Background
Positional obstructive sleep apnoea (POSA), defined as a supine apnoea–hypopnoea index (AHI) twice or more as compared to the AHI in the other positions, occurs in 56 % of obstructive ...sleep apnoea patients. Positional therapy (PT) is one of several available treatment options for these patients. So far, PT has been hampered by compliance problems, mainly because of the usage of bulky masses placed in the back. In this article, we present a novel device for treating POSA patients.
Methods
Patients older than 18 years with mild to moderate POSA slept with the Sleep Position Trainer (SPT), strapped to the chest, for a period of 29 ± 2 nights. SPT measures the body position and vibrates when the patient lies in supine position.
Results
Thirty-six patients were included; 31 patients (mean age, 48.1 ± 11.0 years; mean body mass index, 27.0 ± 3.7 kg/m
2
) completed the study protocol. The median percentage of supine sleeping time decreased from 49.9 % 20.4–77.3 % to 0.0 % range, 0.0–48.7 % (
p
< 0.001). The median AHI decreased from 16.4 6.6–29.9 to 5.2 0.5–46.5 (
p
< 0.001). Fifteen patients developed an overall AHI below five. Sleep efficiency did not change significantly. Epworth Sleepiness Scale decreased significantly. Functional Outcomes of Sleep Questionnaire increased significantly. Compliance was found to be 92.7 % 62.0–100.0 %.
Conclusions
The Sleep Position Trainer applied for 1 month is a highly successful and well-tolerated treatment for POSA patients, which diminishes subjective sleepiness and improves sleep-related quality of life without negatively affecting sleep efficiency. Further research, especially on long-term effectiveness, is ongoing.
ABSTRACT
The Mid-Infrared Instrument (MIRI) on the James Webb Space Telescope (JWST), has imaging, four coronagraphs, and both low and medium resolution spectroscopic modes. Being able to simulate ...MIRI observations will help commissioning of the instrument, as well as get users familiar with representative data. We designed the MIRI instrument simulator (mirisim) to mimic the on-orbit performance of the MIRI imager and spectrometers using the Calibration Data Products (CDPs) developed by the MIRI instrument team. The software incorporates accurate representations of the detectors, slicers, distortions, and noise sources along the light path including the telescope’s radiative background and cosmic rays. The software also includes a module that enables users to create astronomical scenes to simulate. mirisim is a publicly available python package that can be run at the command line, or from within python. The outputs of mirisim are detector images in the same uncalibrated data format that will be delivered to MIRI users. These contain the necessary metadata for ingestion by the JWST calibration pipeline.
Context. Fomalhaut is a young (2 +/- 1 x 10(8) years), nearby (7.7 pc), 2 M-circle dot star that is suspected to harbor an infant planetary system, interspersed with one or more belts of dusty ...debris. Aims. We present far-infrared images obtained with the Herschel Space Observatory with an angular resolution between 5.7 '' and 36.7 '' at wavelengths between 70 mu m and 500 mu m. The images show the main debris belt in great detail. Even at high spatial resolution, the belt appears smooth. The region in between the belt and the central star is not devoid of material; thermal emission is observed here as well. Also at the location of the star, excess emission is detected. We aim to construct a consistent image of the Fomalhaut system. Methods. We use a dynamical model together with radiative-transfer tools to derive the parameters of the debris disk. We include detailed models of the interaction of the dust grains with radiation, for both the radiation pressure and the temperature determination. Comparing these models to the spatially resolved temperature information contained in the images allows us to place strong constraints on the presence of grains that will be blown out of the system by radiation pressure. We use this to derive the dynamical parameters of the system. Results. The appearance of the belt points toward a remarkably active system in which dust grains are produced at a very high rate by a collisional cascade in a narrow region filled with dynamically excited planetesimals. Dust particles with sizes below the blow-out size are abundantly present. The equivalent of 2000 one-km-sized comets are destroyed every day, out of a cometary reservoir amounting to 110 Earth masses. From comparison of their scattering and thermal properties, we find evidence that the dust grains are fluffy aggregates, which indicates a cometary origin. The excess emission at the location of the star may be produced by hot dust with a range of temperatures, but may also be due to gaseous free-free emission from a stellar wind.
Context. The influence of binarity on the late stages of stellar evolution remains an open issue. Aims. While the first binary post-AGB stars were serendipitously discovered, the distinct ...characteristics of their spectral energy distribution (SED) allowed us to launch a more systematic search for binaries. We selected post-AGB objects, which exhibit a broad dust excess starting either at H or K, pointing to the presence of a gravitationally bound dusty disc in the system. We initiated an extensive multiwavelength study of those systems and here report on our radial velocity and photometric monitoring results for six stars of early F type, which are pulsators of small amplitude. Methods. To determine the radial velocity of low signal-to-noise ratio time-series data, we constructed dedicated autocorrelation masks based on high signal-to-noise ratio spectra, used in our published chemical studies. The radial velocity variations were analysed in detail to differentiate between pulsational variability and variability caused by orbital motion. When available, the photometric monitoring data were used to complement the time series of radial velocity data and to establish the nature of the pulsation. Finally, orbital minimalisation was performed to constrain the orbital elements. Results. All of the six objects are binaries with orbital periods ranging from 120 to 1800 days. Five systems have non-circular orbits. The mass functions range from 0.004 to 0.57 $M_{\odot}$ and the companions are probably unevolved objects of (very) low initial mass. We argue that these binaries must have evolved through a phase of strong binary interaction when the primary was a cool supergiant. Although the origin of the circumstellar disc is not well understood, the disc is generally believed to have formed during this strong interaction phase. The eccentric orbits of these highly evolved objects remain poorly understood. In one object, the line-of-sight grazes the edge of the puffed-up inner rim of the disc. Conclusions. These results corroborate our earlier statement that evolved objects in binary stars create a Keplerian dusty circumbinary disc. With the measured orbits and mass functions, we conclude that the circumbinary discs seem to have a major impact on the evolution of a significant fraction of binary systems.

We present five band imaging of the Vega debris disc obtained using the Herschel Space Observatory. These data span a wavelength range of 70–500 μm with full-width half-maximum angular resolutions of ...5.6–36.9”. The disc is well resolved in all bands, with the ring structure visible at 70 and 160 μm. Radial profiles of the disc surface brightness are produced, and a disc radius of 11” (~85 AU) is determined. The disc is seen to have a smooth structure thoughout the entire wavelength range, suggesting that the disc is in a steady state, rather than being an ephemeral structure caused by the recent collision of two large planetesimals.
Detached circumstellar dust shells are detected around three carbon variables using Herschel-PACS. Two of them are already known on the basis of their thermal CO emission and two are visible as ...extensions in IRAS imaging data. By model fits to the new data sets, physical sizes, expansion timescales, dust temperatures, and more are deduced. A comparison with existing molecular CO material shows a high degree of correlation for TT Cyg and U Ant but a few distinct differences with other observables are also found.

Herschel PACS and SPIRE images have been obtained over a 30' × 30' area around the well-known carbon star CW Leo (IRC +10 216). An extended structure is found in an incomplete arc of ~22' diameter, ...which is cospatial with the termination shock due to interaction with the interstellar medium (ISM) as defined by Sahai & Chronopoulos from ultraviolet GALEX images. Fluxes are derived in the 70, 160, 250, 350, and 550 μm bands in the region where the interaction with the ISM takes place, and this can be fitted with a modified black body with a temperature of 25 ± 3 K. Using the published proper motion and radial velocity for the star, we derive a heliocentric space motion of 25.1 km s-1. Using the PACS and SPIRE data and the analytical formula of the bow shock structure, we infer a de-projected standoff distance of the bow shock of R0 = (8.0 ± 0.3) × 1017 cm. We also derive a relative velocity of the star with respect to the ISM of (106.6 ± 8.7)/$\sqrt{n_{\rm ISM}}$ km s-1, where nISM is the number density of the local ISM.
Herschel PACS and SPIRE images have been obtained of NGC 6720 (the Ring nebula). This is an evolved planetary nebula with a central star that is currently on the cooling track, due to which the outer ...parts of the nebula are recombining. From the PACS and SPIRE images we conclude that there is a striking resemblance between the dust distribution and the H2 emission, which appears to be observational evidence that H2 forms on grain surfaces. We have developed a photoionization model of the nebula with the Cloudy code which we used to determine the physical conditions of the dust and investigate possible formation scenarios for the H2. We conclude that the most plausible scenario is that the H2 resides in high density knots which were formed after the recombination of the gas started when the central star entered the cooling track. Hydrodynamical instabilities due to the unusually low temperature of the recombining gas are proposed as a mechanism for forming the knots. H2 formation in the knots is expected to be substantial after the central star underwent a strong drop in luminosity about one to two thousand years ago, and may still be ongoing at this moment, depending on the density of the knots and the properties of the grains in the knots.
We obtained Herschel PACS and SPIRE images of the thermal emission of the debris disk around the A5V star β Pic. The disk is well resolved in the PACS filters at 70, 100, and 160 μm. The surface ...brightness profiles between 70 and 160 μm show no significant asymmetries along the disk, and are compatible with 90% of the emission between 70 and 160 μm originating in a region closer than 200 AU to the star. Although only marginally resolving the debris disk, the maps obtained in the SPIRE 250–500 μm filters provide full-disk photometry, completing the SED over a few octaves in wavelength that had been previously inaccessible. The small far-infrared spectral index (β = 0.34) indicates that the grain size distribution in the inner disk (<200 AU) is inconsistent with a local collisional equilibrium. The size distribution is either modified by non-equilibrium effects, or exhibits a wavy pattern, caused by an under-abundance of impactors which have been removed by radiation pressure.
