Objective. To determine the effect on voice parameters in pupils and their teacher of a three-week Workplace Vocal Health Promotion Program (WVHPP) in a school setting.
Methods. Longitudinal study ...with a design phase and a pilot phase. Voice recordings of connected speech were used to collect information on voice functioning before and after the implementation of the program. The workplace vocal health promotion program was implemented with two purposes: (1) to reduce noisy conditions inside the classrooms that may cause loud voice production, which has been found associated with voice disorders among teachers and students and (2) to increase awareness about the relationship between noise and voice inside the classrooms.
Results. The WVHPP was found to be statistically associated with a decrease in the fundamental frequency, standard deviation of the fundamental frequency, sound pressure levels, and SPL standard deviation among both pupils and their teacher. However, there was no statistically significant effect on cepstral peak prominence smoothed.
Conclusion. The three-week WVHPP was found to have a significant impact on several acoustic parameters, such as the fundamental frequency and sound pressure levels. These findings highlight the potential of the WVHPP as an effective intervention for improving the vocal health of both teachers and students. However, the lack of a significant effect on cepstral peak prominence smoothed suggests that further research is needed to fully understand the impact of the WVHPP on voice quality.
Summary Objectives This study aimed to determine the changes in self-reported voice and noise condition over a follow-up of 4 days (equivalent to one working week), to define the relationship between ...the objective voice parameters and the self-reported voice condition, as well as to characterize the relationship between classroom acoustics and the self-reported noise condition. Study design This is a cohort study. Methods We performed voice monitoring of 27 teachers for four working days using the Voice-Care device, which provides information on the fundamental frequency, vocal sound pressure level, and phonation time percentage. The participants performed a pre-monitoring, which consisted of a brief conversation, before each monitoring session, and filled in a questionnaire after each monitored lesson, in which they indicated their opinions about their voice condition and the classroom noise conditions. Results The teachers who, during the pre-monitoring, showed a higher standard deviation of the vocal sound pressure level and a greater phonation time percentage difference between the entire monitoring and the pre-monitoring sessions, reported fewer voice complaints. Decay time (DT40ME ), a reverberation measure from the speakers' perspective, resulted to be associated with the self-reporting of the noise condition. Conclusion Voice disorders at work prevention programs should include strategies to exercise the respiratory and laryngeal components of voice production, because these elements may influence the variation in the vocal sound pressure level, which was found to be significantly associated with the self-reported voice condition. This study also highlights the importance of including reverberation measures, from the speakers' perspective, in the design of schools.
Previous studies on the influence of noise and acoustics in the classroom on voice symptoms among teachers have exclusively relied on self-reports. Since self-reported physical conditions may be ...biased, it is important to determine the role of objective measurements of noise and acoustics in the presence of voice symptoms. To assess the association between objectively measured and self-reported physical conditions at school with the presence of voice symptoms among teachers. In 12 public schools in Bogotα, we conducted a cross-sectional study among 682 Colombian school workers at 377 workplaces. After signed the informed consent, participants filled out a questionnaire on individual and work-related conditions and the nature and severity of voice symptoms in the past month. Short-term environmental measurements of sound levels, temperature, humidity, and reverberation time were conducted during visits at the workplaces, such as classrooms and offices. Logistic regression analysis was used to determine associations between work-related factors and voice symptoms. High noise levels outside schools (odds ratio OR = 1.83; 95% confidence interval CI: 1.12-2.99) and self-reported poor acoustics at the workplace (OR = 2.44; 95% CI: 1.88-3.53) were associated with voice symptoms. We found poor agreement between the objective measurements and self-reports of physical conditions at the workplace. This study indicates that noise and acoustics may play a role in the occurrence of voice symptoms among teachers. The poor agreement between objective measurements and self-reports of physical conditions indicate that these are different entities, which argue for inclusion of physical measurements of the working environment in studies on the influence of noise and acoustics on vocal health.
Highlights ► Voice disorders were highly prevalent among teachers. ► A large variation in reported prevalence of voice disorders was observed. ► Various work-related factors seem present, such as ...acoustic and noise conditions. ► Associations were not influenced by the definition of prevalence or by the MQS. ► All evidences were based on cross-sectional studies.
To determine the relationship between working conditions, especially room acoustics, with voice symptoms and voice-related quality of life among Colombian school and college teachers during online ...classes in times of COVID-19 pandemic.
Exploratory cross-sectional study. Participants were thirty-two primary, secondary and university Colombian teachers, who were invited to fill in an online survey about working conditions and voice symptoms during online classes in times of COVID-19 pandemic. In the statistical analysis, four dependent variables and eight independent variables were defined. We used Generalized Linear Model with a Gamma distribution and Binary Logistic Regression to determine the association between voice with working conditions.
High reverberation time and stress during online classes in times of the COVID-19 pandemic reduced slightly the VRQOL scores. For Factor 3 of the VFI, high reverberation time and months teaching online classes were associated at the multivariate analysis. Stress and years of experience were statistically associated with an increased odds of reporting hoarseness.
Similar to pre-COVID-19, teachers with online classes during the COVID-19 pandemic are affected by two work-related factors namely high reverberation time and stress. These two factors are increasing their likelihood of reporting hoarseness and having lower voice-related quality of life, which may confirm the relationship between reverberation, stress, and voice also during online classes. This highlights the need of implementing workplace vocal and mental health programs to decrease the occurrence of voice problems among teachers in times of and post-COVID-19 pandemic.
Since the beginning of the new pandemic, COVID-19 health services have had to face a new scenario. Voice therapy faces a double challenge, interventions using telepractice, and delivering ...rehabilitation services to a growing population of patients at risk of functional impairment related to the COVID-19 disease. Moreover, as COVID-19 is transmitted through droplets, it is critical to understand how to mitigate these risks during assessment and treatment.
To promote safety, and effective clinical practice to voice assessment and rehabilitation in the pandemic COVID-19 context for speech-language pathologists.
A group of 11 experts in voice and swallowing disorders from five different countries conducted a consensus recommendation following the American Academy of Otolaryngology-Head and Neck Surgery rules building a clinical guide for speech-language pathologists during this pandemic context.
The clinical guide provides 65 recommendations for clinicians in the management of voice disorders during the pandemic and includes advice from assessment, direct treatment, telepractice, and teamwork. The consensus was reached 95% for all topics.
This guideline should be taken only as recommendation; each clinician must attempt to mitigate the risk of infection and achieve the best therapeutic results taking into account the patient's particular reality.
Summary Objectives To investigate the medical costs and productivity costs of voice symptoms among teachers and to assess the contribution of the characteristics of voice symptoms, sociodemographic ...characteristics, health conditions, and work-related factors to these costs. Study design This is a cross-sectional study. Methods In 2012, we conducted a longitudinal study in 12 public schools in Bogotá D.C., Colombia. This study is focused on cross-sectional results obtained in the first stage of the data collection process. Participants filled out a questionnaire on sociodemographics, voice symptoms, work-related conditions, use of health care, productivity loss at work, and sickness absence. Multiple logistic regression analysis was used to explore associations among health care use, voice-related absenteeism and productivity loss with duration and severity of voice symptoms, sociodemographic characteristics, health conditions, and work-related factors. Results In total, 621 Colombian teachers participated in this research, 438 of whom had self-reported voice complaints and who therefore made up the study population. Total medical costs and productivity costs due to presence of voice symptoms among teachers with voice complaints equaled around 37% of their monthly wage. Approximately, 3% of the costs were direct costs for health care use, and 97% were indirect costs for productivity losses. Severity of voice symptoms was significantly associated with health care use and absenteeism. Conclusions Voice symptoms among teachers have important economic consequences because of health care use, voice-related absenteeism, and productivity loss at work.
Vocal instructors during their normal workday are exposed to high noise levels that can affect their voice and hearing health. The goal of this study was to evaluate the voice and hearing status of ...voice instructors before and after lessons and relate these evaluations with voice and noise dosimetry taken during lessons.
Eight voice instructors volunteered to participate in the study. The protocol included (1) questionnaires, (2) pre/post assessment of voice quality and hearing status, and (3) voice and noise dosimetry during lessons. Acoustic measurements were taken of the unoccupied classrooms.
In six of eight classrooms, the measured noise level was higher than the safety recommendations set by National Institute for Occupational Safety and Health. The background noise level and the reverberation time in the classrooms were in compliance with the national standard recommendations. We did not find a clear pattern comparing pre- and post-measurements of voice quality consistent among genders. In all subjects, the Sound Pressure Levels mean increased, and the standard deviation of fundamental frequency decreased indicating association to vocal fatigue. Previous studies link these changes to increasing vocal fatigue. The audiometric results revealed seven out of eight instructors have sensorineural hearing loss.
The interaction of the acoustic space and noise levels can contribute to the development of hearing and voice disorders for voice instructors. If supported by larger sample size, the results of this pilot study could justify the need for a hearing and voice conservation program for music faculty.
We determined the natural course of voice complaints among school workers and established the risk factors associated with incidence and chronic voice complaints.
We conducted a longitudinal study ...with an 11-month follow-up among 682 school workers. Participants filled out a questionnaire on individual and work-related conditions and the nature and severity of voice complaints. All participants who provided baseline data were contacted in the 11-month follow-up, if they were still working in the school. Short-term environmental measurements of physical work-related factors were conducted during visits at the workplaces. Logistic regression analysis was used to determine associations between work-related factors and voice complaints.
We found a high occurrence of chronic voice complaints, a low recovery of 22%, and an annual incidence of 44%. A self-reported high noise level at the workplace was associated with the incidence of voice complaints (odds ratio = 2.45). Self-reported poor acoustics in the classroom was associated with chronic voice complaints (odds ratio = 1.76).
This unique longitudinal study among school workers presented some indications that self-reported high noise levels may contribute to the incidence of voice complaints, whereas self-reported poor acoustic conditions may be an important associated factor of chronic voice complaints.
