El capital social se considera un determinante estructural de desarrollo y bienestar social. Su componente cognitivo evalúa el grado de confianza de la población en sus sistemas de organización ...social, así como las interacciones comunitarias que estructuran respuestas sociales a los problemas sociales. Existen pocas escalas disponibles para la medición de este constructo. Este trabajo presenta los resultados de la adaptación al español y validación psicométrica de una escala para la medición de capital social en contextos rurales. Se adaptó al español la escala de capital social cognitivo de Wang. Se aplicaron 1200 cuestionarios a adultos en 12 veredas de Tierralta (Colombia) seleccionados con muestreo aleatorio simple estratificado. Se realizó análisis factorial de la escala a partir de una matriz de correlación policórica. El análisis factorial exploratorio sugiere la existencia de dos factores principales distribuidos así: 7 ítems para el factor 1 (confianza) (valor propio 3.23.) y 2 ítems para el factor 2 (desconfianza) (valor propio 1.40). Como fue observado por Wang, Q9 y Q10 parecen preguntas ambiguas que no aportan suficiente a ninguno de los factores. Se presenta la primera validación factorial al español de la escala de capital social de Wang en el contexto social de la Colombia rural.
RESUMEN Introducción En 2013, los Estados Miembros de la OPS reconocieron la epidemia de enfermedad renal crónica de causas no tradicionales (ERCnT) como un grave problema de salud pública. Este ...artículo describe el establecimiento de prioridades de investigación para abordar de manera integral la ERCnT en Centroamérica. Métodos Se estructuró una encuesta virtual utilizando la metodología Delphi mediante una búsqueda de estudios de investigación efectuados en Centroamérica y de agendas de investigación previas sobre la ERC. Los encuestados se identificaron en diversas fuentes. La primera ronda buscó refinar y añadir tópicos de investigación y priorizar los más relevantes. La segunda ronda priorizó los tópicos más relevantes. Se realizó un análisis por fuzzy sets para estimar umbrales de decisión y puntajes por tópico. Resultados La encuesta se envió a 83 personas de habla hispana y 38 de habla inglesa y respondió 46,2%. Para la segunda ronda, se envió la encuesta a 56 personas en español y 16 en inglés que habían contestado a la la primera. Se priorizaron 18 tópicos de investigación enmarcados en 10 áreas: políticas públicas, determinantes, etiología, diagnóstico y tratamiento de la ERC, prevención primaria, prestación de servicios, recursos humanos, sistemas de información y financiamiento. Se comprobó que la investigación en ERCnT es escasa y está restringida a ciertos tópicos. Conclusiones Además de los factores etiológicos, se dio gran relevancia a aspectos relacionados con la respuesta de los sistemas de salud, incluidos el abordaje de la prestación de servicios, los recursos humanos, el financiamiento y aspectos ocupacionales y ambientales.
ABSTRACT Introduction In 2013, the PAHO Member States recognized the epidemic of chronic kidney disease of non-traditional causes (CKDnT) as a serious public health problem. This article describes ...the establishment of research priorities to comprehensively address CKDnT in Central America. Methods Following a search of the literature for research studies carried out in Central America and prior research agendas on CKD, a virtual survey was conducted using the Delphi methodology. The respondents were identified from various sources. The first round sought to refine and add research topics and to prioritize those deemed most relevant. The second round prioritized the most relevant topics. A fuzzy-sets analysis was carried out to estimate decision thresholds and scores for each topic. Results The survey was sent to 83 Spanish-speaking and 38 English-speaking prospective respondents. The response rate was 46.2%. For the second round, the survey was sent to 56 Spanish-speaking and 16 English-speaking first-round respondents. Eighteen topics within 10 research areas were prioritized: public policies, determinants, etiology, diagnosis and treatment of CKD, primary prevention, service delivery, human resources, information systems, and funding. Research on CKDnT was found to be scarce and restricted to certain topics. Conclusions In addition to etiological factors, great importance was assigned to aspects related to the health system response, including service delivery approaches, human resources, funding, and occupational and environmental aspects.
To describe strategies that contribute to the comprehensive approach to the judicialization of health in countries of Latin America and the Caribbean.
A search was structured to identify articles ...presenting strategies to approach the judicialization of health. A survey was designed, which included actors of the health system and judiciary sector. We prioritized the strategies qualified by more than the 50.0% of the participants as "very relevant". Strategies were categorized according to: governance, provision of services, human resources, information systems, financing, and medical products.
We included 64 studies, which identified 50 strategies, related to the sub-functions and components of health systems. Of the 165 people who answered the survey, 80.0% were aged 35-64 years. The distribution of men and women was homogeneous. Half of the respondents were from Colombia (20.0%), Uruguay (16.9%), and Argentina (12.7%). We prioritized strategies that addressed aspects of generation of useful scientific evidence for decision making according to the health needs of the population, empowerment for the society, and creating spaces for discussion of measures of inclusion or exclusion of health technologies. The executive and judiciary decision makers prioritized questions that dealt with strategies that would ensure accountability.
The results of this study contribute to the identification of effective strategies to approach the phenomenon of judicialization of health, guaranteeing the right to health.
Describir estrategias que contribuyan al abordaje integral de la judicialización de la salud en países de América Latina y El Caribe.
Se estructuró una búsqueda para identificar artículos que presentaran estrategias para el abordaje de la judicialización en salud. Se diseñó una encuesta, en donde se incluyeron actores del sistema de salud y del sector judicial. Se priorizaron las estrategias calificadas por más del 50,0% de los participantes como "muy relevantes". Se categorizaron las estrategias según: gobernanza, prestación de servicios, recursos humanos, sistemas de información, financiación y productos médicos.
Se incluyeron 64 estudios en los cuales se identificaron 50 estrategias, relacionadas con las sub-funciones y componentes de los sistemas de salud. De las 165 personas que respondieron la encuesta, el 80,0% tenían entre 35-64 años. La distribución de hombres y mujeres fue homogénea. La mitad de los respondientes fue de Colombia (20,0%), Uruguay (16,9%) y Argentina (12,7%). Se priorizaron mayormente las estrategias que abordaron los aspectos de generación de evidencia científica útil para toma de decisión según las necesidades de salud de la población, el empoderamiento para la sociedad, y la generación de espacios de discusión de las medidas de inclusión o exclusión de tecnologías de salud. Los tomadores de decisión de la rama ejecutiva y judicial priorizaron las preguntas que abordaron las estrategias que garantizaran la rendición de cuentas.
Los resultados de este estudio contribuyen a la identificación de estrategias efectivas para el abordaje del fenómeno de la judicialización en salud, garantizando el derecho a la salud.
RESUMEN Objetivos Identificar los comportamientos de riesgo para la salud y describir su relación con las características sociodemográficas en los adolescentes de un colegio del municipio de Tabio, ...provincia de Sabana Centro, Cundinamarca, Colombia. Metodología Estudio descriptivo y de corte transversal en población adolescente escolar de 14 a 19 años, durante el año 2018, elegidos por medio de muestreo por conveniencia. Se obtuvo información mediante el cuestionario en español de la herramienta RAAPS (Rapid Assessment for Adolescent Preventive Services), que evaluó los comportamientos de riesgo, previa firma de consentimiento y asentimiento informado. Se realizó análisis estadístico en el programa STATA versión 14.0. Resultados Se incluyó una muestra de 217 estudiantes, con edad promedio de 15,8 años (53% mujeres y 47% hombres). La mayoría de zona rural y estrato 2. Dentro de los comportamientos de riesgo detectados se destacan la falta de uso de cinturón de seguridad y casco en casi la mitad de los participantes, síntomas de tristeza en 37,3% (que afecta más a las mujeres), consumo de alcohol en el 45,2% y actividad sexual en el 30,9% (son más frecuentes en el grupo de 17 a 19 años y en el sexo masculino). Conclusión Los hallazgos confirman la presencia de comportamientos de riesgo para la salud en los adolescentes de la población estudiada, que deben ser intervenidos a través de estrategias preventivas efectivas y que alertan sobre la necesidad de implementar un sistema periódico de vigilancia.

La segunda ronda fue dirigida a los investigadores y a los decisores. Cada pregunta se priorizó cuando el 50% (o más) de los encuestados la calificó como "relevante" o "muy relevante". RESULTADOS: En la primera ronda se enviaron 20 preguntas sobre recursos humanos y 33/66 investigadores respondieron. Se agregaron preguntas sugeridas por los investigadores, quedando finalmente 26 preguntas para la segunda ronda que fueron enviadas a 121 investigadores y decisores. La representación de los respondientes por países fue homogénea en ambas rondas. En la segunda ronda 14/26 (54%) preguntas fueron calificadas como muy relevantes; se priorizaron temas relacionados con la regulación del mercado, integración de la formación y las necesidades de atención, y distribución del recurso humano. METHODS: An online survey was designed based on a search in PubMed, Cochrane Library, and LILACS that contributed previously prioritized research questions. Respondents, mainly researchers and decision-makers, were identified through various sources. The first round, directed at researchers, aimed at refining and adding research questions and prioritizing questions that researchers regarded as relevant or very relevant. The second round was directed at researchers and decision-makers. A question was considered a priority when 50% (or more) of respondents described it as "relevant" or "very relevant." RESULTS: The first round included 20 questions on human resources and 33/66 researchers responded. Questions suggested by the researchers were added, resulting in 26 questions for the second round, which were sent to 121 researchers and decision-makers. Respondent representation by country was uniform in both rounds. In the second round, 14/26 (54%) questions were described as very relevant. Priority issues related to regulation of the market, integration of education and health care needs, and distribution of human resources. The response rate was 50% in the first round (33/66), and 34% in the second round (41/121). CONCLUSIONS: The results of this exercise provide a starting point for mobilization of resources for health policy and systems research. Identification of health systems research priorities is an effective and efficient strategy for reorienting political, financial, management, and social organization efforts for attaining universal health coverage.

Objetive.Evaluating the safety of acute management of migraine during pregnancy. Materials and methods. A systematic review was made of the literature concerning observational analytical studies. A systematic search and selection was made of all analytical studies (cohort studies and cases and controls studies) regarding the acute management of migraine during pregnancy published between January 1966 and September 2007. The search covered the COCHRANE, MEDLINE, EMBASE and LILACS databases. Data were extracted using the PECOT strategy bearing in mind the intervention strategy, methodological quality and presence of greater or lesser congenital malformations related to the different medicaments used for the acute management of migraine. Results. A total of 389 references were obtained of which 7 articles were selected by title and summary. Four articles complied with the inclusion criteria. No articles were found describing the risk of congenital malformations before being exposed to acetaminophen, anti-inflammatory agents non-steroidal, ergot alkaloids and/or opioids; just articles related to tryptans (specifically sumatryptan) were found. Conclusions. Only data concerning the risk of congenital malformations arising from sumatryptan use was found regarding all the medicaments used for acute migraine attack, this being insufficient as the information was really poor and the studies had limitations, thereby making it difficult to make statements concerning their safety during pregnancy.
