Team-based organizational structures are now widely adopted for activities such as product development, customer support and process-improvement initiatives due to their increased likelihood of making better decisions and solving problems. However, team collaboration often faces pitfalls like information overload or misunderstandings due to goal misalignment. In this paper, we put forward the idea that computer-supported collaboration environments can have a positive impact on team collaboration by increasing team members’ awareness, focusing attention on task execution, and fostering the frequency of interaction among team members. We study the impact of recommender systems on team processes in computer-supported collaboration environments, describing the results of two experiments that show how recommendations impact interactions in teams. Teams using recommendations spent less effort on information handling and engaged more in communication than teams without recommendations.
Background
The venous access for the insertion of permanent leads of cardiac implantable electronic devices is often achieved by venous cutdown of the cephalic vein, or by “blind” puncture of the ...subclavian vein using anatomical landmarks, or by fluoroscopy‐assisted methods.
Methods
We have retrospectively analyzed our clinical experience to verify the feasibility, the safety, and efficacy of the adoption of ultrasound‐guided puncture/cannulation of the axillary vein for this purpose.
Results
Nine hundred eighty‐seven leads were placed during 548 consecutive procedures, accessing the axillary vein in the infraclavicular area using real‐time ultrasound guidance. Venipuncture was successful in 99.8% of cases. The access time was 11 seconds (range 4‐580). We recorded three cases of pneumothorax (0.5%), but no hemothorax and no hemo‐mediastinum. The incidence of local hematoma was 2.1% (12 cases). No injury to the brachial plexus or to the phrenic nerve was recorded. In a follow‐up of 33 months (range 16‐39), we observed no cases of “subclavian crush syndrome” (damage of the leads at the level of the thoracic inlet), and the rate of pocket infection/infective endocarditis was 0.7%.
Conclusion
In our experience, ultrasound‐guided puncture/cannulation of the axillary vein for implantation of permanent leads is feasible, effective, and safe. It might be considered as a first option for this procedure.
Hemorrhagic shock is the main cause of death in the prehospital environment, which highlights the need to standardize measures aiming at bleeding control and volume replacement in this environment. ...In Brazil, the first prehospital packed red blood cell transfusion service started in September 2020, in Bragança Paulista, state of São Paulo.
Describe the trends and characteristics of patients who received prehospital transfusions prior to hospital treatment during the first year of operation.
A retrospective data review was made of all patients who received transfusions from the mobile intensive care unit in Bragança Paulista over one year.
In this period, 19 patients were transfused. Since activation, the average response time was 20 minutes. The mean shock indexes before and after blood transfusion were 2.16 and 1.1, respectively. During the course of the 1st year of prehospital transfusions no blood was wasted and there were no adverse effects.
Introduction of the prehospital packed red blood cell transfusion service was successful, with significant improvement in hemodynamic parameters.
In this paper we analyze the principal preconditions and limitations for designing a competence-based Recommender System. In detail this analysis is contextualized in the ARISTOTELE European project. ...In the second part of the paper an architectural view is proposed taking in consideration the objective to propose standard and non-standard suggestions. This solution will permit to insert serendipity approaches into classical solutions.
