Congenital indifference to pain (CIP) is a rare condition in which patients have severely impaired pain perception, but are otherwise essentially normal. We identified and collected DNA from ...individuals from nine families of seven different nationalities in which the affected individuals meet the diagnostic criteria for CIP. Using homozygosity mapping and haplotype sharing methods, we narrowed the CIP locus to chromosome 2q24–q31, a region known to contain a cluster of voltage‐gated sodium channel genes. From these prioritized candidate sodium channels, we identified 10 mutations in the SCN9A gene encoding the sodium channel protein Nav1.7. The mutations completely co‐segregated with the disease phenotype, and nine of these SCN9A mutations resulted in truncation and loss‐of‐function of the Nav1.7 channel. These genetic data further support the evidence that Nav1.7 plays an essential role in mediating pain in humans, and that SCN9A mutations identified in multiple different populations underlie CIP.
Life history theory (LHT) predicts that individuals vary in their sexual, reproductive, parental, familial, and social behavior according to the physical and social challenges imposed upon them ...throughout development. LHT provides a framework for understanding why non-monogamy may be the target of significant moral condemnation: individuals who habitually form multiple romantic or sexual partnerships may pursue riskier, more competitive interpersonal strategies that strain social cooperation. We compared several indices of life history (i.e., the Mini-K, the High-K Strategy Scale, pubertal timing, sociosexuality, disease avoidance, and risk-taking) between individuals practicing monogamous and consensually non-monogamous (CNM) romantic relationships. Across several measures, CNM individuals reported a faster life history strategy than monogamous individuals, and women in CNM relationships reported earlier pubertal development. CNM individuals also reported more social and ethical risk-taking, less aversion to germs, and greater interest in short-term mating (and less interest in long-term mating) than monogamous individuals. From these data, we discuss a model to explain how moral stigma toward non-monogamy evolved and how these attitudes may be mismatched to the modern environment. Specifically, we argue that the culture of sexual ethics that pervades contemporary CNM communities (e.g., polyamory, swinging) may attenuate risky interpersonal behaviors (e.g., violent intrasexual competition, retributive jealousy, partner/child abandonment, disease transmission) that are relatively more common among those who pursue multi-partner mating.
Summary Hand hygiene is considered to be the most effective measure to prevent microbial pathogen cross-transmission and healthcare-associated infections. In October 2005, the World Health ...Organization (WHO) World Alliance for Patient Safety launched the first Global Patient Safety Challenge 2005–2006, ‘Clean Care is Safer Care’, to tackle healthcare-associated infection on a large scale. Within the Challenge framework, international infection control experts and consultative taskforces met to develop new WHO Guidelines on Hand Hygiene in Healthcare. The taskforce was asked to explore aspects underlying hand hygiene behaviour that may influence its promotion among healthcare workers. The dynamics of behavioural change are complex and multi-faceted, but are of vital importance when designing a strategy to improve hand hygiene compliance. A reflection on challenges to be met and areas for future research are also proposed.

To analyze, in a pilot study, rapidly acquired dynamic contrast-enhanced (DCE)-MRI data with a general two-compartment exchange tracer kinetic model and correlate parameters obtained with ...measurements of hypoxia and vascular endothelial growth factor (VEGF) expression in patients with squamous cell carcinoma of the head and neck.
Eight patients were scanned before surgery. The DCE-MRI data were acquired with 1.5-s temporal resolution and analyzed using the two-compartment exchange tracer kinetic model to obtain estimates of parameters including perfusion and permeability surface area. Twelve to 16 h before surgery, patients received an intravenous injection of pimonidazole. Samples taken during surgery were used to determine the level of pimonidazole staining using immunohistochemistry and VEGF expression using quantitative real-time polymerase chain reaction. Correlations between the biological and imaging data were examined.
Of the seven tumors fully analyzed, those that were poorly perfused tended to have high levels of pimonidazole staining (r = -0.79, p = 0.03) and VEGF expression (r = -0.82, p = 0.02). Tumors with low permeability surface area also tended to have high levels of hypoxia (r = -0.75, p = 0.05). Hypoxic tumors also expressed higher levels of VEGF (r = 0.82, p = 0.02).
Estimates of perfusion obtained with rapid DCE-MRI data in patients with head-and-neck cancer correlate inversely with pimonidazole staining and VEGF expression.
Accurate nuclear data is a key factor in determining the suitability and reliability of many theoretical nuclear models and large-scale calculations. One of the main ingredients of these calculations ...is how nuclei respond to an electromagnetic field. The excitation of the isovector giant dipole resonance (GDR) is of particular importance in both nuclear structure studies as well as being the main mode of interaction of ultra-high-energy cosmic rays with the extra-galactic medium. This study investigates the photoabsorption cross section in the region of the GDR in 24Mg through the use of proton inelastic scattering and the equivalent virtual photon method. The K600 spectrometer at the iThemba LABS facility was used to obtain high resolution, low background 24Mg(p,p’)24Mg* inelastic scattering data. The virtual photon absorption method is described and the result of applying the method is presented and compared to a previous real photon absorption cross section.
Purinergic signalling regulates airway defence mechanisms, suggesting that extracellular purines could serve as airway inflammation biomarkers in cystic fibrosis (CF). The purines adenosine ...triphosphate (ATP), adenosine diphosphate (ADP), adenosine monophosphate (AMP) and adenosine were measured in sputum from 21 adults (spontaneously expectorated from seven CF patients, induced from 14 healthy controls) to assess normal values and CF-associated changes. Subsequently, purine levels were measured in bronchoalveolar lavage fluid (BALF) from 37 children (25 CF patients, 12 disease controls) and compared with neutrophil counts, presence of airway infection and lung function. To noninvasively assess airway purines, ATP levels were measured using luminometry in exhaled breath condensate (EBC) from 14 children with CF and 14 healthy controls, then 14 CF children during a pulmonary exacerbation. Both ATP and AMP were elevated in sputum and BALF from CF subjects compared with controls. In BALF, ATP and AMP levels were inversely related to lung function and strongly correlated with neutrophil counts. In EBC, ATP levels were increased in CF relative to controls and decreased after treatment of CF pulmonary exacerbation. The purines adenosine triphosphate and adenosine monophosphate are candidate biomarkers of neutrophilic airways inflammation. Measurement of purines in sputum or exhaled breath condensate may provide a relatively simple and noninvasive method to track this inflammation.
Affirmative therapy is a type of psychotherapy used to validate and advocate for the needs of sexual and gender minority clients. Therapists use verbal and nonverbal means to demonstrate an affirming ...stance toward lesbian, gay, bisexual, and transgender (LGBT) clients. Although this therapeutic approach can be used with LGBT individuals across the lifespan, the case presented in this paper highlights the benefit of using this approach with older LGBT individuals. Discussion of the unique challenges faced by older LGBT people will be followed with an illustrative case that shows ways that affirmative therapy can help individuals achieve greater self‐actualization. The case formulation and conclusion will highlight specific ways that therapists can convey an affirmative stance and help clients feel welcome and accepted in the therapeutic setting. Recommendations for environmental cues, intake questions, and treatment skills will be offered at the conclusion.
