En el contexto del paradigma contemporáneo de las industrias culturales y creativas, la institución museal pierde su tradicional predominio como lugar de legitimación y visibilización de la actividad ...artística y ha de compartir espacios de poder con otros agentes mediadores. Distinguimos dos factores fundamentales que han conducido a esta evolución; por un lado, un acusado y constante crecimiento del número de artistas y, por otro, un cambio radical en la percepción del arte por parte de la sociedad. Por favorable que parezca, esta situación presenta, sin embargo, complejos desafíos, en términos de sostenibilidad y capacidad de integración, que los dos modelos confrontados en este artículo deben afrontar. Bajo estas premisas, nos basamos en varios estudios que abordan estas cuestiones centrados en el contexto del arte en el Estado español. Además de analizar las similitudes y diferencias existentes entre ambos modelos, proponemos algunas recomendaciones centradas en la deseada mejora de las condiciones socioeconómicas del colectivo de artistas y en un mayor aprovechamiento de su trabajo, en términos de productividad, mediante la optimización de la interactividad entre artistas y el público.
ABSTRACT
The purpose of this study was to describe an unusual case of a lingual schwannoma associated with a mixed connective tissue disease (MCTD). A case report. A lingual schwannoma with loss of ...lingual sensitivity and slightly increased size in an 18‐year‐old patient with MCTD was correctly diagnosed through a biopsy and no reoccurrence was observed one year after the surgical removal of the tumor and sensitivity returned 3 months after surgery. This case was considered uncommon, making the clinical diagnosis challenging in view of the diversity of possibilities for its differential diagnosis, thus showing the importance of a biopsy to confirm the diagnosis and long term follow up in such cases.
Reducing blood loss and transfusions in patients operated on primary TKR is associated with a better clinical and functional outcome. The use of Tranexamic Acid (ATX) is one of the methods used to ...decrease that bleeding.
Results in Hb, Hto, rate of bleeding and transfusion, surgical time and pain between groups A (with ATX) and group B (without tranexamic acid) after TKR are compared. Results: Statistically significant differences were found during surgery and perisurgical bleeding, in the decrease of Hb at 24 hours, transfusion rate, surgical time and pain to wandering in favor of group A.
There are no published studies in Mexico on the use of ATX in patients operated on TKR. We consider its use to be increasingly common, the optimal dose and route of administration remains a controversial topic.
The use of ATX in the proposed scheme is safe and effective in reducing the rate of bleeding and transfusions in patients operated on ATR.
On his first day in office, on 1 December 2018, freshman President of Mexico, Andrés Manuel López Obrador (AMLO) delivered a speech outlining 100 policy priorities of his administration. The present ...study analyzed the contributions of this government's program relating to food security and their environmental implications, and whether they contributed to strengthen the state or improved human security, considering that the poor and marginalized were at the center of AMLO's campaign. In total 45 policy priorities were geared to consolidate the state, while 55 contributed to improving human security. Only six were related to food security, including stipends to food producers and purchasing grains at guaranteed prices, a fertilizer distribution program and subsidies for cattle husbandry and fisheries/aquaculture. These programs contributed to advancing 10 of the 17 Sustainable Development Goals, especially those related to Zero Hunger and Reduced Inequalities. Various policy programs had explicit considerations towards climate change and land degradation, including the exclusion of natural protected areas from agricultural subsidies, and recognized that food production is vulnerable to climate change. The four agricultural programs analyzed may advance AMLO's goal of avoiding food imports, while curbing rural poverty. However, available evidence is mixed regarding animal acquisition loans, which are likely to have adverse environmental outcomes. Finally, the program for developing agroforestry operations is already contributing to deforestation, and further ecosystem degradation is most likely to occur from the introduction of timber and fruit species to natural forests as this program does not preclude the inclusion of recently cleared plots. If human development goals are to be reached, along with fulfilling the international commitments on sustainable development and environmental conservation, policies need to be implemented that simultaneously tend to a booming transnational industry, while bringing forward the rural poor, who amount to nearly half of the country's population.
To evaluate respiratory muscle strength, oxygenation and chest pain in patients undergoing off-pump coronary artery bypass (OPCAB) using internal thoracic artery grafts comparing pleural drain ...insertion site at the subxyphoid region versus the lateral region.
Forty patients were randomized into two groups in accordance with the pleural drain site. Group II (n = 19) -pleural drain exteriorized in the intercostal space; group (SI) (n = 21) chest tube exteriorized at the subxyphoid region. All patients underwent assessment of respiratory muscle strength (inspiratory and expiratory) on the pre, 1, 3 and 5 postoperative days (POD). Arterial blood gas analysis was collected on the pre and POD1. The chest pain sensation was measured 1, 3 and 5 POD.
A significant decrease in respiratory muscle strength (inspiratory and expiratory) was seen in both groups until POD5 (P <0.05). When compared, the difference between groups remained significant with greater decrease in the II (P <0.05). The blood arterial oxygenation fell in both groups (P <0.05), but the oxygenation was lower in the II (P <0.05). Referred chest pain was higher 1, 3 and 5 POD in the II group (P <0.05). The orotracheal intubation time and postoperative length of hospital stay were higher in the II group (P <0.05).
Patients submitted to subxyphoid pleural drainage showed less decrease in respiratory muscle strength, better preservation of blood oxygenation and reduced thoracic pain compared to patients with intercostal drain on early OPCAB postoperative.
