Several societies have published recommendations for evaluating older adults with cancer in standard conditions. It is vital to assure a proper systematic patient condition evaluation, not only in ...the oldest (geriatric assessment) but in all adult patients. We have investigated the feasibility of a systematic evaluation of the general condition of all patients diagnosed with hematologic malignancies, and the degree of acceptance by the clinical team, in a prospective cohort of 182 consecutive adults, by using the ECOG performance status scale (ECOG, age 18 and over, 18+), Lee Index for Older Adults (LEE, 50+), Geriatric Assessment in Hematology (GAH, 65+), and the Comprehensive Geriatric Assessment (CGA, 75+). Clinical team acceptance was analyzed with a visual analogue scale, and the objective feasibility was calculated as the proportion of patients that could be finally evaluated with each tool. Acceptance was high, but the objective feasibility was progressively lower as the complexity of the different tools increased (ECOG 100%, LEE 99.4%, GAH 93.2%, and CGA 67.9%). LEE and GAH categories showed a weak concordance (Cohen’s Kappa 0.24) that was slight between LEE and CGA (Kappa 0.18). Unexpectedly, we found no significant association between the GAH and CGA categories (p = 0.16). We confirm that a systematic evaluation of all adult patients diagnosed with hematologic malignancies is feasible in daily practice by using an age-adapted approach. Direct comparisons among the different predictive tools in regard to patients’ tolerance to treatments of different intensities must be a priority research subject in the coming years.
Type 2 diabetes exerts a negative impact on sleep breathing. It is unknown whether a long-term improvement in glycemic control ameliorates this effect. We conducted an interventional study with 35 ...patients with type 2 diabetes and obstructive sleep apnea (OSA) to explore this. At home, sleep breathing parameters were assessed at baseline and after a 4-month period in which antidiabetic therapy was intensified. Patients who decreased their body mass index ≥2kg/m
were excluded. Those with an HbA1c reduction ≥0.5% were considered good responders (
= 24). After the follow-up, good responders exhibited an improvement in the apnea-hypopnea index (AHI: 26-1 (95% IC: 8.6-95.0) vs. 20.0 (4.0-62.4) events/hour,
= 0.002) and in time with oxygen saturation below 90% (CT90: 13.3 (0.4-69.0) vs. 8.1 (0.4-71.2) %,
= 0.002). No changes were observed in the group of non-responders (
= 0.722 and
= 0.138, respectively). The percentage of moderate and severe OSA decreased among good responders (
= 0.040). In the wider population, the change in HbA1c correlated positively to decreases in AHI (r = 0.358,
= 0.035) and negatively to increases in the minimum arterial oxygen saturation (r = -0.386,
= 0.039). Stepwise multivariate regression analysis showed that baseline AHI and the absolute change in HbA1c independently predicted decreased AHI (R
= 0.496). The improvement of glycemic control exerts beneficial effects on sleep breathing parameters in type 2 diabetes, which cannot be attributed merely to weight loss.
PURPOSE:To present successful management of bilateral limbal stem cell deficiency (LSCD) by using an allogeneic limbal epithelial stem cell transplantation together with solid activated platelet-rich ...plasma (PRP).
METHODS:A 59-year-old man with a history of bilateral LSCD due to penicillin-induced Stevens–Johnson Syndrome suffered from a lime corneal burn in his right eye, leading to a total LSCD with severely reduced visual acuity. After stabilizing the ocular surface, we performed an allogeneic limbal epithelial transplantation from a cadaveric donor using an autologous clot of PRP to cover the limbal grafts to nourish the ocular surface microenvironment.
RESULTS:At the first week after the procedure, the corneal epithelium had fully reepithelized. At month 3, visual acuity improved from hand motion to 20/70.
CONCLUSIONS:In this case, this new modified procedure was a promising, easy-to-perform, apparently safe, and effective treatment option to enhanced epithelial wound healing in ocular surface diseases. To our knowledge, this is the first report describing the incorporation of solid PRP in limbal transplantation procedures.
In order to compare spirometric maneuvers in adults according to the presence of type 1 diabetes, a case-control study including 75 patients with type 1 diabetes and 75 controls matched by sex, age, ...and body mass index were designed. In addition, 75 patients with type 1 diabetes were added to examine the potential the impact of subcutaneous insulin therapy on pulmonary function. Lung function measurements were assessed according to the global initiative for chronic obstructive lung disease guidelines. Basal insulin included long-acting insulin analogues and the delivered background insulin in patients with pump therapy. Bolus insulin included rapid-acting insulin analogues and the delivered insulin to cover postprandial hyperglycemias. Patients with type 1 diabetes showed lower spirometric values in comparison to the control group, together with a higher prevalence of forced expiratory volume in the first second (FEV1) <80% (10.7% vs. 2.7%,
= 0.044) and restrictive ventilatory pattern (10.7% vs. 0%,
= 0.006) The dose of basal insulin (U/kg/day) showed a negative correlation with forced vital capacity (FVC) (
= -0.205,
= 0.012) and FEV1 (
= -0.182,
= 0.026). The optimal cut-off value for identifying patients with a restrictive spirometric pattern was 0.5 U/kg/day of basal insulin. Additionally, basal insulin (U/kg/day) independently predicted the presence of both a restrictive spirometric pattern (OR = 77.1 (3.2 to 1816.6),
= 0.007) and an abnormal FEV1 (OR = 29.9 (1.5 to 562.8),
= 0.023). In patients with type 1 diabetes, higher basal insulin dosage seems to be related with an impairment of pulmonary function.
Propolis is a natural product of bees with biological activities that are mainly associated with bee type and geographic origin. Propolis extract has been proposed with several applications in ...environmental health. The ethanol extracts have shown good antimicrobial activity. The association of this technique with ultrasound-assisted extraction has been studied to improve the characteristics of the obtained extracts. Thus, the objective of this work is to verify the antiviral action against two strains of bacteriophages of two extracts of Brazilian propolis (green and red) obtained by conventional extraction and ultrasonic extraction. The activities of the propolis red and green extracts were confirmed by the significant ~3 and ~4.5 Log 10 PFU/mL reduction in the concentrations of the MS2 and Av-08 bacteriophages, respectively. It was found that ultrasound-assisted extraction is comparable to the maceration process and demonstrated the best antiviral activities. Brazilian red propolis was more effective than green propolis in viral reduction in all treatments.
Pregnant women who are infected with SARS-CoV-2 are at an increased risk of adverse perinatal outcomes. With this study, we aimed to better understand the relationship between maternal infection and ...perinatal outcomes, especially preterm births, and the underlying medical and interventionist factors. This was a prospective observational study carried out in 78 centers (Spanish Obstetric Emergency Group) with a cohort of 1347 SARS-CoV-2 PCR-positive pregnant women registered consecutively between 26 February and 5 November 2020, and a concurrent sample of PCR-negative mothers. The patients’ information was collected from their medical records, and the association of SARS-CoV-2 and perinatal outcomes was evaluated by univariable and multivariate analyses. The data from 1347 SARS-CoV-2-positive pregnancies were compared with those from 1607 SARS-CoV-2-negative pregnancies. Differences were observed between both groups in premature rupture of membranes (15.5% vs. 11.1%, p < 0.001); venous thrombotic events (1.5% vs. 0.2%, p < 0.001); and severe pre-eclampsia incidence (40.6 vs. 15.6%, p = 0.001), which could have been overestimated in the infected cohort due to the shared analytical signs between this hypertensive disorder and COVID-19. In addition, more preterm deliveries were observed in infected patients (11.1% vs. 5.8%, p < 0.001) mainly due to an increase in iatrogenic preterm births. The prematurity in SARS-CoV-2-affected pregnancies results from a predisposition to end the pregnancy because of maternal disease (pneumonia and pre-eclampsia, with or without COVID-19 symptoms).
Objetivo: analizar las propiedades psicométricas de la escala de experiencias de recuperación en trabajadores dependientes (asalariados) peruanos.
Método: estudio transversal donde participaron ...voluntariamente 462 trabajadores dependientes de ambos sexos, de entre 20 a 66 años de edad, de las tres regiones del Perú. Los participantes fueron reclutados de empresas del sector financiero, turismo, educación, salud y telecomunicaciones. El instrumento objeto de validación fue la escala de experiencias de recuperación validada para profesores peruanos. Se analizó la estructura interna mediante el análisis factorial confirmatorio, la consistencia interna con el coeficiente Alpha de Cronbach y la validez convergente a través del coeficiente de correlación de Pearson.
Resultados: el análisis factorial confirmatorio reportó un ajuste adecuado para la estructura original de cuatro factores (x2= 141.782., p < 0,001, CFI = 0,981, RMSEA = 0,065. y SRMR = 0,035). Las cargas factoriales resultantes se encuentran entre 0,71 y 0,90 y la consistencia interna ? entre 0,81 y 0,92. Con respecto a la validez convergente, se hallaron correlaciones directas y significativas entre las dimensiones de experiencias de recuperación y la escala de bienestar general (r= 0,126 a r=0,287).
Conclusiones: la escala de experiencias de recuperación demuestra validez basada en la estructura interna, fiabilidad y validez convergente, y puede ser aplicada en la gestión de la salud ocupacional para trabajadores dependientes peruanos.
Pyrethroid contact insecticides are mainstays of malaria control, but their efficacies are declining due to widespread insecticide resistance in Anopheles mosquito populations, a major public health ...challenge. Several strategies have been proposed to overcome this challenge, including insecticides with new modes of action. New insecticides, however, can be expensive to implement in low-income countries. Here, we report a simple and inexpensive method to improve the efficacy of deltamethrin, the most active andmost commonly used pyrethroid, bymore than 10 times against Anopheles mosquitoes. Upon heating for only a few minutes, the commercially available deltamethrin crystals, form I, melt and crystallize upon cooling into a polymorph, form II, which is much faster acting against fruit flies and mosquitoes. Epidemiological modeling suggests that the use of form II in indoor residual spraying in place of form I would significantly suppress malaria transmission, even in the presence of high levels of resistance. The simple preparation of form II, coupled with its kinetic stability and markedly higher efficacy, argues that form II can provide a powerful, timely, and affordable malaria control solution for low-income countries that are losing protection in the face of worldwide pyrethroid resistance.
