Depuis l’autorisation du riluzole en 1995, aucun nouveau traitement n’a pas pu être autorisé. Néanmoins l’organisation de la prise en charge multidisciplinaire, autours des centres experts, et des ...réseaux de santé, a permis en parallèle d’améliorer la survie des patients et leur qualité de vie. L’efficacité neuroprotectrice de riluzole a été démontrée sur un critère solide celui de la survie, mais ses mécanismes d’action demeurent inconnus, même si le point de départ était ses qualités anti-glutamates.
Depuis, d’autres molécules, avec d’autres mécanismes d’action ont pu démontré certaines efficacité, Quand à la survie, elle a été abandonnée comme critère principal d’évaluation, au détriment d’autres critères, moins durs, comme l’ALS FRS, qui est une échelle fonctionnelle.
En septembre 2022 la FDA a approuvé Relyvrio® suite à une étude phase II, après avoir rendu un avis négatif en mars 2022. Ce produit est une combinaison de composés génériques, le phénylbutyrate de sodium et le taurursodiol. L’étude d’efficacité en double aveugle a porté sur 9 mois. Elle a montré un effet significatif sur l’ALSFRS. Mais, une analyse post-hoc sur la phase ouverte qui a duré 18 mois a conclu à une amélioration de la survie.
La décision de la FDA divise la communauté scientifique, de nombreux spécialistes jugeant celle-ci prématurée en l’absence de données de phase 3, mais suscite un nouvel espoir chez les patients.
En mai 2022, la FDA a également approuvé la forme orale de l’edaravone, un autre produit dont la forme IV est autorisée aux États-Unis depuis 4 ans, mais pas en Europe, en raison d’une efficacité jugée trop faible, et d’une absence d’effet dans les études sur la survie, En France, la forme injectable est disponible en ATU mais très peu utilisée.
Enfin on peut citer le Tofersen, un oligonucléotide antisens (ASO) de 2e génération, qui a été développé dans la SLA génétique avec mutation du gène SOD1.
D’autres molécules sont également en développement à des stades plus au moins avancés, et la vieille idée de « cocktail » de produits neuroprotecteurs dans les maladies neurodégénératives, et plus particulièrement dans la SLA, retrouve de nouveau sa place, soutenue par les instances consultatives de la FDA, sans avoir même le besoin de réaliser des études d’associations pharmacologiques !! Affaire à suivre.
La thérapie génique a été initialement conçue comme une approche thérapeutique destinée aux maladies monogéniques, délivrant aux cellules un gène « sain » capable de suppléer le gène « malade ». ...Aujourd’hui, les modalités et les indications se révèlent beaucoup plus larges, les approches se sont beaucoup diversifiées, reposant sur différentes stratégies correctives, vecteurs et modalités de thérapies géniques. Des progrès ont été obtenus dans l’adrénoleucodystrophie, les cellules souches sanguines des patients sont corrigées ex vivo à l’aide d’un lentivirus, puis réinjectées. Avec un passage réussi chez l’homme. On souligne également les résultats prometteurs, avec une approche similaire, chez des enfants atteints de leucodystrophie métachromatique et d’autres travaux dans la maladie de Sanfilippo. Ils visent à obtenir la production de l’enzyme manquante par les cellules cérébrales. Un vecteur de thérapie génique de type AAV est pour cela injecté dans différentes zones du cerveau des patients, avec une bonne tolérance et une amélioration du développement intellectuel et comportemental. Les approches de thérapie génique s’appliquent également à d’autres maladies plus fréquentes telles que la maladie de Parkinson et d’Alzheimer. Pour ces maladies, qui ne sont pas d’origine génétique, les mécanismes sont diversifiés : correction des modifications biochimiques responsables des symptômes par une stratégie de type « remplacement ou restauration de fonction », comme augmenter la synthèse de dopamine ou l’inhibition de la production des protéines anormales pathogènes dans la maladie de Parkinson, ou restaurer l’équilibre du cholestérol dans le cerveau. Ces mécanismes permettraient de prévenir des dysfonctions neuronales causés par la maladie d’Huntington et la maladie d’Alzheimer. Les mutations dans les formes familiales est une autre cible, c’est le cas des mutations SOD et C9orf72 dans la SLA. Enfin, les indications des cellules souches pour la « réparation » des séquelles des maladies dégénératives non génétiques, est un besoin important, dont les techniques nécessitent encore d’évoluer.
Objective
We conducted a randomized double‐blind trial of riluzole in Huntington's disease to investigate the efficacy of this antiexcitotoxic drug in slowing disease progression.
Methods
The study ...included 537 adult patients with a clinical diagnosis of Huntington's disease confirmed by genotyping. Patients were randomized (2:1) to treatment with riluzole (50mg twice daily) or placebo for 3 years. Concomitant use of antichoreic medication was forbidden, and introduction of such medication was a predefined end point. The primary outcome measure was change in a combined score derived from the motor and total functional capacity subscores of the Unified Huntington's Disease Rating Scale. Safety was also evaluated.
Results
A total of 379 patients completed the study (mean age, 47 standard deviation, 9.5 years; 50% female patients). The principal reason for discontinuation was introduction of antichoreic medication. The median change from baseline in the combined score (primary outcome) for the “per protocol” population was 13.7 (95% confidence interval, 11.1–17.2) in the placebo group and 14.3 (95% confidence interval, 11.7–16.6) in the riluzole group. No intergroup difference in outcome could thus be demonstrated (p = 0.93, Mann–Whitney U test). No differences in secondary efficacy outcome variables were observed except for more frequent recourse to antichoreic medication in the placebo group. No unexpected adverse events were reported, and tolerability was acceptable.
Interpretation
No neuroprotective or beneficial symptomatic effects of riluzole in Huntington's disease were demonstrated. Ann Neurol 2007
Advances in our understanding of the pathophysiology of migraine have resulted in important breakthroughs in treatment. For example, understanding of the role of serotonin in the cerebrovascular ...circulation has led to the development of triptans for the acute relief of migraine headaches, and the identification of cortical spreading depression as an early central event associated wih migraine has brought renewed interest in antiepileptic drugs for migraine prophylaxis. However, migraine still remains inadequately treated. Indeed, it is apparent that migraine is not a single disease but rather a syndrome that can manifest itself in a variety of pathological conditions. The consequences of this may be that treatment needs to be matched to particular patients. Clinical research needs to be devoted to identifying which sort of patients benefit best from which treatments, particularly in the field of prophylaxis. We propose four patterns of precipitating factors (adrenergic, serotoninergic, menstrual, and muscular) which may be used to structure migraine prophylaxis. Finally, little is known about long-term outcome in treated migraine. It is possible that appropriate early prophylaxis may modify the long-term course of the disease and avoid late complications.
Elderly subjects diagnosed with mild cognitive impairment (MCI) are becoming the target of intervention trials. The criteria used for MCI are principally issued from prospective clinical studies, ...although longitudinal population-based studies having identified several cognitive predictors of dementia can be of great contribution in the definition of these criteria. This study was conducted to explore the external validity of MCI criteria issued from a longitudinal population-based study, and subsequently to identify the best predictors of the short-term conversion to Alzheimer’s disease 2 years after the MCI diagnosis. Ninety elderly volunteers with memory complaint diagnosed with MCI on the basis of their functional and neuropsychological performances were followed up within 2 years. The potential predictors of the conversion to dementia collected at baseline included age, gender, educational level, size of temporal lobe, apolipoprotein E genotype and a series of neuropsychological measures (Mac Nair Scale, Mini-Mental State Examination, Benton Visual Retention Test, Isaacs Set Test, Digit Symbol Substitution Task, Letter Cancellation Task, digit span tasks and finger-tapping test). Within the 2 years, 29 subjects (32.2%) presented a conversion to dementia. The risk of conversion to dementia was associated with age and size of temporal lobe but not with gender, education, or apolipoprotein E4 genotype. Several neuropsychological measures were associated with the risk of conversion to dementia, but in a logistic regression performed with the significant variables found in the univariate analysis, only the Letter Cancellation Test was shown to be an independent predictor. In conclusion, the quite elevated conversion rates obtained show the usefulness, when defining MCI criteria, of considering not only memory impairment but also impairment in other cognitive areas, as well as mild impairment on higher-order activities of daily living. Among the variables considered, the Letter Cancellation Test proved to be a major predictor of short-term conversion to dementia.
De nombreux travaux expérimentaux, animaux et cliniques, confirment l’intérêt de la stimulation cognitive et motrice dans le traitement des maladies neurodégénératives.
Ces données évoquent un rôle ...bénéfique dans la récupération après un accident vasculaire ischémique, dans la sclérose en plaques (SEP), la maladie de Parkinson, celle d’Alzheimer, et même dans la sclérose latérale amyotrophique (SLA).
Les mécanismes de cette stimulation ne sont pas complètement élucidés, on évoque un effet modulateur des neurofilaments qui intervient dans la régulation énergétique de la cellule neuronale, mais également des phénomènes de remyélinisation, d’où la neuroprotection suggérée.
Ces observations vont à l’encontre des idées qui ont dominé la pratique neurologique dans les années1980 et 1990, et qui évoquent une aggravation de l’atteinte neuronale dégénérative par les efforts physiques. Mais très probablement il existe une forme de fenêtre thérapeutique, où une dose mesurée d’activité génère un effet bénéfique, cet effet se perdra une fois un seuil de cette activité est dépassé, allant même jusqu’à la toxicité et l’aggravation de la perte neuronale.

Fifteen years ago, a role for excitotoxic damage in the pathology of amyotrophic lateral sclerosis (ALS) was postulated. This stimulated the development of riluzole, the only available treatment for ...the disease. Since then, the identification of abnormal forms of superoxide dismutase as the genetic basis of certain familial forms of ALS has provided a huge impetus to the search for new effective treatments for this devastating disease. Transgenic mouse models have been developed expressing these aberrant mutants that develop a form of motor neurone disease the progress of which can be slowed by riluzole. Studies in these mice have provided evidence for a role for excitotoxic, apoptotic and oxidative processes in the development of pathology. The mice can be used for testing molecules targeting these processes as potential therapies, to allow the most promising to be evaluated in humans. Several such agents are currently in clinical trials. Many previous clinical trials in ALS were insufficiently powered to demonstrate any relevant effect on disease progression. This situation has been to some extent remedied in the more recent trials, which have recruited many hundreds of patients. However, with the exception of studies with riluzole, the results of these have been disappointing. In particular, a number of large trials with neurotrophic agents have revealed no evidence for efficacy. Nonetheless, the need for large multinational trials of long duration limits the number that can be carried out and makes important demands on investment. For this reason, surrogate markers that can be used for rapid screening in patients of potential treatments identified in the transgenic mice are urgently needed.

5-HT(2A) receptor antagonism seems to explain the low incidence of extrapyramidal side effects with atypical neuroleptics. Whether the neuroleptic cyamemazine, which at low doses is also devoid of ...extrapyramidal side effects, possesses 5-HT(2A) receptor antagonist properties is unknown. Cyamemazine was tested for its ability to antagonize 5-HT(2A)-mediated responses in isolated rat aorta and guinea pig trachea and to displace 3Hketanserin specifically bound to rat brain membranes. In isolated rat aorta, cyamemazine potently and competitively antagonized serotonin-dependent contractions (pA(2)=8.82+/-0.26, n=7; Schild's slope=1.02+/-0.29). In this test, cyamemazine was of similar potency as ketanserin (pA(2)=8.23). In isolated guinea pig trachea, cyamemazine reduced maximum contractile responses to serotonin with pIC(50)=7.92+/-0.35, (n=4), whereas ketanserin exhibited a pIC(50)=8.79. Finally, cyamemazine displaced 3Hketanserin specifically bound to rat brain membranes with pK(i)=8.76+/-0.53 (n=3). In conclusion, cyamemazine behaves as a potent antagonist at 5-HT(2A) receptors, which compares well with the reference compound, ketanserin. Whether this 5-HT(2A) receptor antagonist action of cyamemazine can explain its low incidence of extrapyramidal side effects deserves further investigation.
