ABSTRACT
Complexity is everywhere. From biology to genomics, from anthropology to public policy, experts are marveling at the complexity of the phenomena they study. Yet in recent years, scholars ...have warned against taking all of this complexity at face value. In this spirit, this article frames “complexity” as an epistemic artifact traceable through particular histories and traditions of knowledge‐making. Attributions of relative complexity to different societies have a long history in anthropology. Archaeologists, in particular, have defined and redefined, debated and deconstructed complexity, leaving behind a lively textual trail. Examining this trail as a case study, the article investigates some of the different and sometimes conflicting logics behind what gets to count as complex, when, and why, as well as behind the more general expansion of complexity's reach in recent decades. By doing so, it exemplifies a way of approaching complexity as an anthropological problem and as a dominant problematic of our times. knowledge, complexity, epistemology, archaeology
Family Constellations are an emerging therapeutic approach for working with local and non-local consciousness. First developed by German psychoanalyst Bert Hellinger, and now practiced by thousands ...of licensed and un-licensed facilitators globally, Family Constellations are a transpersonal and systemically oriented therapeutic process. Their aim is to address a focus client's emotional, behavioral, relational, or somatic issues by uncovering and resolving transgenerational entanglements within their family system. The author expands on the proposal of symbiogenesis as a mediator of local and non-local consciousness to query whether applying the Observer Effect to inherited trauma may influence epigenetic marks. An expanded perspective on consciousness, life, death, and quantum fields may provide a more comprehensive framework to address therapeutic interventions for common emotional and behavioral disorders. Innovative features of Family Constellations are its phenomenological orientation, reference to family system entanglements, and potential for symptom relief through cellular mediation of ancestral memory. Family Constellations utilize techniques called representative perception and tuning-in to identify and release ancestral traumas. These are akin to remote viewing and mediumship. While the scientific basis for Family Constellations is speculative, the text references research on the quantum theory of consciousness, mediumship and remote viewing as potential supporting evidence. Four case studies are presented.
Purpose
Nationwide mass vaccination against Covid-19 started in Israel in late 2020. Soon we identified on
18
FFDG PET-CT studies vaccine-associated hypermetabolic lymphadenopathy (VAHL) in axillary ...or supraclavicular lymph nodes (ASLN) ipsilateral to the vaccination site. Sometimes, differentiation between the malignant and benign nature of the hypermetabolic lymphadenopathy (HLN) could not be made, and equivocal HLN (EqHL) was reported. The purpose of the study was to determine the overall incidence of VAHL after BNT162b2 vaccination and also its relevance to PET-CT interpretation in oncologic patients.
Methods
A total of 951 consecutive patients that underwent
18
FFDG PET-CT studies in our department were interviewed regarding the sites and dates of the vaccine doses. A total of 728 vaccinated patients (All-Vac group) were included: 346 received the first dose only (Vac-1 group) and 382 received the booster dose as well (Vac-2 group). Studies were categorized as no HLN, malignant-HLN (MHL), VAHL, or EqHL. In studies with VAHL, location,
18
FFDG-intensity uptake and nodes size were recorded.
Results
The incidences of HLN were 45.6%, 36.4%, and 53.9% in All-Vac, Vac-1, and Vac-2 groups, respectively. VAHL was reported in 80.1% of vaccinated patients with HLN. Lower incidences of VAHL were found during the first 5 days or in the third week after the first vaccine and beyond 20 days after the booster dose. In 49 of 332 (14.8%) vaccinated patients, we could not determine whether HLN was MHL or VAHL. Breast cancer and lymphoma were the leading diseases with EqHL.
Conclusion
VAHL is frequently observed after BNT162b2 administration, more commonly and with higher intensity following the booster dose. To minimize false and equivocal reports in oncological patients, timing of
18
FFDG PET-CT should be based on the time intervals found to have a lower incidence of VAHL, and choice of vaccine injection site should be advised, mainly in patients where ASLN are a relevant site of tumor involvement.
This paper explores the ad hoc networks that form to move policies to new sites by examining the process through which the city of Bandung, Indonesia adopted a creative city policy. As revealed ...through the Bandung case, attention to the formation of these networks highlights the way that power operates within them, privileging certain ideas and structuring the ways in which information flows through their channels. This insight suggests that the concept of policy mobilities would be enriched by a closer attention to the way power is used to construct networks that promote specific policies and places as sites of ‘best practice’. Furthermore, this paper extends the concept of policy mobilities to a city ‘off the map’. In contrast to most existing policy mobilities research that tends to focus on the stable, large‐scale networks through which policies travel and on the high‐profile cities constructed as policy models, the case of Bandung provides a study of how policy mobilities may occur differently outside of its well‐established locations.

Journal Article
Background:
Clozapine is the most effective medication for treatment refractory schizophrenia, but is associated with cardiac adverse drug reactions. Myocarditis and cardiomyopathy are the most ...serious cardiac adverse drug reactions although reported rates of these conditions vary in the literature. We systematically reviewed and meta-analysed the event rates, the absolute death rates and case fatality rates of myocarditis and cardiomyopathy associated with clozapine.
Methods:
PubMed, EMBASE and PsycINFO were searched for studies that reported on the incidence of cardiomyopathy or myocarditis in people exposed to clozapine. Data were meta-analysed using a random effects model, with subgroup analysis on study size, time frame, region, quality, retrospective vs prospective, and diagnostic criteria of myocarditis or cardiomyopathy.
Results:
28 studies of 258,961 people exposed to clozapine were included. The event rate of myocarditis was 0.007 (95% confidence interval CI = 0.003, 0.016), absolute death rate was 0.0004 (95% CI = 0.0002, 0.0009) and case fatality rate was 0.127 (95% CI = 0.034, 0.377). The cardiomyopathy event rate was 0.006 (95% CI = 0.002, 0.023), absolute death rate was 0.0003 (95% CI = 0.0001, 0.0012) and case fatality rate was 0.078 (95% CI = 0.018, 0.285). Few included studies provided information on criteria for diagnosis of myocarditis and cardiomyopathy. Event rates of cardiomyopathy and myocarditis were higher in Australia.
Conclusion:
Clarity of diagnostic criteria for myocarditis remains a challenge. Observation bias may, in part, influence higher reported rates in Australia. Monitoring for myocarditis is warranted in the first 4 weeks, and treatment of comorbid metabolic syndrome and diabetes may reduce the risk of cardiomyopathy. The risks of myocarditis and cardiomyopathy are low and should not present a barrier to people with treatment refractory schizophrenia being offered a monitored trial of clozapine.
The content of these educational interventions varies but essentially covers following aspects: increased training in clozapine in identifying suitable patients for clozapine, education in side ...effects management, developing and implementing standardized protocols for medical review, supporting documentation and adverse event management, shared decision-making, and training in use of point care devices. Involving service users in shared decision-making This should include principles of shared decision-making (SDM) in initiating and maintaining clozapine treatment, support for patients and families, decision aid tools to assist SDM, capacity and consent for starting clozapine use, and internet-based educational programs to provide information for consumers, family members, and clinicians, involving service users in monitoring for safety. The syllabus, learning methods, and evaluation of learning outcomes for such a program can be developed in consultation with psychiatry residency programs across Europe in collaboration with EPA.
This is the second installment in a multi-part series starting with Corwin-Dan-Cohen arXiv:1812.05707v3. Building on previous work by Dan-Cohen-Wewers, Dan-Cohen, and F. Brown, we push the ...computational boundary of our explicit motivic version of Kim's method in the case of the thrice punctured line over an open subscheme of \operatorname {Spec}\mathbb{Z}. To do so, we develop a refined version of the algorithm of Dan-Cohen-Wewers tailored specifically to this case. We also commit ourselves fully to working with the polylogarithmic quotient. This allows us to restrict our calculus with motivic iterated integrals to the so-called depth- 1 part of the mixed Tate Galois group studied extensively by Goncharov. An application was given in Corwin-Dan-Cohen arXiv:1812.05707v3, where we verified Kim's conjecture in an interesting new case.
