Le nombre de passage aux urgences dans les établissements hospitaliers publics en France augmente tous les ans. L’adéquation entre l’admission au service accueil-urgence (SAU) et l’état clinique du ...patient doit être vérifiée dans le but de tenter une maîtrise en amont du SAU.
Une étude prospective a été effectuée au SAU du CHU de Nantes afin d’évaluer la proportion de patients susceptibles de bénéficier d’une hospitalisation directe, programmée dans un service de Médecine spécialisée ou polyvalente.
Cette proportion est de 10 %. Soixante-treize pour cent des patients sont âgés de plus de 60 ans. Ils sont adressés dans 77 % des cas par leur médecin traitant et dans 10,4 % des cas par le médecin de garde. Ils ont dans 33 % des cas des pathologies médicales non spécifiques d’organe, dans 20 % des cas des affections dermatologiques, dans 12 % des cas des infections broncho-pulmonaires et dans la même proportion des pathologies rhumatologiques, les autres affections étant plus rares.
Les résultats de cette étude doivent être confirmés par une étude pilote où le médecin généraliste fera hospitaliser directement en médecine, sans passage par le SAU, des patients qu’il aurait eu l’intention d’adresser au SAU. Pour ce faire, il contactera, sur un téléphone portable, le médecin hospitalier qui se chargera de trouver le lit d’hospitalisation. L’impact sur le SAU de cette nouvelle modalité d’hospitalisation sera évalué en terme de nombre d’admissions évitées au SAU.

The results of this study must be confirmed in a pilot study in which the general practitioner would refer any patient, that he would have sent to an SAU, directly to a medical department without passing through the SAU. To do so, using a cell phone, the practitioner would contact the hospital physician who would find a hospital bed. The impact of this new modality of hospitalisation on the SAU could be assessed in terms of the number of admissions avoided to the SAU.

This paper provides a snapshot of the current state of central banking doctrine in the aftermath of the crisis, using data from a questionnaire produced in 2011 and sent to central bankers (from 13 ...countries plus the euro zone) and economists (31) for a report by the French Council of Economic Analysis to the Prime Minister. The results of our analysis of the replies to the questionnaire are twofold. First, we show that the financial crisis has led to some amendments of pre-crisis central banking. We highlight that respondents to the questionnaire agree on the general principle of a 'broader' view of central banking extended to financial stability. Nevertheless, central bankers and economists diverge or give inconsistent answers about the details of implementation of this 'broader' view. Therefore, the devil is once again in the details. We point out that because of central bankers' conservatism, a return to the status quo cannot be excluded.

OBJECTIVETo describe neuroimaging findings and to report the epidemiologic and clinical characteristics of patients with coronavirus disease 2019 (COVID-19) with neurologic manifestations.
METHODSIn ...this retrospective multicenter study (11 hospitals), we included 64 patients with confirmed COVID-19 with neurologic manifestations who underwent a brain MRI.
RESULTSThe cohort included 43 men (67%) and 21 women (33%); their median age was 66 (range 20–92) years. Thirty-six (56%) brain MRIs were considered abnormal, possibly related to severe acute respiratory syndrome coronavirus. Ischemic strokes (27%), leptomeningeal enhancement (17%), and encephalitis (13%) were the most frequent neuroimaging findings. Confusion (53%) was the most common neurologic manifestation, followed by impaired consciousness (39%), presence of clinical signs of corticospinal tract involvement (31%), agitation (31%), and headache (16%). The profile of patients experiencing ischemic stroke was different from that of other patients with abnormal brain imagingthe former less frequently had acute respiratory distress syndrome (p = 0.006) and more frequently had corticospinal tract signs (p = 0.02). Patients with encephalitis were younger (p = 0.007), whereas agitation was more frequent for patients with leptomeningeal enhancement (p = 0.009).
CONCLUSIONSPatients with COVID-19 may develop a wide range of neurologic symptoms, which can be associated with severe and fatal complications such as ischemic stroke or encephalitis. In terms of meningoencephalitis involvement, even if a direct effect of the virus cannot be excluded, the pathophysiology seems to involve an immune or inflammatory process given the presence of signs of inflammation in both CSF and neuroimaging but the lack of virus in CSF.

With fewer than 8000 genes and a minimalist cellular organization, the green picoalga Ostreococcus tauri is one of the simplest photosynthetic eukaryotes. Ostreococcus tauri contains many ...plant‐specific genes but exhibits a very low gene redundancy. The haploid genome is extremely dense with few repeated sequences and rare transposons. Thanks to the implementation of genetic transformation and vectors for inducible overexpression/knockdown this picoeukaryotic alga has emerged in recent years as a model organism for functional genomics analyses and systems biology. Here we report the development of an efficient gene targeting technique which we use to knock out the nitrate reductase and ferritin genes and to knock in a luciferase reporter in frame to the ferritin native protein. Furthermore, we show that the frequency of insertion by homologous recombination is greatly enhanced when the transgene is designed to replace an existing genomic insertion. We propose that a natural mechanism based on homologous recombination may operate to remove inserted DNA sequences from the genome.
