The aim of this exploratory study was to examine how the working conditions and job stress of private practice oncologists (PPOs) affect their interaction with patients. Data for the study were ...collected through semi‐structured interviews with PPOs selected based on purposeful sampling criteria. The data were evaluated using content analysis. Factors perceived by PPOs to influence their level of stress and patient care were some of their own personal characteristics as well as working conditions both within and outside their practices. Apart from being able to name specific stressors (e.g. dysfunctional organisational workflow and interruptions during medical encounters), the oncologists also mentioned individual and organisational resources (e.g. professional experience, well‐educated nurses and good work organisation) for coping with job stress and improving interaction with patients. Within this study, we identified some obstacles on the individual and organisational level for good patient care, as working conditions which might lead to time pressure and stress, which subsequently have an impact on quality in patient care (e.g. less time for personal issues during patient consultations). Future stress research should conduct a more in‐depth investigation of these and other interventions at both the individual and organisational levels in order to improve patient care.
While a lot is known about potential and actual turnover of non-medical hospital staff, only few data exist for the outpatient setting. In addition, little is known about actual instruments which ...leaders can use to influence staff turnover in physician practices. In the literature, the social capital of an organisation, which means the amount of trust, common values and reciprocal behaviour in the organisation, has been discussed as a possible field of action. In the present study, staff turnover as perceived by outpatient haematologists and oncologists is presented and analysed as to whether social capital is associated with that staff turnover. In conclusion, measures to increase the social capital of a practice are presented.
The present study is based on data gathered in a questionnaire-based survey with members of the Professional Organisation of -Office-Based Haematologists and Oncologists (N=551). The social capital of the practice was captured from the haematologists and oncologists using an existing and validated scale. To analyse the impact of the practice's social capital on staff turnover, as perceived by the physicians, bivariate correlations and linear regression analyses were calculated.
In total, 152 haematologists and oncologists participated in the study which represents a response rate of 28%. In the regression analyses, social capital appears as a significant and strong predictor of staff turnover (beta=-0.34; p<0.001).
Building social capital within the practice may be an important contribution to reducing staff turnover although the underlying study design does not allow for drawing causal conclusions regarding this relationship. To create social capital in their practice, outpatient physicians may apply measures that facilitate social interaction among staff, foster trust and facilitate cooperation. Such measures may already be applied when hiring and training new staff, but also continuously when leading employees and when organising work tasks, e.g., by establishing regular team meetings.
Zusammenfassung
Für eine sichere und erfolgreiche Therapie mit oralen Tumormedikamenten benötigen Patient*innen eine ausreichende Arzneimittelkompetenz. Die Betroffenen müssen in der Lage sein ...Arzneimittelinformationen zu verstehen und korrekt anzuwenden. Dies betrifft auch den richtigen Umgang mit zu erwartenden Nebenwirkungen. Im Zuge der Entwicklung eines bisher nicht existierenden Fragebogens zur Messung der Arzneimittelkompetenz bei Patient*innen mit oraler Tumortherapie wurde unter anderem ein Fokusgruppeninterview mit Expert*innen aus Onkologie, Pflege, Pharmazie sowie mit Patientenvertreter*innen durchgeführt. Notwendige Kompetenzen und Fähigkeiten sowie zentrale Herausforderungen und Schwierigkeiten bei der Therapie in der Häuslichkeit der Betroffenen wurden identifiziert. Die Ergebnisse dienen der Ergänzung und Weiterentwicklung des Fragebogens, welcher als patientenrelevanter Endpunkt für zukünftige Forschungsprojekte sowie zur Identifizierung von Patient*innen mit erhöhtem Beratungs- und Informationsbedarf dienen kann.
Currently, elements of managed care are being implemented in the German health-care system. The legal basis for these innovations are § 140, § 73, § 137, and §§ 63 et seq. of the German Social Code - ...Part 5 (SGB V). For the model projects according to §§ 63 et seq. of the German Social Code a scientific evaluation and publication of the evaluation results is mandatory. The present study examines the status of evaluation of German model projects.
The present study has a mixed method design: A mail and telephone survey with the German Federal Social Insurance Authority, the health insurance funds, and the regional Associations of Statutory Health Insurance Physicians has been conducted. Furthermore, an internet research on "Medpilot" and "Google" has been accomplished to search for model projects and their evaluation reports.
34 model projects met the inclusion criteria. 13 of these projects had been terminated up to 30/9/2008. 6 of them have published an evaluation report. 4 model projects have published substantial documents. One model project in progress has published a meaningful interim report. 12 model projects failed to give information concerning the evaluator or the duration of the model projects.
The results show a significant deficit in the mandatory reporting of the evaluation of model projects in Germany. There is a need for action for the legislator and the health insurance funds in terms of promoting the evaluation and the publication of the results. The institutions evaluating the model projects should obligate themselves to publish the evaluation results. The publication is an essential precondition for the development of managed care structures in the health-care system and in the development of scientific evaluation methods.
Hintergrund:
In multidisziplinären Tumorkonferenzen (MTK) werden die Diagnose und weitere Behandlung von Krebspatienten im Behandlungsteam diskutiert und daraufhin eine Behandlungsstrategie ...entwickelt. Die Vorgängerstudie WORG OUT konnte zeigen, dass in einigen nordrheinwestfälischen Brustzentren auch Patientinnen selbst an der Besprechung ihres Falls im Rahmen der MTK teilnehmen. Die Studienlage zu Organisation, Machbarkeit sowie Risiken und Nutzen der Patiententeilnahme an der MTK ist jedoch spärlich und liefert keine gesicherten Erkenntnisse. Forschungsfragen: Forschungsfragen sind: 1.) Wie unterscheiden sich MTK mit und ohne Patiententeilnahme hinsichtlich Organisation, Interaktion und Patientenorientierung? 2.) Wie erleben Patienten die Teilnahme und welche unmittelbaren kognitiven und emotionalen Auswirkungen hat die Teilnahme für die Patientinnen? 3.) Wie bewerten die an der MTK teilnehmenden Versorger die Teilnahme von Patienten an MTK hinsichtlich Machbarkeit und Qualität der Entscheidungsfindung? Studiendesign: In einer explorativen Studie werden MTK in sechs nordrheinwestfälischen Brustzentren mit und ohne Patientinnenteilnahme mit einer Triangulation unterschiedlicher Methoden untersucht. Im qualitativen Teil werden teilnehmende Beobachtungen in MTK durchgeführt und mittels Videografie aufgezeichnet. Die Datenauswertung erfolgt zum einen durch die Deskription und Gegenüberstellung der Organisation der MTK (z.B. Dauer, Sitzordnung) und zum anderen anhand eines Schemas zur Erfassung der Qualität von MTK. Zudem werden die teilnehmenden Versorger in Fokusgruppen eingeladen, in denen die Machbarkeit der Patientinnenteilnahme sowie der Qualität der Entscheidungsfindung diskutiert werden sollen. Die Ergebnisse werden inhaltsanalytisch ausgewertet. Im quantitativen Teil werden an der MTK teilnehmende Patientinnen vor und nach der Teilnahme mit einem einseitigen Kurzfragebogen zu Angst, Therapiezuversicht und psychosozialem Informationsbedarf befragt. Es werden statistische Tests auf Unterschiede zwischen den Erhebungszeitpunkten durchgeführt. Erkenntnisgewinn: Aus den Ergebnissen können, neben Informationen zur Gestaltung der MTK, auch erste Hinweise auf Machbarkeit, Risiken und Nutzen der Patientinnenteilnahme an MTK abgeleitet werden. Auf Basis der Ergebnisse dieser explorativen Studie können dann die potentiellen Auswirkungen auf Patientenoutcomes systematisch und interventionell untersucht werden.

Bisher ist unerforscht, wie Tumorboards mit Patientinnenteilnahme ablaufen und welche Vor- und Nachteile die Teilnahme für die Patientinnen selbst und für die Versorger hat. In der Literatur werden patientenseitige Vorteile wie Partizipation an der Entscheidungsfindung und erhöhte Compliance, aber auch Nachteile wie Überforderung und Angst diskutiert, jedoch nahezu ohne empirische Grundlage. Dieser Beitrag erforscht daher die retrospektive Sicht der teilnehmenden Patientinnen.
Methodik:
Die jährliche poststationäre Patientinnenbefragung in den nordrhein-westfälischen Brustzentren wurde in 2014 um eine Definition des Tumorboards und weitere Fragen zur Information über und zur Teilnahme am Tumorboard ergänzt. Es wurden n = 4194 Patientinnen (Rücklauf 87%) mit primärem Mammakarzinom zwischen Februar und Juli 2014 befragt. Die Ergebnisse werden deskriptiv dargestellt.
Ergebnisse:
Es berichten 8,4% der Patientinnen über das Angebot der Teilnahme am Tumorboard und 4,2% (n = 165) über die tatsächliche Teilnahme. Die meisten teilnehmenden Patientinnen berichteten über eine Dauer ihrer Teilnahme von bis zu 20 Min. (70,1%) und über die Teilnahme von bis zu 4 weiteren Personen (52,6%). Dauer und Teilnehmerzahl variieren jedoch stark. Zudem konnten etwa 95% der Patientinnen ihre Meinung zur weiteren Behandlung äußern und etwa 90% wurden im Tumorboard in die Entscheidung einbezogen. Etwa 92% der Patientinnen antworteten auf die Frage, ob sie die Teilnahme bereuten, mit „überhaupt nicht“ oder „eher nicht“.
Diskussion:
Die Patientinnenteilnahme ist in den Häusern unterschiedlich organisiert. Die Einschätzung der Patientinnen bzgl. ihrer Partizipation und das seltene Bereuen der Teilnahme deuten auf eine hohe Zufriedenheit der Patientinnen hin. Weitere Forschung zur Evaluation der Patientinnenteilnahme und der Machbarkeit erscheinen notwendig. Ein hier anknüpfendes Forschungsprojekt ist bereits in Planung.

On 1 July 2009, the German Network for Health Services Research Deutsches Netzwerk Versorgungsforschung e. V. (DNVF e. V.) approved the Memorandum III "Methods for Health Services Research", ...supported by the member societies mentioned below and published in this journal (Gesundheitswesen 2009; 71: 505-510). The focus of this part of the Memorandum III "Methods for health services research" is on the questions and methods of organisational health services research. In a first step, we describe the central questions which are at the core of organisational health services research. In a second step, we describe the methodological standards and requirements with regard to a) sampling, b) measurement and c) research design. We present a phase model for complex intervention trials. This model allows to conduct high quality organisational health services research, to integrate different methods of social research and to show in which phase they are of special importance.
Abstract Objective Investigate multiple sclerosis patients’ desire to communicate with their physicians about their disease progression and end-of-life issues. Methods Persons meeting the inclusion ...criteria of feeling severely affected by MS were invited via the German MS society to complete a needs questionnaire. Replies to questions on physician empathy and wishes about communication regarding disease progression and death and dying were quantitatively analyzed. Endpoints (point 1 + 2/4 + 5) of 5-point-likert scales are summarized under results. Results 573 of 867 questionnaires meeting our criteria were analyzed. In response to a general question 64% ( n = 358) indicated a wish for disease progression and death and dying to be addressed by their doctor. A majority (76%, n = 427) considered it important that progression of their disease be discussed, while 44% ( n = 246) regard addressing death and dying as unimportant. No objective disease criteria could be identified to explain the wish for communicating end-of-life issues. Doctors who were retrospectively viewed as avoiding raising critical aspects of the illness were perceived as less empathetic ( p < 0.001). Conclusion People with MS have a desire to talk about progression of their disease with their doctors. Practice implications Physicians should be empathetic in raising critical aspects of the patients’ illness individually.
