Abstract Objective Investigate multiple sclerosis patients’ desire to communicate with their physicians about their disease progression and end-of-life issues. Methods Persons meeting the inclusion ...criteria of feeling severely affected by MS were invited via the German MS society to complete a needs questionnaire. Replies to questions on physician empathy and wishes about communication regarding disease progression and death and dying were quantitatively analyzed. Endpoints (point 1 + 2/4 + 5) of 5-point-likert scales are summarized under results. Results 573 of 867 questionnaires meeting our criteria were analyzed. In response to a general question 64% ( n = 358) indicated a wish for disease progression and death and dying to be addressed by their doctor. A majority (76%, n = 427) considered it important that progression of their disease be discussed, while 44% ( n = 246) regard addressing death and dying as unimportant. No objective disease criteria could be identified to explain the wish for communicating end-of-life issues. Doctors who were retrospectively viewed as avoiding raising critical aspects of the illness were perceived as less empathetic ( p < 0.001). Conclusion People with MS have a desire to talk about progression of their disease with their doctors. Practice implications Physicians should be empathetic in raising critical aspects of the patients’ illness individually.
Einleitung/Zielsetzung:
Die Patientinnenbefragung in den nordrhein-westfälischen Brustzentren ergab in 2013, dass Patientinnen in einigen Zentren und abhängig von Patientinnenmerkmalen zur Teilnahme ...am Tumorboard eingeladen werden (Ansmann et al. 2014). Bisher ist unerforscht, wie Tumorboards mit Patientinnenteilnahme ablaufen und welche Vor- und Nachteile die Teilnahme für die Patientinnen selbst und für die Versorger hat. Dieser Beitrag erforscht die Sicht der teilnehmenden Patientinnen.
Material & Methoden:
Die jährliche poststationäre Patientinnenbefragung in den nordrhein-westfälischen Brustzentren wurde in 2014 um eine Definition des Tumorboards und weitere Fragen zur Information über und zur Teilnahme am Tumorboard ergänzt. Es wurden n = 4194 Patientinnen (Rücklauf 87%) mit primärem Mammakarzinom zwischen Februar und Juli 2014 befragt. Die Ergebnisse werden deskriptiv dargestellt.
Ergebnisse:
Es berichten 8,4% der Patientinnen über das Angebot der Teilnahme am Tumorboard und 4,2% (n = 165) über die tatsächliche Teilnahme. Die meisten teilnehmenden Patientinnen berichteten über eine Dauer ihrer Teilnahme von bis zu 20 Min. (70,1%) und über die Teilnahme von bis zu 4 weiteren Personen (52,6%). Dauer und Teilnehmerzahl variieren jedoch stark. Zudem konnten etwa 95% der Patientinnen ihre Meinung zur weiteren Behandlung äußern und etwa 90% wurden im Tumorboard in die Entscheidung einbezogen. Etwa 92% der Patientinnen antworteten auf die Frage, ob sie die Teilnahme bereuten, mit „überhaupt nicht“ oder „eher nicht“.
Zusammenfassung:
Die Patientinnenteilnahme ist in den Häusern unterschiedlich organisiert. Die Einschätzung der Patientinnen bzgl. ihrer Partizipation und das seltene Bereuen der Teilnahme deuten auf eine hohe Zufriedenheit der Patientinnen hin. Weitere Forschung zur Evaluation der Patientinnenteilnahme und der Machbarkeit erscheinen notwendig.
Einleitung/Zielsetzung:
Die erste Zeit nach der Entlassung aus dem Brustzentrum ist für viele Patientinnen mit Unsicherheiten verbunden, beispielsweise bzgl. Informationen, Weiterversorgung und ...Alltagsbewältigung. Eine Ursache dafür ist die mangelnde Verzahnung von stationärer und ambulanter onkologischer Versorgung. Dieser Beitrag beschäftigt sich mit der Zuversicht der Patientinnen, nach der Entlassung zu Hause zurecht zu kommen und identifiziert Prozesse während des Aufenthalts im Brustzentrum, die die Zuversicht beeinflussen.
Material & Methoden:
Es werden Daten der jährlichen Patientinnenbefragung in den nordrhein-westfälischen Brustzentren zwischen Februar und Juli 2014 (n = 4194 Patientinnen aus 83 Krankenhäusern, Rücklauf 87%) ausgewertet. Patientinnen mit primärem Mammakarzinom werden poststationär mit dem Kölner Patientenfragebogen für Brustkrebs 2.0 befragt. In einem schrittweisen linearen Regressionsmodell wurden Zusammenhänge zwischen Abläufen und Informationen während des stationären Aufenthaltes und der Zuversicht der Patientinnen, zu Hause zurecht zu kommen, untersucht.
Ergebnisse:
Die Patientinnen sind insgesamt sehr zuversichtlich, zu Hause zurecht zu kommen (MW 8,4 auf einer Skala von 0 – 10). Die Zuversicht der Patientinnen hängt mit der Vorbereitung auf die Entlassung (durch Informationen über Nebenwirkungen bzw. Symptome, über die Weiterversorgung sowie die Nennung von Ansprechpartnern) zusammen. Ebenfalls spielen Informationen zu behördlichen und finanziellen Angelegenheiten sowie zu Hilfen und Betreuung für Zuhause eine Rolle. Auch eine angemessene Aufenthaltsdauer sowie die Unterstützung der Angehörigen sind mit einer stärkeren Zuversicht assoziiert. Zudem sind Patientinnen mit Mastektomie, gesetzlich versicherte Patientinnen und Patientinnen mit schlechterem Gesundheitszustand weniger zuversichtlich.
Zusammenfassung:
Der Beitrag liefert Hinweise für das Management und die Mitarbeiter onkologischer Zentren, wie sie ihre Patientinnen bestmöglich auf die Zeit nach der Entlassung vorbereiten können.
Objective. Through patient-physician communication, physicians can support breast cancer patients in coping with the diagnosis and treatment of their cancer. Research on the influence of hospital ...characteristics on patient-reported outcomes suggests that patient-physician communication may be shaped by the hospital environment. The aim of this study is to investigate the relationship between hospital characteristics and breast cancer patients' perceptions of the support provided by physicians. Design. Data from two cross-sectional surveys conducted in 2007 were combined and hierarchical logistic regression models were calculated. Setting. Newly diagnosed breast cancer patients treated in breast center hospitals in North Rhine-Westphalia and at least one key person from these hospitals were surveyed. Participants. Data from 3285 newly diagnosed breast cancer patients and 172 key persons from 87 breast center hospitals were used. Main Outcome Measure. The patients' perceptions of support from physicians were measured with three items from the Cologne Patient Questionnaire. Physician support was studied in relation to patient characteristics as well as hospital structure and processes. Results. The multilevel model showed that breast cancer patients perceived themselves as receiving less support from the physician when there were problems within the hospitals' organization of care, independent of patient characteristics. Conclusion. This study provides preliminary evidence that the quality of patient-physician communication depends not only on the patient or physician but also on hospital organization. Further studies should be conducted to determine the extent to which interventions for improving quality at the hospital level can modify physician communication performance.
Recent studies from Germany show that a small amount of breast and gynecological cancer patients participate in multidisciplinary tumor conferences (MTCs) at some cancer centers. One reason for the ...variation by center might be the providers' attitudes about and experiences with MTC patient participation (MTCpp), which has not been analyzed before. Therefore, it is the aim of this study to analyze the providers' expected or experienced feasibility concerning MTCpp at breast and gynecological cancer centers in Germany.
This paper presents cross-sectional qualitative interview data from the PINTU study. From April to December 2018, n=30 health-care providers from n=6 breast and gynecological cancer centers in North-Rhine-Westphalia, Germany, were interviewed. One-half of the providers had no experience and the other half had experience with MTCpp. Inductive and deductive coding was performed in order to capture the feasibility aspects of participation.
MTCpp seems not to be feasible in routine cancer care following providers' expected barriers and negative experiences. However, MTCpp seems to be feasible for selected cancer patients following providers' expected opportunities and positive experiences. Our results show that both provider groups report positive and negative experiences or expectations.
The mixed findings regarding expected or experienced feasibility of MTCpp provide first insights into differences concerning MTCpp between organizations. Our results suggest that the providers' perceptions (expectations and experiences) influence the possibility for patients to participate in an MTC in a cancer center.
This study examines drug changes from therapeutic medication given during hospitalization to that in further out-of-hospital treatment prescribed by medical practitioners of patients with acute ...coronary syndrome. The main focus of this trial was to demonstrate any change in such medication after hospital discharge.
During a six-month period a "health diary" covering health status and medication was filled in weekly by 104 patients who had been hospitalized for acute coronary syndrome in the Cologne area of Germany. The mean age of the patients was 62 years and 76 (73%) of them were men. Changes in medication between hospital discharge and further treatment were recorded. Prescribed daily doses and guideline-recommended daily doses were compared for each drug class.
Changes in medication were not observed in a population-based approach but occurred on an individual patient's level in 40% of them when only those drugs recommended in therapeutic guidelines were analysed. A fifth more lipid-lowering drugs and over a third more beta-blockers had been prescribed than recommended in the appropriate guidelines. For other groups of drugs no significant deviations from guideline recommendations were noted.
No obvious interface problem between hospital and further-treatment medication was observed in a population-based approach. However, variations in medication were found when drug prescriptions of individual patients were compared But despite these variations in individual patient the overall prescribing practice by physicians out of hospital showed good implementation of the therapeutic guidelines.
