Group II had only nasal lavage. On the sixth day after nasal packing removal, the quantity of nasal secretion was determined using a visual analog scale and nasal endoscopic examination.
The group treated with standardized dry ivy leaf extract syrup had significantly lesser nasal secretion both by subjective patients’ assessment (p<0.001) and by nasal endoscopic examination (p=0.003). The post-surgical follow up examination on the sixth day after nasal packing removal showed no development of local infection in the Group I, while in the Group II a local infection was evident in five patients (14.29%) and antibiotic therapy was required.
The use of the standardized dry ivy leaf extract after nasal packing removal significantly lowers the proportion of nasal secretion.
Após a remoção do tampão nasal pós-septoplastia, ocorre produção de secreção nasal, predispondo infecções locais e, por vezes, sistêmicas.
O objetivo foi determinar se a aplicação do extrato padronizado de folhas de hera seca após a remoção do tampão nasal influencia a redução da secreção nasal e diminui a ocorrência de infecções locais.
O estudo incluiu 70 pacientes pós-septoplastia (divididos em dois grupos iguais) cujo tampão nasal foi retirado no terceiro dia após o procedimento. O grupo I foi tratado com xarope padronizado de extrato de folha seca de hera juntamente com irrigação nasal regular por cinco dias após a remoção do tamponamento nasal, enquanto ao grupo II foi recomendado apenas lavagem nasal. No sexto dia após a remoção do tampão nasal, a quantidade de secreção nasal foi determinada pela escala EVA (escala visual analógica) e pelo exame endoscópico nasal.
O grupo tratado com xarope de extrato seco de folhas de hera apresentou secreção nasal significativamente menor tanto pela avaliação subjetiva dos pacientes (p<0,001) quanto pelo exame endoscópico nasal (p=0,003). O exame de acompanhamento pós-cirúrgico no sexto dia após a remoção do tampão nasal não mostrou desenvolvimento de infecção local nos pacientes do grupo I, enquanto que no grupo II, cinco apresentaram sinais de infecção local (14,29%) com necessidade de antibioticoterapia.
O uso do extrato padronizado de folhas secas de hera após a remoção do tampão nasal reduz significativamente a produção de secreção nasal.

=70) adult patients with moderate-to-severe SAR were included in this prospective study and treated by four different procedures: (1) 21 patients received azelastine intranasal spray for 30 days and, after that, azelastine spray and Lertal® tablets for the next 30 days; (2) 19 patients received combined azelastine with fluticasone intranasal spray for 30 days and azelastine with fluticasone spray and Lertal® tablets for the next 30 days; (3) 15 patients received azelastine spray only for 60 days; (4) 15 patients received combined azelastine with fluticasone spray only for 60 days. Levels of SAR symptoms (sneezing, tearing, ocular itching, rhinorrhea, nasal obstruction, hyposmia, and cough), as well as Total Symptom Scores (TSS), were evaluated at the start of this investigation (visit 0), after 30 days of treatment (visit 1), and after 60 days of treatment (visit 2) using a visual analog scale. After 30 days of treatment, better effects were achieved in groups in which patients were treated with combined (antihistamine with corticosteroid) spray. After 60 days of therapy, we found the best effects in procedure 2 and slightly worse effects in procedure 1. The high differences in the reduction of TSS between the 60th and 30th day were found for procedure 2 (

<0.001) and procedure 1 (

<0.001). The worse improvement of symptoms we found was in procedure 4 (

<0.01), and, for the procedure 3, we found no significant difference (

=0.140). None of the patients reported adverse effects during the therapy.
Conclusion
Our results suggest that addition of food supplement Lertal® to the standard topical therapy of patients with moderate-to-severe SAR increases the effects of intranasal therapy in reducing nasal and ocular symptoms.
Introduction/Objective. Fungal rhinosinusitis comprises of a wide range of immune-pathological responses, including invasive, chronic, granulomatous, and allergic diseases. Aim of this study was to ...determine total number of patients, their characteristics, frequency of symptoms, the manner of disease manifestation and the success of therapy. Methods. Study included 21 patients with fungal rhinosinusitis diagnosis made according to the 2020 EPOS guidelines. Results. Based on the visual analogue scale, feeling of localized pressure, i.e., facial pain, was dominant with a score of 9.57 ? 0.98, followed by the secretion from the nose with 8.14 ? 1.62, problems with breathing through the nose with 6.67 ? 3.25, and reduction the sense of smell with 2.14 ? 3.00. The t-test showed a statistically significant difference between mucosal changes on the diseased and healthy sides of the patient?s face (p < 0.0001). Only one sinus was affected intraoperatively in 18 (85.71%) patients. The most commonly affected sinus was the maxillary one, in 13 (54.17%) patients, followed by the sphenoid sinus in five (20.83%) patients. Aspergillus was proven ?s the couse of rhinosinusitis in 12 (57.14%) patients. Conclusion. The dominant symptom of patients with fungal rhinosinusitis was localized pain/pressure in the area of the affected sinus. Endoscopically, on the side of the affected sinus, the pathological mucosa with thick, pithy, mucous secretion dominated. The maxillar sinus was primarily unilaterally affected, in more than half of the patients. Aspergillus has been proven to be the most common cause of rhinosinusitis.
Background/Aim. Chronic rhinosinusitis (CRS) is one of the most common chronic conditions that is diagnosed on the basis of the condition symptoms, nasal endoscopy and computed tomograhpy (CT) of the ...nose and paranasal sinuses. There are two forms of CRS: CRS with nasal polyps (CRSwNP) and CRS without nasal polyps (CRSsNP). The aim of this paper was to determine if there is a correlation between the symptoms, nasal endoscopy and CT in patients with CRSsNP. Methods. The study included 110 patients with CRSsNP. The intensity of the symptoms assessed on the visual analogue scale (VAS) and the condition of nasal mucosa and the presence of nasal secretion was estimated by endoscopic examination of the nose while CT was used to determine the Lund-Mackay (LM) score values. Pearson?s coefficient of correlation was used for statistic data processing. Results. The severity of the disease as a whole (r = 0.509) and nasal discharge (r = 0.562) moderately correlated with CT. Nasal congestion (r = 0.354) and the reduction of loss of smell (r = 0.324) mildly correlated with CT, while facial pain/pressure (r = 0.218) had a very weak correlation with CT. The severity of the disease as a whole (r = 0.717) and nasal discharge (r = 0.821) strongly correlated with nasal endoscopy. Nasal congestion (r = 0.525) had a moderate correlation with nasal endoscopy while facial pain/pressure (r = 0.345) and the reduction of the loss of smell (r= 0.394) had a mild correlation with nasal endoscopy. A moderate correlation was found between nasal endoscopy and CT (r = 0.630). Conclusion. The severity of the disease as a whole and nasal discharge have more significant correlations both with nasal endoscopy and CT in relation to the correlations between other symptoms and nasal endoscopy, as well as CT. More significant correlations between the symptoms and nasal endoscopy in relation to the correlations between the symptoms and CT and the existence of a moderate correlation between nasal endoscopy and CT, enable a lesser use of CT diagnostics and only in precisely defined situatiations.

Backgound/Aim. Allergic rhinitis is the most frequent type of rhinitis affecting more than 600 million people worldwide. As incidence increases, it is important to know about the characteristics, the ...allergens that exacerbate it as well as effects of allergic rhinitis on population. The aim of this study was to determine among patients with chronic rhinitis the number of patients positive to standard inhaled allergens, their distribution by sex, age and to determine the type and frequency of allergic sensitization to specific inhalatory allergens. Methods. Data was collected from 514 patients tested for standard inhalatory allergens via the skin prick test from 01.01.2016 to 31.12. 2016. Age, sex and concomitant diseases as well an analysis of type and frequency of hypersensitivity to different allergens were assessed and recorded. Results. Of 514 patients, 307 patients, with an average age of 29.6 ? 8.88 years had a positive skin prick test. The sex ratio was 1.2 : 1 in favor of the females. Outdoor allergens affected 81.2% of all patients while indoor allergens 66.4% of them. Weed pollen (71%), grass pollen (61%) and Dermatophagoides pteronyssinus (46%) were the most common allergens. Most patients were sensitized to 1 (22%) or 2 (22%) allergens, while 20% of patients were simultaneously sensitized to 3 allergens. Conclusion. Most patients with the symptoms of chronic rhinitis had a positive allergic reaction. Those in their third decade of life were the most commonly affected. Outdoor allergens were the most prevalent allergen group, and weed pollen was the most frequent type of allergen.

Background/Aim. Cochlear implantation (CI) is a therapeutic modality that provides a sense of sound to children and adults with profound sensorineural hearing loss or deafness. The aim of this work ...was to evaluate the lateral semicircular canal function using a high frequency video head impulse test in children after CI. Methods. A prospective descriptive study included 28 children (6?17 years old) with profound sensorineural hearing loss and unilateral CI. The control group included 20 healthy children with normal hearing. The measurement of vestibular function of the lateral semicircular canal was performed using video head impulse test. After cochlear implantation, the children underwent the vestibular testing. Values vestibulo-ocular reflex of lateral semicircular canal were measured using the video head impulse test in the children with cochlear implant and the control group. The values of vestibulo-ocular reflex were compared between the group. Also, in the children with CI values of vestibulo-ocular reflex were compared between the non-implanted ear and the ear with the embedded CI. Results. All 28 children with sensorineural hearing loss underwent the placement of CI through cochleostomy at the average age of 4.8 ? 2.92 years. Children with the cochlear implant had a significantly lower vestibulo-ocular reflex gain of the lateral semicircular canal measured by a high frequency video head impulse test compared to the control group of children with normal hearing (T test: t = 3.714; p = 0.001). However in these children there was no statistically significant difference of vestibulo-ocular reflex gain in the lateral semicircular canal measured in ears with embedded CI and non-implanted ears (T test: t = 0.419; p = 0.677). Conclusion. The values of vestibulo-ocular reflex gain in the lateral semicircular canal evaluated by the video head impulse test are significantly lower in the children with a profound sensorineural hearing loss compared to the children with normal hearing. The CI did not appear to have a negative impact on the lateral semicircular canal.

Current recommendations proposed by pediatric audiologists are to commence with hearing amplification in children aged 6 months and above, after previous determination of the type and degree of ...hearing impairment and audiometric configuration. The goal of this study was to compare results obtained by click-evoked auditory brainstem response (c-ABR) and auditory steady state response (ASSR) in a group of children. This study included 68 children with different degrees of hearing impairment evaluated by c-ABR and ASSR. It is well-known that the c-ABR threshold highly correlates with behavioral hearing level at 2 kHz. In our study, the correlation between the c-ABR and ASSR thresholds in the whole sample was 0.58, 0.73, 0.97, 0.96, 0.95, 0.97; in the group of children with c-ABR thresholds up to 40 dBHL, it was 0.42, 0.73, 0.86, 0.74, 0.81, 0.81; and in the group with c-ABR thresholds worse than 40 dBHL, it was 0.46, 0.56, 0.89, 0.83, 0.85, 0.89 at 0.5, 1, 2, 4, 1-4, 2-4 kHz, respectively. Individual differences between the c-ABR and ASSR thresholds in the whole sample were up to 95, 90, 20, 25 dB at 0.5, 1, 2, 4 kHz, respectively. Study results indicated that there was strong correlation between the c-ABR and ASSR thresholds at 2, 4, 1-4, 2-4 kHz. The ASSR can be used as a valuable clinical tool and an excellent complementary method which, along with other audiologic techniques, provides more accurate hearing threshold estimation at an early age in children.
Introduction/Objective. Chronic rhinosinusitis is one of the most frequent chronic disorders which significantly influences the patients? quality of life. The objectiv? of this paper was to examine ...which are the most frequent and intensive symptoms in patients with chronic rhinosinusitis, and also to determine whether there is a correlation between a subjective assessment of the disease as a whole and individual symptoms. Methods. The study encircled 90 patients with clinical diagnosis of chronic rhinosinusitis that was endoscopically proven and computer tomography of the nose and paranasal sinuses. Every possible symptom was recorded in every patient (nasal congestion, nasal discharge, facial pain/pressure, reduction or loss of smell, headache, fatigue, cough, halitosis and ear pain/fullness), the intensity of every possible symptom as well as the disorder as a whole. The patients assessed the intensity of their symptoms on the visual analogue scale. Results. Nose congestion is the most frequent symptom. It occured in 82 patients (91.1%), followed by nasal discharge in 81 patients (90%) and there was no difference in frequency of these two symptoms. Nasal discharge has been recorded as the most intensive symptom (x? = 5.4) and it is significantly more intensive in comparison to nasal congestion which was the second on the intesity list (x? = 4.1, p = 0.002). All other symptoms were significantly less frequently and less intensive. The average intensity value of the disease as a whole is the same as the average intensity value of the nasal discharge (x? = 5.4) while the average intensity values of all other symptoms are statistically significantly lower than the average intensity value of the disease as a whole; in all comparisons p < 0.001. Conclusion. Nasal congestion and nasal discharge are the most common symptoms in the patients with chronic rhinosinusitis. Nasal discharge is the most intensive symptom in patients with chronic rhinosinusitis while its intensity determines the sensation of the intesity of the disorder as a whole.

Nasal obstruction is one of the most frequent disorders because of which patients see their Ear, Nose and Throath (ENT) doctors. Impaired nose breath ing is a subjective symptom and it often does not ...coincide with clinical nose findings and functional tests of breath ing function. Therefore, the aim of this study was to es tablish if there is an accordance between a subjective nose breathing assessment and objective methods (rhinoma nometry and acoustic rhinometry) in assessing nose breathing function in patients with diverse nasal septum deformity degrees, as well as to establish an accordance between these two objective methods.
This study involved the total of 90 examinees divided into three groups. The group I consisted of examinees with nasal septum deformities less than 10 degrees. The group II consisted of examinees with nasal septum deformities ranged from 10 degrees to 15 degrees. The group III involved examinees with nasal septum deformities over 15 degrees. Each examinee had subjec tively graded his/her nasal breathing on the side of the nose septum deformity from 0 to 10, and afterwards the whole noses. Rhinomanometry and acoustic rhinometry were done on the side of the nasal septum deformities and after that on the other side of the nose using the Interacoustics SRE 2000 device.
In the groups II and III there was a positive correlation between a subjective nose breathing assessment and rhinomanometric values both on the side of the nasal septum deformities and the nose as a whole, (p < 0.05), and no correlation between these traits in the group I (p > 0.05). In none of the exam ined groups correlation was found between a subjective nose breathing assessment and rhinometric values, both minimum cross-sectional area (MCA) and volume (VOL), both on the side of the nasal septum deformities and the nose as a whole (p > 0.05). There was no correlation found between rhinomanometric and rhinometric MCA and VOL values in either on the sides of nasal septum deformities or the nose as a whole in any of the examined groups (p > 0.05).
Rhinomanometry significantly correlates with the subjective nose breathing assessment and it can be used as a reliable and objective indicator of nose breathing in everyday clinical practice. Acoustic rhinometry, on the other hand, which does not correlate with a subjective nose breathing assessment could have a greater significance in a scientific sense than in clinical applying.

ABSTRACT Introduction: Maximal effort physiological tests provide information about the current functional capacity of athletes. Objective: The aim of this study was to evaluate anaerobic performance ...parameters in elite athletes and to compare them in terms of the specific demands of each sport. We also created and applied the new software which enables us to quantify a new parameter -explosive muscle power (EP), a major component in sports requiring explosive bursts of movement lasting from a few seconds to 1 or 2 minutes. This new parameter reflects the velocity of energy transformation from intramuscular ATP and high-energy phosphates into mechanical power. Methods: All Wingate test parameters (standard parameters) - anaerobic power (AP), anaerobic capacity (AC), and explosive power (EP) as the new parameter were recorded in 104 subjects: 30 non-athletes and 74 athletes divided into different groups depending on their sport specialty (20 rowers, 28 wrestlers and 26 soccer players). Results: Anaerobic power (AP), anaerobic capacity (AC) and explosive power (EP) were significantly higher in the group of athletes compared to non-athletes. Among athletes, significant differences were observed in some parameters according to the type of activities they are involved in. The highest values were recorded in the group of wrestlers (AP=836W; AC=16.6kJ; EP=139W/s). The values of AP (absolute values) and EP (absolute and relative values) were significantly higher in wrestlers than in soccer players and rowers, but there was no significant difference in AC among these groups. The EP variable had a distribution similar to AP. Conclusions: Alongside anaerobic power and anaerobic capacity, the assessment of explosive power may complement the anaerobic profile of athletes. Experts in the field of sports medicine and exercise physiology could find these results useful in improving test variables, which are more important for specific sports, and for evaluating and monitoring training progress. RESUMO Introdução: Os testes fisiológicos de esforço máximo fornecem informações sobre a capacidade funcional atual dos atletas. Objetivo: O objetivo deste estudo foi avaliar os parâmetros de desempenho anaeróbico em atletas de elite e compará-los em relação às demandas específicas de cada esporte. Além disso, criamos e aplicamos o novo software que possibilita a quantificação de um novo parâmetro - força muscular explosiva (FE), um componente importante em esportes que requerem explosões de movimento que duram de alguns segundos a 1 ou 2 minutos. Este novo parâmetro reflete a velocidade de transformação de energia a partir de ATP e fosfatos de alta energia intramusculares em potência mecânica. Métodos: Todos os parâmetros de teste de Wingate (parâmetros padrão) - potência anaeróbica (PA), capacidade anaeróbica (CA) e força explosiva (FE) como um novo parâmetro foram registrados em 104 indivíduos: 30 não atletas e 74 atletas divididos em diferentes grupos, dependendo da sua especialidade esportiva (20 remadores, 28 lutadores e 26 jogadores de futebol). Resultados: A potência anaeróbica (PA), a capacidade anaeróbica (CA) e a força explosiva (FE) foram significativamente maiores no grupo de atletas em comparação com não atletas. Entre os atletas, diferenças significativas foram observadas em alguns parâmetros, de acordo com o tipo de atividades nas quais eles estão envolvidos. Os valores mais altos foram registrados no grupo de lutadores (PA = 836 W, CA = 16,6 kJ, FE = 139 W/s). Os valores de PA (valores absolutos) e FE (valores absolutos e relativos) foram significativamente maiores em lutadores do que em jogadores de futebol e remadores, mas não houve diferenças significativas na CA entre esses grupos. Métodos: Todos los parámetros de prueba de Wingate (parámetros estándar) - potencia anaeróbica (PA), capacidad anaeróbica (CA) y fuerza explosiva (FE) como un nuevo parámetro fueron registrados en 104 sujetos: 30 no atletas y 74 atletas divididos en diferentes grupos dependiendo de su especialidad deportiva (20 remeros, 28 luchadores y 26 jugadores de fútbol). Resultados: La potencia anaeróbica (PA), la capacidad anaeróbica (CA) y la fuerza explosiva (FE) fueron significativamente mayores en el grupo de atletas en comparación con los no atletas.
