Aim
The use of biomarkers of osteoarthritis (OA) have potential for early diagnosis, evaluation of disease severity and monitoring treatment. Serum and synovial fluid YKL‐40 levels are increased in ...severe knee OA. Pulsed electromagnetic field (PEMF) therapy is a novel treatment method for OA. However, studies evaluating the PEMF therapy in treatment of knee OA revealed conflicting results. This study was conducted to objectively assess the effect of PEMF therapy in patients with knee OA, by using ultrasonographic measurements and a novel biomarker, YKL‐40.
Methods
Forty patients were randomized into two treatment groups. Both groups received conventional physical therapy, while Group 1 received additional PEMF therapy. The patients were asked to rate their pain on a visual analogue scale (VAS) and complete a Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) questionnairre. Serum YKL‐40 levels were measured, and knee effusion and cartilage degeneration level were evaluated with ultrasonography before and after treatment.
Results
Pre‐treatment YKL‐40 level was correlated with WOMAC pain subscale (P = 0.032, r = 0.339). VAS and WOMAC scores significantly improved in both treatment groups (P < 0.05). The effusion in the right knee significantly decreased in Group 1. The change in YKL‐40 level was not correlated with the change in VAS, WOMAC scores and knee effusion.
Conclusion
This study revealed that adjuvant PEMF therapy has no additional effect on pain in patients with knee OA. Serum YKL‐40 level seems to be unuseful for monitoring the treatment in knee OA.
The purpose of this study was to evaluate the feasibility of multislice computed tomographic (MSCT) angiography as a noninvasive method for detecting ostial, proximal, and middle segment coronary ...stenosis or occlusion and anatomy in patients with transposition of the great arteries who had undergone arterial switch operation (ASO). Sixteen-detector-row MSCT angiography was performed in 16 patients treated with ASO for transposition of the great arteries. The median age was 10.3 years (range, 6.2–16.3 years). Sixteen-detector-row MSCT angiography was performed in 16 patients who had undergone ASO. CT imaging was performed in the craniocaudal direction from 2 cm above the carina up to the heart basis. Noninvasive assessment of coronary artery stenosis and anatomy were investigated by MSCT angiography. Two patients were excluded from the study because of artifacts. Of 14 evaluated patients, 1 patient had ostial stenosis (7.1%). A coronary artery anatomy variant was present in six patients: left main artery (LMA) and right coronary artery (RCA) originating from the right sinus as a single orifice (

= 2); left circumflex artery (LCX) originating from the RCA (

= 1); LMA and RCA, after branching to the LCX, originating separately from the right sinus (

= 2) was detected. Five patients were normal. In conclusion, MSCT angiography, as a noninvasive, feasible technique for assessing coronary stenosis or occlusion and anatomy, can be used in the follow-up of patients who have undergone ASO.
Mediastinal cysts are rare anomalies. The purposes of this study were to present our experience with mediastinal cysts, which were thoracoscopically treated in our clinic, and to discuss our findings ...along with those from the literature. We retrospectively investigated 13 patients who were diagnosed and thoracoscopically treated for mediastinal cysts in our clinic between January 2008 and December 2011. Seven patients were female and six were male. The average age of the patients was 41.3 ± 20.3 (7–82 years old). The mediastinal cysts comprised five pericardial cysts: four bronchogenic cysts, one hydatid cyst, one benign cystic teratoma, one thymic cyst, and one neurenteric cyst. In the case of a ruptured hydatid cyst, we passed it to thoracotomy intra-operatively due to the presence of advanced adhesion related to inflammation. Postoperative complications and mortality did not occur in any case. The average postoperative hospitalisation period was 3.8 days (2–7 days). Video-assisted thoracoscopic surgery in mediastinal cysts is a reliable and effective approach with low morbidity and a shorter hospital stay.

The purpose of this case report is to present the orthodontic, surgical and restorative treatments in the case of an operated cleft lip and palate and severe maxillary deficiency in a ...14-year-old female patient.
Background
Only orthodontic treatment could be inefficient for cleft lip and palate patients characterized with maxillary hypoplasia. Orthodontic and surgical treatment shows sufficient results, especially with severe skeletal deficiency.
Case report
A cleft lip and palate patient required complex multidisciplinary treatment to preserve health and restore esthetics. Dental leveling and alignment of the maxillary and mandibular teeth were provided before the surgery. Maxillary advancement and clockwise rotation of the maxillary-mandibular complex was applied by a Le Fort 1 osteotomy with two internal distraction devices. After the active treatment including orthodontic treatment and orthognathic surgery, upper full mouth ceramic restoration was applied.
Conclusion
This report shows the efficiency of internal distraction devices in cleft lip palate patients and exemplifies the multidisciplinary care required for such difficult cases.
Clinical significance
Stable improved occlusion and skeletal relations were observed after a follow-up examination period of 12 months.

The aim of this study was to assess the accuracy of multidetector computed tomography (MDCT), including virtual endoscopy (VE) for detection, precise localization, preoperative evaluation and staging ...of esophageal cancer (EC) by comparison with surgical and histopathological findings.
Between September 2003 and April 2005, 44 patients with histologically proven EC underwent MDCT and VE. Among 44 patients, the findings were confirmed in 24 at surgery. The accuracy of three-dimensional MDCT for detection, localization, and staging of EC was determined, and compared with surgical finding and histopathology.
The overall accuracy of three-dimensional multidetector row CT for detection of EC was 100% (24/24). MDCT staging was correct in 20 patients (83.3%). The T parameter was correctly assessed in 22 (91.7%) cases (understaged in 1 and overstaged in 1). The N parameter was correctly evaluated in 20 (83.4%) patients (understaged in 2 and overstaged in 2). The overall accuracy of VE for the morphologic classification of EC was 81.5%.
Three-dimensional MDCT, along with VE is a promising method for preoperative evaluation and staging of EC. Although accuracy in N staging remains low in comparison to PET, it provides a larger amount of diagnostic and staging information.

Verinin yapısal eşitlik modellemesi ile analiz edilmesi sonucunda gösterişçi tüketim eğiliminin benlik imaj uyumu, sosyal statü gösterimi ve tüketici materyalizmi faktörleri ile pozitif yönlü ilişkileri belirlenmiştir. Faktörler arasında aracılık ilişkileri ortaya konmuş, bireysel ve toplumsal anlamda tüketimin işlevleri üzerinde durulmuştur. Analiz sonuçlarına göre model açısından cinsiyetin anlamlı bir düzenleyici olduğu ve gösterişçi tüketim ile gelirin pozitif, yaşın negatif ilişkisi olduğu görülmüştür. Sonuçlar kuramsal açıdan yorumlanırken, gelecek çalışmalar için öneriler yapılmıştır.
「Abstract.」 Hypogonadism is a clinical condition that occurs due to infrequent abnormalities in the hypothalamic-pituitary-gonadal (HPG) axis in adolescence. Symptoms include weakening of muscle and ...bone strength. 30 young male patients with congenital hypogonadotropic hypogonadism (CHH) and 20 healthy young males were included in the present study. Quadriceps and hamstring muscle strength, balance and anaerobic performance capacities of the study group were measured both before and six months after testosterone replacement therapy (TRT). The strength of the extensor and flexor muscles of both legs showed a statistically significant increase in the isokinetic test values at 60°/sec and 180°/sec angular velocity (p < 0.05). When the parameters related to balance were investigated, a statistically significant difference was found for stability indices of left and right between pre-TRT and post-TRT (p = 0.001 for both comparisons). According to the patients' anaerobic performance measurement results, a statistically significant improvement (p < 0.001) was also found between pre-TRT and post-TRT values for each parameter. It was shown that TRT significantly increases muscle strength, balance, and anaerobic performance of patients with male CHH. As a result, we absolutely recommend the use of TRT in patients with male CHH.
