We present a detailed modeling and computation methodology to determine the optical Cherenkovsignals produced by upward-moving extensive air showers (EASs) induced byτ-leptons and muons,sourced from ...the interaction of high-energy astrophysical neutrinos interacting in the Earth. Fol-lowing and extending the physics modeling and Cherenkov signal simulations performed in 1, thisscheme encompasses a new, state-of-the-art computation of the muon neutrino propagation insidethe Earth and the contribution to theτ-lepton muon decay channel. The modeling takes into ac-count all possibleτ-lepton decay and muon energy loss channels that feed the optical Cherenkovemission, produced by both tau and muon initiated EASs. The EAS modeling uses the electronenergy, angular, and lateral distributions in the EAS and their evolution as well as the wavelengthdependence of the Cherenkov emission and its atmospheric attenuation. The results presented hereare focused on the detection capabilities of sub-orbital (balloon-borne) and orbital (satellite) basedinstruments. The latter case was calculated for POEMMAato compare to that presented in 1,specifically including the muon-decay channel ofτ-leptons and the muonic EAS Cherenkov signalfrom muon neutrino interactions in the Earth. By detailing all these individual contributions tothe optical Cherenkov emission and detection, we show how the ensemble that includes muonicchannels provides a large detection capability for space-based, high-energy cosmic neutrino detec-tion. Specifically, we show that for neutrino energies∼<10 PeV, the upward-EAS sensitivity due tomuon neutrino interactions in the Earth begin to dominate over that for tau neutrino interactions,effectively extending the neutrino sensitivity to lower energies.
Future experiments based on the observation of Earth's atmosphere from sub-orbital and orbital altitudes plan to include optical Cherenkov cameras to observe extensive air showers produced by ...high-energy cosmic radiation via its interaction with both the Earth and its atmosphere. As discussed elsewhere Phys. Rev. D 103, 043017 (2021); Phys. Rev. D 100, 063010 (2019), particularly relevant is the case of upward-moving showers initiated by astrophysical neutrinos skimming and interacting in the Earth. The Cherenkov cameras, by looking above Earth's limb, can also detect cosmic rays with energies starting from less than a PeV up to the highest energies (tens of EeV). Using a customized computation scheme to determine the expected optical Cherenkov signal from these high-energy cosmic rays, we estimate the sensitivity and event rate for balloon-borne and satellite-based instruments, focusing our analysis on the Extreme Universe Space Observatory aboard a Super Pressure Balloon 2 (EUSO-SPB2) and the Probe of Extreme Multi-Messenger Astrophysics (POEMMA) experiments. We find the expected event rates to be larger than hundreds of events per hour of experimental live time, enabling a promising overall test of the Cherenkov detection technique from sub-orbital and orbital altitudes as well as a guaranteed signal that can be used for understanding the response of the instruments.
The Probe Of Extreme Multi-Messenger Astrophysics (POEMMA) is designed to accurately observe ultra-high-energy cosmic rays (UHECRs) and cosmic neutrinos from space with sensitivity over the full ...celestial sky. POEMMA will observe the air fluorescence produced by extensive air showers (EASs) from UHECRs and potentially UHE neutrinos above 20 EeV. Additionally, POEMMA has the ability to observe the Cherenkov signal from upward-moving EASs induced by Earth-interacting tau neutrinos above 20 PeV. The POEMMA spacecraft are designed to quickly re-orientate to follow up transient neutrino sources and obtain currently unparalleled neutrino flux sensitivity. Developed as a NASA Astrophysics Probe-class mission, POEMMA consists of two identical satellites flying in loose formation in 525 km altitude orbits. Each POEMMA instrument incorporates a wide field-of-view (45°) Schmidt telescope with an optical collecting area of over 6 sq. m. The hybrid focal surface of each telescope includes a fast (1 μs) near-ultraviolet camera for EAS fluorescence observations and an ultrafast (10 ns) optical camera for Cherenkov EAS observations. In a 5-year mission, POEMMA will provide measurements that open new multi-messenger windows onto the most energetic events in the universe, enabling the study of new astrophysics and particle physics at these extreme energies.
OBJECTIVES: There is a need for a rapid screening test for mild cognitive impairment (MCI) and dementia to be used by primary care physicians. The Rapid Cognitive Screen (RCS) is a brief screening ...tool (< 3 min) for cognitive dysfunction. RCS includes 3-items from the Veterans Affairs Saint Louis University Mental Status (SLUMS) exam: recall, clock drawing, and insight. Study objectives were to: 1) examine the RCS sensitivity and specificity for MCI and dementia, 2) evaluate the RCS predictive validity for nursing home placement and mortality, and 3) compare the RCS to the clock drawing test (CDT) plus recall. METHODS: Patients were recruited from the St. Louis, MO Geriatric Research Education and Clinical Center (GRECC), Veterans Affairs Medical Center (VAMC) hospitals (study 1) or the Saint Louis University Geriatric Medicine and Psychiatry outpatient clinics (study 2). Study 1 participants (N=702; ages 65–92) completed cognitive evaluations and 76% (n=533/706) were followed up to 7.5 years for nursing home placement and mortality. Receiver operator characteristic (ROC) curves were computed to determine sensitivity and specificity for MCI (n=180) and dementia (n=82). Logistic regressions were computed for nursing home placement (n=31) and mortality (n=176). Study 2 participants (N=168; ages 60–90) completed the RCS and SLUMS exam. ROC curves were computed to determine sensitivity and specificity for MCI (n=61) and dementia (n=74). RESULTS: RCS predicted dementia and MCI in study 1 with optimal cutoff scores of ≤ 5 for dementia (sensitivity=0.89, specificity=0.94) and ≤ 7 for MCI (sensitivity=0.87, specificity=0.70). The CDT plus recall predicted dementia and MCI in study 1 with optimal cutoff scores of ≤ 2 for dementia (sensitivity=0.87, specificity=0.85) and ≤ 3 for MCI (sensitivity=0.62, specificity=0.62). Higher RCS scores were protective against nursing home placement and mortality. The RCS predicted dementia and MCI in study 2. CONCLUSIONS: The 3-item RCS exhibits good sensitivity and specificity for the detection of MCI and dementia, and higher cognitive function on the RCS is protective against nursing home placement and mortality. The RCS may be a useful screening instrument for the detection of cognitive dysfunction in the primary care setting.

Maternal use of certain drugs during pregnancy can result in transient neonatal signs consistent with withdrawal or acute toxicity or cause sustained signs consistent with a lasting drug effect. In ...addition, hospitalized infants who are treated with opioids or benzodiazepines to provide analgesia or sedation may be at risk for manifesting signs of withdrawal. This statement updates information about the clinical presentation of infants exposed to intrauterine drugs and the therapeutic options for treatment of withdrawal and is expanded to include evidence-based approaches to the management of the hospitalized infant who requires weaning from analgesics or sedatives.
Two short (< 2 s) γ-ray bursts (GRBs) have recently been localized and fading afterglow counterparts detected. The combination of these two results left unclear the nature of the host galaxies of the ...bursts, because one was a star-forming dwarf, while the other was probably an elliptical galaxy. Here we report the X-ray localization of a short burst (GRB 050724) with unusual γ-ray and X-ray properties. The X-ray afterglow lies off the centre of an elliptical galaxy at a redshift of z = 0.258 (ref. 5), coincident with the position determined by ground-based optical and radio observations. The low level of star formation typical for elliptical galaxies makes it unlikely that the burst originated in a supernova explosion. A supernova origin was also ruled out for GRB 050709 (refs 3, 31), even though that burst took place in a galaxy with current star formation. The isotropic energy for the short bursts is 2-3 orders of magnitude lower than that for the long bursts. Our results therefore suggest that an alternative source of bursts-the coalescence of binary systems of neutron stars or a neutron star-black hole pair-are the progenitors of short bursts.

Telomeres, the repeated sequences that cap chromosome ends, undergo shortening with each cell division, and therefore serve as markers of a cell’s replicative history. In vivo, clonal expansion of T ...cells during immune responses to both foreign and autoantigens is associated with telomere shortening. To investigate possible immune alterations in Alzheimer’s disease (AD) that might impact current vaccine-based therapeutic strategies, we analyzed telomere lengths in immune cell populations from AD patients. Our data show a significant telomere shortening in PBMC from AD versus controls (
P=0.04). Importantly, telomere length of T cells, but not of B cells or monocytes, correlated with AD disease status, measured by Mini Mental Status Exam (MMSE) scores (
P=0.025). T cell telomere length also inversely correlated with serum levels of the proinflammatory cytokine TNFα (a clinical marker of disease status), with the proportion of CD8+ T cells lacking expression of the CD28 costimulatory molecule, and with apoptosis. These findings suggest an immune involvement in AD pathogenesis.
The Paisley Caves in Oregon record the oldest directly dated human remains (DNA) in the Western Hemisphere. More than 100 high-precision radiocarbon dates show that deposits containing artifacts and ...coprolites ranging in age from 12,450 to 2295 ¹⁴C years ago are well stratified. Western Stemmed projectile points were recovered in deposits dated to 11,070 to 11,340 ¹⁴C years ago, a time contemporaneous with or preceding the Clovis technology. There is no evidence of diagnostic Clovis technology at the site. These two distinct technologies were parallel developments, not the product of a unilinear technological evolution. "Blind testing" analysis of coprolites by an independent laboratory confirms the presence of human DNA in specimens of pre-Clovis age. The colonization of the Americas involved multiple technologically divergent, and possibly genetically divergent, founding groups.
To improve the efficacy of Plasmodium falciparum malaria vaccine RTS,S/AS02, we conducted a study in 2001 in healthy, malaria-naïve adults administered RTS,S/AS02 in combination with FMP1, a ...recombinant merozoite surface-protein-1, C-terminal 42kD fragment.
A double-blind Phase I/IIa study randomized N = 60 subjects 1:1:1:1 to one of four groups, N = 15/group, to evaluate safety, immunogenicity, and efficacy of intra-deltoid half-doses of RTS,S/AS02 and FMP1/AS02 administered in the contralateral (RTS,S + FMP1-separate) or same (RTS,S + FMP1-same) sites, or FMP1/AS02 alone (FMP1-alone), or RTS,S/AS02 alone (RTS,S-alone) on a 0-, 1-, 3-month schedule. Subjects receiving three doses of vaccine and non-immunized controls (N = 11) were infected with homologous P. falciparum 3D7 sporozoites by Controlled Human Malaria Infection (CHMI).
Subjects in all vaccination groups experienced mostly mild or moderate local and general adverse events that resolved within eight days. Anti-circumsporozoite antibody levels were lower when FMP1 and RTS,S were co-administered at the same site (35.0 µg/mL: 95 % CI 20.3–63), versus separate arms (57.4 µg/mL: 95 % CI 32.3–102) or RTS,S alone (62.0 µg/mL: 95 % CI: 37.8–101.8). RTS,S-specific lymphoproliferative responses and ex vivo ELISpot CSP-specific interferon-gamma (IFN-γ) responses were indistinguishable among groups receiving RTS,S/AS02. There was no difference in antibody to FMP1 among groups receiving FMP1/AS02. After CHMI, groups immunized with a RTS,S-containing regimen had ∼ 30 % sterile protection against parasitemia, and equivalent delays in time-to-parasitemia. The FMP1/AS02 alone group showed no sterile immunity or delay in parasitemia.
Co-administration of RTS,S and FMP1/AS02 reduced anti-RTS,S antibody, but did not affect tolerability, cellular immunity, or efficacy in a stringent CHMI model. Absence of efficacy or delay of patency in the sporozoite challenge model in the FMP1/AS02 group did not rule out efficacy of FMP1/AS02 in an endemic population. However, a Phase IIb trial of FMP1/AS02 in children in malaria-endemic Kenya did not demonstrate efficacy against natural infection. ClinicalTrials.gov identifier: NCT01556945.
Introduction The 2009 Family Smoking Prevention and Tobacco Control Act banned characterizing flavors other than menthol in cigarettes but did not restrict their use in other forms of tobacco (e.g., ...smokeless, cigars, hookah, e-cigarettes). Methods A cross-sectional analysis of Wave 1 data from 45,971 U.S. adults and youth, aged ≥12 years in the Population Assessment of Tobacco and Health (PATH) Study collected in 2013–2014, was conducted in 2016. This study examined (1) the prevalence and reasons for use of flavored tobacco products; (2) the proportion of ever tobacco users reporting that their first product was flavored; and (3) correlates of current flavored tobacco product use. Results Current flavored (including menthol) tobacco product use was highest in youth (80%, aged 12–17 years); and young adult tobacco users (73%, aged 18–24 years); and lowest in older adult tobacco users aged ≥65 years (29%). Flavor was a primary reason for using a given tobacco product, particularly among youth. Eighty-one percent of youth and 86% of young adult ever tobacco users reported that their first product was flavored versus 54% of adults aged ≥25 years. In multivariable models, reporting that one’s first tobacco product was flavored was associated with a 13% higher prevalence of current tobacco use among youth ever tobacco users and a 32% higher prevalence of current tobacco use among adult ever users. Conclusions These results add to the evidence base that flavored tobacco products may attract young users and serve as starter products to regular tobacco use.
